On 06/28/2023, I conducted an onsite inspection at the center. I interviewed program director, Tamika Stewart. She was working in the toddler classroom where she said only 9 children are enrolled. The only time there would be more than nine is if there was a mixed age group. She is aware of classroom ratios, and she will fill in when needed. Mrs. Stewart showed verification that she had hired two additional CCSMs on that same day with files signed with today's date. While at the center I observed ratio compliance. I also noted I conducted an unannounced interim inspection at the center on 06/06/2023 where they were in compliance with ratio throughout the center. In the infant classroom there was one CCSM with four infants ages 6 months to 14 months. In the preschool classroom they were seven children with two CCSMs. There were 12 children ages five and up with two CCSMs, two CCSMs were out to lunch during my onsite, however, the center still maintained ratio. Mrs. Stewart said the infant room services ages birth through 2 years. The toddler room services ages two though three years, and the school age classroom during the summer services children ages four and up. During the summer children 2 years through 4 years may stay in a mixed classroom setting (preschool classroom) if attendance is low for the day. She is aware that the ratio 2 during this time is one CCSM to every four children based on the age of the youngest child. On most days in the morning a mixed group will gather but by 8:30 a.m. the toddlers, ages 2 through 3 years will go to the classroom. I interviewed CCSM1. She said there are not enough CCSMs, but the center is usually in compliance. She stated that she was told that two additional CCSMs have been hired. She mentioned that one CCSM was out sick for several days in arow which made for a lot of troubleshooting during child daycare hours. I interviewed CCSM2 and CCSM3 both were aware of the ratio requirements for the ages of the children that they serve. They deny the ratio is a problem and were receptive to my consulting regarding regular attendance. I reminded all of the CCSMs that keeping call offs down to a minimum aid the center in maintaining ratio compliance. I randomly selected two weeks of preschool classroom attendance records to review. During the weeks of May 29 through June 9th no more than 7 preschoolers were present. The require ratio for that number of children requires at least one CCSM for 8 children. CCSM4 worked during the two weeks but is no longer employed at the center. CCSM5 was present during those two weeks but was not present at the center to be interviewed. The center was in ratio on the date of this inspection and during the two weeks reviewed with CCSM4 and CCSM5 caring for not more than 7 toddlers, ages 30 months of age until 3 years of age. On 07/11/2023 I held an exit conference with Mrs. Stewart over the phone. I share my recommendation of no change in the status of the license. We discussed the onboarding of her two new hires and the center ability to continue compliance with ratio with the new CCSMs being hired.
The center is out of ratio with one child care staff member (CCSM) with 12 children, ages 2 to 3 years. The ratio should be one CCSM to '/2 eight child care children.

On Monday, 10/24/2022, I conducted an unannounced inspection at the facility to investigate the allegations. I interviewed program director, Cora McSwain. I asked her if the center had a recent COVID outbreak. Ms. Cora said on Friday, 10/07/2022, she received a call from Child A's (10-month-old male) Mother that her child tested positive for COVID. Ms. Cora notified the parents on Saturday, 10/08/2022 of the positive COVID case on Bright Wheel. Infant child care staff member, Cierra Norwood-Strong came in on Monday, 10/10/2022 and sanitized the infant classroom. The infants who attended the facility on Monday, were place in the toddler classroom. Ms. Cora said the two infants were out and could not return until they got a negative COVID test result. Ms. Cora said on Tuesday, 10/11/2022, Child B's (15-month-old female) Mother was asked to pick her up from the center because she was sick with COVID-like symptoms. Child B's Mother said she would take Child B to the doctor. Later that evening, Child B's Mother called the center and told them that Child B tested positive for COVID. Ms. Cora sent another message on Bright Wheel on 10/11/2022 to the parents, notifying them that another infant tested positive for COVID. They made the decision to shut the infant classroom down. Ms. Cora showed me both messages from 10/08/2022 and 10/11/2022 on the Bright Wheel App that were sent out to parents notifying them of the positive COVID cases. While on site, I interviewed child care staff members, Tiffany Mailey, Jayla Mailey, Schyla Hodges and Brenda Mailey. I asked each of them individually if they were aware that the center had two COVID cases and if they had knowledge of whether the parents were notified of the two cases. Each of them individually said they were aware of the two COVID cases. Each of them individually said they were aware that the center notified the parents on the Bright Wheel App as well as at pick up time. Child care staff member Stephanie Calloway said she just started last week on 10/17/2022, so she was not aware. I called three parents whose children attend the center. I called Child C's (4-year-old male) Mother to ask if she was notified of the COVID cases. Child C's Mother said she received notification on Bright Wheel on 10/08 and 10/11 that there were COVID cases in the center. She said when she picked up her son on Tuesday, 10/11/2022, the staff was at the door telling the parents as well. Child C's Mother said the Bright Wheel notification also said they would clean and sanitize the infant room. I called Child D's (4-year-old male) Mother. I asked her if she had been notified that there were two COVID cases at the center. She said yes, she was notified on Bright 3 Wheel. Child D's Mother said she received two notifications: one on 10/08/2022 and the other on 10/11/2022. I called Child E's (8-month-old male) Mother. I asked her if she had been notified that there were two COVID cases in her child's classroom. She said yes. She said she was notified on Bright Wheel, and she received a telephone call. Child E's Mother said the center shut down the infant classroom. She had to take her son to the toddler classroom on Wednesday, 10/12/2022 where his teacher, Ms. Cierra kept him. On 10/25/2022, I contacted the complainant. The complainant has two children who have attended the facility since 04/01/2022. She said she is upset that the center always asks her to pick up her children when they are sick. She said the center called her about green snot coming out of her child's nose about a week ago. The complainant said, "all kids have runny noses." The complainant said most times her children are not sick in the mornings before she drops them off. She said she feels the center calls the parents too much to pick up their children. I asked the complainant if she was informed of the two COVID cases. She said yes but when her children had COVID, the center did not inform the other parents that her children had COVID. I asked
