  On 11/13/2025 a completed child information card was on file at the child care home. Ms. Cook only called the emergency contacts provided by Child A's Mother on the child information card. Both Child A's Mother and Ms. Cook did confirm that Child A's Mother made a verbal request that Child A's Grandmother not be called. However, the physical child information card was never updated to reflect this request.

  Based on interviews of staff and Child A's Grandfather, there is no evidence that Child A was verbally threatened by Ms. Cook. Ms. Cook did ensure appropriate care and supervision of Child A was provided and Child A was unaware of care being terminated.
