On 05/15/2023 an incident report was received from Rogers City ECC. On 05/11/2023 child care staff members were teaching children how to use the elementary school playground slide for Kindergarten readiness. Child A (male, 5 years) did not get off the slide before another child came down the slide. Child A was crying. At lunch Child A took off his hooded sweatshirt and said he hurt his wrist. A red mark was observed on his wrist; Child A was given a cold carton of milk to put on his wrist. Child A took a nap and did not mention his wrist again. On 05/15/2023 Child A's father stated to program director Joan Lehman that Child A was hurt at school last week and was taken to the emergency room on 05/14/2023. On 05/15/2023 the hospital informed Child A's father that Child A had a broken arm at the elbow area. On 05/17/2023 I interviewed Child A's father via telephone. Child A's mother picked him up from Rogers City ECE on 05/11/2023; Child A was holding his arm when she 2 picked Child A up. Child A reported that he was at the bottom of the slide when another child came down before he could get off the slide. On 05/12/2023 they took Child A for x-rays because he was crying and wouldn't move his arm. Alpena radiology called on 05/15/2023 and stated that Child A had a hairline fracture. They were never contacted by Rogers City ECE about the incident on 05/11/2023. He has no concerns with the care and supervision that Child A receives at Rogers City ECE. On 05/23/203 I completed an on-site inspection at Rogers City ECE. I reviewed the original licensing study report dated 08/16/2012 and the addendum to the original licensing study report dated 07/10/2019. There was no approved playground equipment for Rogers City ECE. I observed the slide on the elementary school playground that was used by the child care children on 05/11/2023. I interviewed child care staff member Angie Mulka. On 05/11/2023 three child care staff members were supervising 14 children ranging in age from 4 years to 5 years of age going up and down the slide; preparing the children for kindergarten. She was supervising the children not using the slide; and did not see the incident occur. After recess Child A was taking his sweatshirt off and said "ow". There was a red mark on his arm and he was given a cold carton of milk to put on it. Child A did not complain about it again that day. Thursday 05/11/2023 was the first time the children had been allowed to use the elementary school playground slide. She has no concerns with the care and supervision children receive at Rogers City ECE. I interviewed child care staff member Shaylee Zampich. On 05/11/2023 there were three child care staff members outside with 14 children ranging in age from 4 years to 5 years of age. It was the first time the children had been allowed to use the slide. She was supervising the bottom of the slide. Child A was sitting at the bottom of the slide waiting for his friends and she asked Child A to move. Two children came down the slide, bumped into Child A and pushed him off the slide. Child A said his arm hurt. After recess the children came inside and Child A said his arm hurt and he was given a cold carton of milk to put on it. The rest of the day Child A had no complaints about his arm. I interviewed child care staff member Andrea Wagner. She was not present on 05/11/2023. She has never observed the children using any playground equipment. She has no concerns with the care and supervision children receive at Rogers City ECE. On 05/24/2023 I interviewed program director Joan Lehman via telephone. On 05/11/2023 she was helping the 14 children ranging in age from 4 years to 5 years of age up the ladder and Ms. Zampich was supervising at the bottom of the slide. This was the first time the children has used the elementary school playground slide as part of kindergarten transition. Child A was angry and told her that they pushed him off the slide. (11) The playground equipment, use zones, and surfacing in the outdoor play area must be inspected by a certified playground safety inspector and an approval granted for playground equipment and areas used before issuance of an original license, upon request of the department, and before using any newly added playground equipment. The center shall provide documentation of the inspection to the department upon request and shall keep it on file at the center.
Licensee designee Brenda Bradbury failed to ensure the outdoor play area was inspected by a certified playground safety inspector prior to child use. On 05/11/2023 child care children were allowed to use a slide located on the Rogers City Elementary school playground without a playground inspection by a certified playground inspector.
