On 10/19/2021 I conducted an on-site inspection. I interviewed program director and lead caregiver, Bonnie Shabinaw. Child A is in her GSRP classroom where 18 children are enrolled. She said on 10/12/2021 she was present with two other child care staff members, Sue Mathes and Melissa Miller, when Child A fell on the playground off the playscape. No child care staff member saw exactly how she fell but responded with first aid when she Child A cried out. Child A hurt her right foot and ice was applied to her head. Mrs. Shabinaw called Child A's Mother and Child A was picked up at 3:15 p.m. The following day the center was told that Child A's right foot was fractured and that she had a mild concussion. Mrs. Shabinaw notified her licensing consultant, Selika Johnson, verbally, and in writing as required. She denied having a history of inappropriate supervision or care. The center does have cameras throughout including on the playground. Unfortunately, the angle of the camera does not capture the piece of play equipment where the injury occurred. I consulted and recommended that the camera be adjusted to capture the entire playground. All child care staff members are fingerprinted with 2 child abuse and neglect statements on file. They all also have current CPR/First Aid training on file and provided appropriate aid when the child was injured. While at the center I interviewed child care staff members, Sue Mathes and Melissa Miller. Both describe being attentive on the day of the incident and described the incident as an accidental fall. Neither child care staff member saw exactly how she fell but responded with first aid when she Child A cried out. Neither child care staff member had concerns about how the incident occurred nor inappropriate supervision. On 11/11/2021, I interviewed Child B's Mother and Child C's Mother over the phone. Neither parent had concerns about the child care services that they are receiving. Child C's Mother spoke highly of the child care staff members especially Mrs. Shabinaw. She said Child C really likes Mrs. Shabinaw and talks about her on the weekends. She looks forward to going to school every day. On 11/16/2021, I interviewed Child A's Mother over the phone. Child A has recovered without incident and continues to attend the center. She was notified by Mrs. Shabinaw and had Child A's grandfather to pick her up from school. She took her to emergency room later that day and was told of the injuries. She has no concerns about how the injury occurred nor inappropriate care or supervision. Child A's mother said it was an unfortunate accident.
On 10/12/2021 Child A (4-year-old female) fell on the playground and fractured her foot.
