On 12/03/2022, 1 spoke with Child A's Mother via telephone about the allegation. Child A's Mother reported that Child A is in the Movers and Grovers classroom. She reported that CCSM 1 is typically Child A's primary caregiver. She has noticed that Child A was not getting his diaper changed enough and talked to the toddler lead caregiver CCSM 2 and the assistant director CCSM 4 about her concern. Child A's Mother said that they both told her that Child A was getting changed but CCSM 1 was not documenting it in the Bright Wheel application that the center uses to document. Child A's Mother said that today Child A only had diaper changes recorded at 9:45 AM and 3:45 PM but after she spoke with the CCSMs, a diaper change at 2:45 PM was added. Child A's Mother said that she provides the diapers for Child A but she gives the center a large package of diapers, so she is not noticing that they are not using enough diapers. Child A's Mother said that each time Child A is not being changed enough, CCSM 1 is his caregiver. Child A's Mother sent screenshots of the Bright Wheel documentation via text message. 2 The screenshots showed that diaper changes for today occurred at 7:47 AM, 9:45 AM, 11:45 AM, 2:45 PM and 3:45 PM. Screenshots for 11/28/2022 showed diaper changes at 9:46 AM and 3:05 PM. I asked Child A's Mother if the 11:45 AM diaper changed had been added later too and she confirmed that it had. On 12/02/2022, I completed an unannounced on-site inspection and interviewed CCSM 1 about the allegations. CCSM 1 reported that she regularly takes Child A to the bathroom as he is currently toilet training. She admitted that she has not been great at documenting in the Bright Wheel application and has missed documenting some diaper changes. She said that she knows of two times that it happened. CCSM 1 explained that after Child A's Mother addressed the issue, two of her coworkers messaged her about it and she added the diaper change when she was at home. The typical diaper routine for the classroom is diapering at 9:30 AM, 11:30 AM, 2:30 PM and 4:30 PM or when children have a soiled diaper. She works 8:15 AM to 3:00 PM and often works by herself with three or four children. She does not think she is very good with technology and her focus is on caregiving since she is working alone, which has led to her missing some of her documentation responsibilities. I interviewed CCSM 2 who said that at pick up on 11/30/2022, Child A's Mother had looked for Child A in her classroom and then made a comment to her about being frustrated that his diaper not being changed. CCSM 2 told Child A's Mother that she knows that Child A is being changed because she has seen him in the bathroom because her classroom's bathroom time is after Child A's classroom's time. CCSM 2 explained that CCSM 1 struggles with putting them in Bright Wheel because she does not like to be on her phone when she is caring for the children. CCSM 2 also spoke with CCSM 3 about the situation because CCSM 3 message CCSM 1 to update the diaper change times in Bright Wheel. CCSM 2 said that she feels bad for CCSM 1 because she is a "really good caregiver" and is "confident that he is being checked and changed regularly." I interviewed CCSM 3 who recalled talking to Child A's Mother a few months ago about Child A not being changed after nap time but she reassured Child A's Mother that Child A had been changed, because everyone gets changed after nap time. She said that she told Child A's Mother that it just had not been documented in the Bright Wheel application. I spoke with program director Katherine Kwiatkowski who had talked to Child A's Mother about this in the summer when someone forgot to document a diaper change. Child A's Mother did not speak with her about this week's concerns. Child A's Mother did speak with CCSM 4. Ms. Kwiatkowski explained that she is confident that Child A's diaper is being changed regularly and that diapering and toileting is occurring every two hours. She s
