(1) At the time of a child’s initial attendance, a center shall obtain a child information card, using a form provided by the department or a comparable substitute, that is completed, signed and dated by the child’s parent and updated as changes occur. The center shall keep current hard copies of the child information cards at the center.

Ten child information cards were reviewed. Two cards were missing the child’s date of birth, four were missing employee name and/or number, two were missing the physician’s phone number, and one was missing the center’s name for medical treatment.

Child care staff members did not provide appropriate care and supervision to Child A on 3/10/2025. Child A fell onto a rock injuring her arm. She complained multiple times throughout the day that her elbow continued to hurt. Child care staff members did not attempt to contact Child A’s Mother to notify her of the injury, even though parental contact is part of their injury policy.
