    On 11/04/2022, I received a written complaint that alleges infants in the Infant A classroom can be heard screaming and crying all day long. The lead caregiver in Infant A, CCSM 1, has told staff to let children cry because they need to learn to self-soothe. On 11/15/2022, I completed an on-site inspection. I interviewed program director Flonnie "Sue" Buway, CCSM 1, and CCSM 2. Ms. Buway stated that the allegation is not true. She stated that the infants cry no more than five to 10 minutes. She advised that the center was permanently closing on 12/16/2022. 2 CCSM 1 stated that the infants in her classroom cry 10 minutes at the most unless she is alone and feeding another child. She stated that some of the children will not finish their bottle if the feeding is interrupted. CCSM 2 stated that she relieves staff for breaks. She has no knowledge of children in any room crying for an extended period of time.

    On 11/04/2022, I received a written complaint alleging that CCSM 1 did not clean the sink she used to rinse a child after they had a bowel movement. Additionally, CCSM 1's classroom has dried spit up on the play mat. On 11/15/2022, I completed an on-site inspection. I interviewed program director Flonnie "Sue" Buway, CCSM 1, and CCSM 2. Ms. Buway stated CCSM 1 rinsed a child off in the sink because they had a "blow out" on their way to the center, but the sink was immediately cleaned and sanitized. She stated that the play mats and other soft objects are laundered daily. CCSM 1 stated that she cleaned and sanitized the sink after she rinsed a child who had an explosive bowel movement on their way to the center. She stated that she cleans spit up on the floor with soapy water and a disinfectant and soft object, including play mats, are laundered daily. CCSM 2 stated that CCSM 1 cleaned and sanitized the sink immediately after she rinsed a child who had an explosive bowel movement on their way to the center. She stated that spit up is cleaned up right away and soft objects are laundered daily. 3

    On 11/04/2022, I received a written complaint alleging that CCSM 1 ignores children not on her primary care assignment, letting them remain in a soiled diaper. On 11/15/2022, I completed an on-site inspection. I interviewed program director Flonnie "Sue" Buway, CCSM 1, and CCSM 2. Ms. Buway stated that a former staff member who was recently fired previously reported to her that CCSM 1 was allowing children not on her primary care assignment to sit in soiled diapers. She talked with CCSM 1 about the allegation. CCSM 1 denied the allegation. CCSM 1 adamantly denied the allegation. She stated that she and CCSM 2 are a good team, and they routinely assist with each other's primary care assignments when necessary. She stated that she has never let a child sit in a soiled diaper. CCSM 2 stated that the allegation is not true. She stated that she and CCSM 1 are a good team and CCSM 1 takes care of what needs to be done in the classroom, including changing the diapers of her primary care assignments when she is busy caring for another child.

    On 11/15/2022, I completed an on-site inspection. I interviewed program director Flonnie "Sue" Buway, CCSM 1, and CCSM 2. Ms. Buway stated that the only concern that was brought to her attention was that CCSM 1 would not change soiled diapers of children not on her primary care assignment, which she did address with CCSM 1. She denied receiving any other concerns by staff or parents about CCSM 1. CCSM 1 stated that she has no knowledge of staff or parents having concerns with her. CCSM 2 stated that she has no knowledge of staff or parents having any concern with CCSM 1. She stated that Ms. Buway promptly addresses concerns that are brought to her attention.
