On 02/03/2023, I spoke with the complainant who reported that she does not think that CCSM 1 is physically unfit and that it impacts her ability to take care of the children. She heard from CCSM 2 that Child A was crying because CCSM 1 shoved food so far in her mouth that she gagged and almost choked and commented "maybe that will shut you up." The complainant reported that CCSM 1 hit Child B on the head yesterday with a toy so hard that his head hit the ground. CCSM 2 reported the incident to program director Amy Edgar. This occurred in a spot where the camera does not reach. I asked the complainant why CCSM 1 only changed the diapers of male infants, but they said that they did not know. The complainant abruptly ended the call, so I was unable to obtain any additional information. On 02/03/2023, I completed an unannounced on-site inspection and interviewed program director Amy Edgar who reported that they had an employee who was bragging about reporting the center and specific employees and that there was a lot of drama among the staff. Ms. Edgar reported that Child A is only 4 months old and just started eating solids at her parents' request. Child A was served a kale/raspberry food and she cries and gags when they feed it to her. Ms. Edgar was aware of the situation with Child B, and he was banging a toy on the nap room window, so CCSM 1 took the toy away from him and he lost his balance, sat down and began crying. CCSM 5 was working in the classroom at the time and told her about it. CCSM 5 sent her a text message about it; I took a picture of the text message. The message stated, "Hey I just wanted to let you know that today [CCSM1] ripped a toy out of [Child B's] hand and it made him smack his head off the ground and she laughed and continued to rock [Child D] like it didn't happen. But I said something, and she got mad at me so idk what to do about that". Ms. Edgar said that CCSM 1 is physically capable of caring for the children and was able to evacuate the children during a drill. I interviewed CCSM 5 who reported that she is the lead caregiver in the infant classroom. She observed CCSM 1 feeding Child A on 01/31/2023. CCSM 1 was feeding a puree to Child A that her parents provided. Child A was crying, and her parents had reported to the CCSMs that she was getting used to sitting in a high chair. She thinks that CCSM 1 pushed the spoon so far back that Child A gagged. She thinks that four to five inches of the spoon were in Child A's mouth. She saw CCSM 1 do it two more times and then she offered to take over for CCSM 1. CCSM 5 said that CCSM 1 yelled at her to walk away and to take care of the other children. CCSM 5 said that CCSM 1 "ripped" the toy out of Child B's hand which caused him to hit the ground "pretty hard." She was changing a diaper at the time and checked on him after she finished. He was smiling and blowing bubbles. CCSM 1 was sitting in a rocking chair that is off camera and leaned over to grab the toy from him and 3 laughed when Child B fell over. He was sitting and banging on the nap window, she told him to stop, he did it again and she got mad. I asked CCSM 5 if she had told any of her coworkers that Child B hit his head, CCSM 5 admitted that she had because she thought that he might have. She confirmed that she did not see him hit his head. Her back was to the classroom at the time because she was changing a diaper. I asked CCSM 5 about CCSM 1 only changing boys and she said that CCSM 1 says that she doesn't like to change girls. CCSM 5 said that CCSM 1 sits in the chair a lot doing nothing. CCSM 5 has reported this to Ms. Edgar and is always told that she will look into it. CCSM 1 refuses to take care of Child C and if CCSM 5 asks her to feed him while she is feeding a child, CCSM 1 will tell her no. CCSM 5 reported that she had a meeting with Ms. Edgar on 01/19/2023, and Ms. Edgar told her that she was probably afraid and laughed. The chair that CCSM 1 sits in is off camera and that is where sh

On 02/02/03/2023, I asked the complainant for more information about this allegation. She reported that CCSM 3 told a child that was crying to stop yelling at her, and to stop pulling her hair. The complainant abruptly ended the call, so I was unable to obtain any additional information. Ms. Edgar, CCSM 5, CCSM 1, CCSM 2, and CCSM 4 had no information regarding this allegation. I spoke with CCSM 3 who denied the allegations. I asked why she thought someone would make allegations against her. She said that a former coworker caused some drama by lying and was manipulating staff to be against each other. She said that she felt the person had some personal issues going and was not sure of the motivation behind the behavior.

The complainant abruptly ended the call, so I was unable to obtain any additional information. Ms. Edgar had no information regarding this allegation. CCSM 5 has not heard CCSM 1, CCSM 2 or CCSM 3 call the children crybabies. She has heard CCSM 3 call the children "monsters" and the others say the children are naughty. CCSM 5 said that CCSM 3 has called the children "little monsters" in a joking way and out of "pure anger." CCSMs talk to each other about the children being naughty. CCSM 1 had no information about this allegation. She does not call the children names. CCSM 2 said that no one calls the children crybabies. CCSM 4 had no information about this allegation. I spoke with CCSM 3 who denied the allegations.
