On 1110112021 the complainant stated that Child A had bruising on the shaft and tip of his penis and the foreskin was forcibly retracted. 2 On 11/01/2021 Child A's Mother stated that on 10/28/2021 after she picked Child A up from the center and they were home, he started crying and pulling at his diaper. When she changed his diaper, his penis was purple and inflamed and appeared to have a rash. This was not the way that Child A's penis looked in the morning when she changed his diaper before she dropped him off at the center. Child A's Mother called the on-call nurse who instructed her to apply Vaseline or Aquaphor to the affected area, and if it did not improve Child A should be seen by his doctor. On 10/29/2021 Child A was seen by the doctor where it was determined that Child A had bruising on the tip of his penis and the foreskin of his penis had been pushed back exposing raw skin. The doctor determined that the injury could not occur accidently; it was done on purpose. Child A's Mother stated that she was not provided with any information about Child A's primary caregiver. She saw the posting on the door where she drops off Child A and that was the only way she knew that Ms. Megan Byrd was Child A's primary caregiver. She has only spoken with Ms. Leah Dykstra about Child A's care at the center. On 11/01/2021 Child A's Father emailed the report from the doctor. The report stated bruising on shaft and tip of penis, and retracted foreskin. The report is on file with the department. On 11/03/2021 Ms. John stated that she was not aware that Child A was injured at the center. She checked the centers app that is used in the classrooms, and she stated that Child A's diaper was changed last on 10/28/2021 at 4:09 pm by Ms. Dykstra. Child A's Mother picked him up from the center at 5:19 pm. Ms. John read the information from the centers app. Ms. Dykstra changed Child A's diaper at 4:09 pm and he had a bowel movement with fecal matter that was in the front and back of the diaper. She pulled the skin under the tip back from the penis to clean the fecal matter out of the area because there was a lot of fecal matter on Child A's penis and scrotum. After cleaning Child A's penis and scrotum, the area was red. It was not from a rash; she believed that the area was red because of the fecal matter that she cleaned off. Child A was crying during the diaper change but stopped crying once she was finished cleaning the area and put on a clean diaper. She was not trained on how to clean a child when the child has an excessive bowel movement. For normal bowel movements she will clean the child from front to back. On 10/29/2021 Child A's Mother sent Ms. Dykstra a message on the center's app informing her that Child A had a rash, and she was taking him to the doctor. Later she messaged Child A's Mother to follow-up, and she stated that she would be keeping Child A at home. Ms. Dykstra stated that Ms. Byrd was Child A's primary caregiver, but they all care for the children and she, as the lead caregiver, is the main person that communicates with the parents. The child care staff members that work in the Infant classroom work together for feeding, diapering, and caring for the 3 children. Ms. Dykstra stated that it is hard to have primary caregivers working with the infants, which is why they all share the responsibilities. Ms. Baker is a floater and she worked in the infant classroom on 10128/2021 and she changed Child A's diaper once at 1:30 pm and he was wet. She did not observe any redness on his penis or scrotum areas. She cleaned the area without pulling on Child A's penis or scrotum. Ms. Byrd worked in the infant classroom on 10/2812021 and changed Child A's diaper at 10:00 am and he was wet. She did not observe any redness on his penis or scrotum areas. MCL 722.115q Child care center, group child care home, or family child care home; contact with child by licensee, child care staff member, or household adult member prohibited; conditions. (1) Except as provided in section 5n(8), a licensee, child care staff member, or adult member of the household may not have contact with a child who is in the care of a child care center, group child care home, or family child care home, until the department obtains documentation from the department of health and human services that he or she has not been named in a central registry case as the perpetrator of child abuse or child neglect. Upon request by the department, the licensee, child care staff member, or adult member of the household shall provide the department with an updated authorization for a central registry clearance. If a central registry clearance documents that a licensee, child care staff member, or adult member of the household is named in a central registry case as a perpetrator of child abuse or child neglect, he or she is ineligible to receive a license to operate a child care center, group child care home, or family child care home, be an adult member of the household, or be a child care staff member.
