On 11/06/2025 Lead Teacher, Teacher 1, and Teacher 2 did not provide appropriate supervision of Child D when the class went outside and they left Child D in the classroom for approximately 30 minutes.

On 10/14/2021 I received and reviewed an incident report (form BCAL 4605) from PreK supervisor Laura Brown. The report documents on 10/13/2021 during clean up time, Child A was running around, throwing toys, and knocking over furniture. Staff 1 tried to calm Child A down. Child A bit Staff 1's arm leaving a mark on Staff 1's arm. Staff 1 yelled "no" and Staff 2 saw Staff 1 slap Child A on the cheek. Staff 2 told Staff 1 to walk away. On 10/14/2021 I spoke with Ms. Brown regarding the incident. She stated she had contacted the Michigan Department of Health and Human Services (MDHHS) in regard to this incident. On 10/18/2021 I completed an on-site inspection to the classroom. was also present at the school, and we "iscussedthe allegation. I interviewed to 2 about the allegation. She stated Child A has tried to bite her before and Child A had previously bit other staff members before this incident. Staff 2 stated Child A has exhibited some severe behavior problems the past two weeks. Child A hasl knocked furniture over, thrown toys, hit children and staff, and screamed. Staff 2 stated on 10/13/2021 around 11:00am, the children were cleaning up the toys. Child A started to knock over furniture and throw toys. Child A was trying to pull down a bookshelf when Staff 1 grabbed Child A's arm. Child A then bit Staff 1's forearm. Staff 2 stated she heard Staff 1 scream "ow" and saw Staff 1 "quickly smack" Child A's face. I asked Staff 2 if Child A reacted to Staff 1 smacking her face. Staff 2 stated Child A's "behavior didn't change". Staff 2 stated when this incident happened, she was standing in another area of the classroom, and she immediately walked over to Child A and Staff 1 and said to Staff 1, "Let me have (Child A)". Staff 1 then walked away from Child A. I asked Staff 2 if she observed any mark or injury to Child A's face. Staff 2 stated she did not see any mark or injury on Child A. I asked Staff 2 if she later spoke with Staff 1 about what happened, and Staff 2 stated she did not speak with Staff 1 about the incident. I asked Staff 2 if she had ever seen Staff 1 use physical punishment with a child before this incident. Staff 2 stated she 3 had never observed Staff 1 use any physical punishment or any form of prohibited punishment with any child. On 10/18/2021 I spoke with Laura Brown. She stated Staff 1 has repeatedly denied she slapped Child A. On 10/19/2021 I interviewed Staff 1 about the allegation. Staff 1 stated Child A had bit another staff member previously and scratched other staff members before this incident. Staff 1 did not know the name of the other staff member that had been bit by Child A. Staff 1 stated it is "okay we ignore it because (Child A) is a kid and too young, if she messes up it is okay". Staff 1 stated on 10/13/2021 she was working with Staff 2, and that morning Child A was "throwing everything and hitting kids", so she (Staff 1) took Child A outside. Staff 1 stated when she was outside with Child A, Child A twice ran away from her and was not listening. Staff 1 stated while they were outside, she told Child A she should not run away because she might fall and get hurt. Staff 1 stated when she and Child A came back inside the classroom, she and Child A sat together and played with toys. Staff 1 stated after this she "doesn't know what happened, (Child A) was out of control" and Child A bit her hand. Staff 1 stated, "Somebody said I hit her (Child A); I don't understand because I did not hit her". Staff 1 stated "this is unfair, I didn't hit her". I asked Staff 1 if Child A was trying to pull down the bookshelf before Child A bit her. Staff 1 stated, "No." Staff 1 stated she was helping another child put a sheet on a cot when Child A came over and bit her. I asked Staff 1 if Staff 2 saw this happen. Staff 1 stated, "(Staff 2) was maybe watching". I asked Staff 1 if she said anything after Child A bit her.
