    On 11/09/2022 program director Kristyn Spence called and emailed Licensing an incident report regarding the allegation. On 11/10/2022 I spoke with Child F's Father. He stated on 11/09/2022 Child F started crying at drop off when they got to the preschool classroom door. Child F's Father stated this was very uncharacteristic for Child F. Child F's Father stated there were a few children in the classroom with a staff, whom he had not met before. He was comforting Child F when the staff came over and grabbed Child F's arm "pretty tight". He gave Child F to the staff and then stood inside the classroom by the door to see if Child F would calm down. The staff was still holding Child F and asked Child F if she wanted to give him one more hug. The staff walked over to him carrying Child F and still holding Child F's arm "really tight". He gave Child F a hug and left. Child F's Father stated he did not say anything to the staff about how tight she was holding Child F and wished he would have done this at the time. He called 2 the center about an hour later and spoke with the director about it. Ms. Spence stated she would go and check on Child F and call him back, which she did. Ms. Spence told him the lead caregiver Vanessa Sanchez was now in the classroom and Child F was doing fine. Child F's Father stated the director and lead caregiver sent him updates on Child F throughout the day, and Child F seemed to be doing okay. Child F's Father stated Child F did not have any mark or injury on her arm. On 11/16/2022 I completed an on-site inspection and interviewed program director Kristyn Spence. She stated the morning of 11/09/2022 staff Ms. Worood Altaee opened the Early Preschool room and was the only staff in the room when Child F arrived. Ms. Spence stated Ms. Altaee has worked at the center since June 2021. She works as a floater and there have been no prior concerns about her. Ms. Altaee has cared for Child F in the past when Child F was in the Get Set room. Ms. Spence stated she has observed Ms. Altaee to be gentle with the children so she was shocked when Child F's Father stated she grabbed Child F by the arm. Ms. Spence stated after Child F's Father contacted her, she spoke with Ms. Altaee who denied grabbing Child F by the arm. Ms. Altaee said she picked Child F up under the arms and held Child F. Ms. Altaee said Child F was upset so she squatted down and put Child F down so Child F could go back to Child F's Father. Ms. Altaee said she did nothing that would have hurt Child F. I interviewed staff Worood Altaee. She stated she works as a floater Monday through Friday from 8:00am to 2:00pm and has worked at the center for the past year and a half. On 11/09/2022 she opened the Early Preschool room when Child F was dropped off. Child F usually does not cry, but that day Child F was crying. Child F's Father was holding Child F and she came over to them and took Child F. Ms. Altaee stated she was holding Child F under the arms when she took Child F. Ms. Altaee stated she hugged Child F and told Child F, "It's okay". Child F looked over to Child F's Father and she asked Child F if she wanted to give him a hug, so she put Child F down and Child F went over to Child F's Father. Ms. Altaee stated she did not grab Child F by the arm or pull Child F away from Child F's Father. Ms. Altaee stated after Child F's Father left Child F was fine and no longer crying. Ms. Altaee stated she loves the children and every family and did not do anything wrong.

    On 10/11/2022 program director Kristyn Spence contacted Licensing and emailed an incident report regarding the complaint allegation. On 10/17/2022 I spoke with Child E's Mother and Child E's Father. They stated they were contacted on 10/10/2022 by the center staff right after the incident happened and the next day, 10/11/2022 they met in person with designee Sandeep Chada. Mr. Chada showed them video of the incident. Child E's Mother stated Child E does not walk yet so staff put Child E in a stroller when they go out to and leave from the playground. Child E's Mother stated staff should have realized Child E was not in the stroller when they were leaving the playground. Child E's Mother and Child E's Father stated they don't know how the two staff forgot Child E as there were only 6 or 7 children with them. Child E's Mother and Child E's Father stated they are satisfied with how Mr. Chada has addressed the concern to prevent it from happening again. On 10/17/2022 I completed an on-site inspection to the center. I interviewed Mr. Chada. He was not at the center when the incident happened. Mr. Chada stated Child E is in the First Steps I class, and staff Shanna Stahlberger and Lily Kowalski were the staff outside on the toddler playground with the children when the incident happened. The First Steps I and First Steps II children were outside on the playground for about 45 minutes with staff Ms. Stahlberger, Ms. Kowalski, and Ms. Fabiana Santana De Faria. A couple of the children went home, so Ms. De Fabria had come inside to close her First Step II classroom. Ms. Stahlberger and Ms. Kowalski brought the children inside; however, they did not see Child E sitting behind the play structure, and they did not do the name to face count. The Young Toddler classroom then came outside to the toddler playground and staff Angela Knapp saw Child E and brought Child E inside to the office. Ms. Stahlberger and Ms. Kowalski did not know Child E had been left outside until Child E was brought inside. Mr. Chada spoke on the phone with Child E's Mother and Child E's Father on 10/10/2022 and met with them the next day. Mr. Chada stated Ms. Stahlberger is no longer working at the center and Ms. Kowalski has been retrained. I interviewed staff Angela Knapp, the lead caregiver for the Young Toddler class. Ms. Knapp stated Ms. Kowalski often works as an assistant staff member in the Young Toddler class. Ms. Knapp trained Ms. Kowalski on how to do name to face counts of the children and Ms. Kowalski has done them in the Young Toddler class. Ms. Knapp stated on 10/10/2022 around 4:30pm she and assistant caregiver Anna Murray took the Young Toddler class outside, and when they walked through the 3 gate on to the playground, they saw Child E alone. Child E seemed okay and not upset. Ms. Knapp immediately picked up Child E and brought him inside to the office. I interviewed staff Lily Kowalski. Ms. Kowalski stated she has worked as an assistant staff member at the center since August 2022. Ms. Kowalski stated she usually works in the Toddler classroom, but occasionally helps in other classrooms. On 10/10/2022 at 3:00pm she went to help in the First Step I classroom, as the lead caregiver was leaving for the day. Ms. Kowalski stated when she arrived in the classroom the staff were getting the six children ready to go outside. She and Ms. Stahlberger took the children out to the playground. Ms. Kowalski stated four of the six toddlers were in a stroller with Ms. Stahlberger, and she walked with the other two toddlers. I asked Ms. Kowalski if she or Ms. Stahlberger did a name to face count when they arrived at the playground. Ms. Kowalski did not remember if they did. Ms. Kowaski stated Ms. Stahlberger had the classroom tablet with her and usually the staff that has the tablet does the name to face count. Ms. Kowalski stated two or three First Step II children were also on the toddler playground with their staff member Ms. Fabiana Santana De Faria. A few chil

    On 05/04/2022 program director Kristyn Spence left a voicemail message stating a child care staff member had left a child unattended outside for an estimated 2 to 3 minutes. Ms. Spence sent an incident report regarding this. The report documents on 05/04/2022 at 11:20am Child F (1 year old) "got stuck/left between the fenced in area" while the classroom transitioned from the classroom to playground. Another staff member noticed Child F and informed Ms. Spence who ran and got Child F. Ms. Spence then informed Child F's staff members what happened. On 05/06/2022 I completed an on-site inspection to the center and interviewed Ms. Spence. She stated on 05/04/2022 the Young Toddler class was transitioning from their classroom to the playground. Young toddler staff Hayeong Bae and Areej Alnaser took the children from the classroom out to the playground. Neither Ms. Bae nor Ms. Alnaser realized Child F was not with the group when they got on the playground. After the incident Ms. Spence spoke with both Ms. Bae and Ms. Alnaser. Ms. Alnaser said she thought Ms. Bae counted the children. Ms. Bae said she counted the children when they left the classroom, and she did not know Child F was not on the playground with them. Ms. Spence showed me the area where Child F was left unattended. This area is on the north side of the building; it has a cement patio with a border of grass along the fence line. This entire area is enclosed by a 6-foot fence. There are several classrooms, including the Young Toddler classroom, with exit doors that open to this area. There are gates on both ends of this area; the west gate opens out to a sidewalk that leads to the parking lot of the center, and the east gate opens to the playground. Ms. Spence stated Child F would not be able to reach the latch to open this gate and the gate also has an alarm that will sound in the office if it is opened. Ms. Spence stated staff Vanessa Sanchez was in the Older Toddler classroom at the time of the incident and saw through a window that Child F was alone in the area. Ms. Sanchez immediately came and told her (Ms. Spence), and Ms. Spence ran out to the patio and retrieved Child F. Ms. Spence stated staff are supposed to use name to face counting of the children when they line up to leave the classroom, when they are out in the enclosed area, and when they go through the east gate and arrive on the playground. Ms. Spence stated since this incident she has reviewed the name to face counting procedures with all staff, and she is monitoring all the classrooms to ensure that staff are doing the name to face counting as expected. I interviewed Older Toddler staff Vanessa Sanchez. She stated on 05/04/2022 at the time of the incident she had just arrived in the Older Toddler classroom to start her 3 shift. She looked out her classroom window and saw the Young Toddler class going outside. She saw the Young Toddler staff Ms. Bae and Ms. Alnaser and children walk through the gate out to the playground, but she did not see the staff doing a name to face count as they walked through the gate. She also did not see the staff turn around to make sure every child was with them before they shut the gate. Ms. Sanchez stated she looked and saw Child F alone in the cement area and start running toward the gate that leads to the parking lot. Ms. Sanchez stated she ran to the office and told the director Kristyn (Spence) who then ran out and got Child F. Ms. Sanchez estimated Child F was left alone for about a minute. I interviewed Young Toddler staff Hayeong Bae. She stated on 05/04/2022 she had just come back from her break as the class was getting ready to go outside. Staff Julia (Shelton) and Areej Alnaser were in the classroom at the time. Ms. Shelton was about to take her break and she did not go outside with the class. Ms. Bae had the children sit down on the floor in front of the exit door and she counted the children. She then opened the door and led the children to the gate that opens

    On 03/29/2022, , I completed an announced onsite inspection at The Goddard School in Rochester. We interviewed Child Care Staff Members Julia Shelton (assistant young toddler staff member) and Hayeong Bae (assistant young toddler staff member). Program Director, Kristin Spence was not working at the center at the time of the inspection. She was at a professional development training. Ms. Shelton stated she has worked in the young toddler room with Ms. Froh since he children in this room range from ages 18 months to 24 months. Ms. Shelton"oseTrv ed Ms. Froh become increasingly frustrated with children in the young toddler room over the past several weeks. She noticed that Ms. Froh was specifically frustrated with Child A. She stated Child A is an active child and sometimes hits other children. Recently, Ms. Froh was told not to communicate with Child A's parents about his behavior, because she is overly negative with Child A's parents in discussing her concerns about Child A. Ms. Shelton indicated this may be the reason Ms. Froh was so frustrated. 3 On 03/17/2022, Ms. Shelton observed Ms. Froh's interactions with Child A. She said during afternoon snack, Child A repeatedly got out of his seat. Ms. Froh told him to sit down and put him back in his seat, however, Child A did not want to sit at the table and eat snack. Ms. Shelton observed Child A get out of his seat again. He laid on the floor beside his seat and laughed. This frustrated Ms. Froh, who was sitting on the floor next to the table. She pulled Child A by the leg towards her and placed him back into his seat. He was about an arm's length away from the table when she pulled him towards her. Child A did not cry, however, he tried to get away from her as she held his leg. She picked him up and placed him back in his seat. Ms. Shelton did not observe any marks on Child A's body. Ms. Shelton observed a second incident between Ms. Froh and Child A the same day. Child A took his shoes off. Ms. Froh told him, "You need to keep your shoes on!" Then, from a seated position, Ms. Froh pulled Child A by both legs towards her. Once he was beside her, she put his shoes on for him. He screamed to get away from Ms. Froh while she put his shoes on. Ms. Shelton said Ms. Froh yelled at children, raised her voice, and used harsh directives. Ms. Froh was angry when children dumped toys. She placed toys out of reach for children so they were not accessible for use so there would be less mess to clean up. Ms. Shelton indicated that Ms. Froh expected children to act older than they were. Ms. Shelton stated that Ms. Froh received additional training in the past few weeks because she gave children time out, placed them in the corner, and held them too tightly, which restricted their movement. I interviewed Ms. Bae. She stated she is the assistant staff member "inte youn"to er classroom and has worked at the center for the last Ms. Bae stated that Child A is an energetic and sweet child, however, he pushes Roter children. Ms. Bae did not observe Ms. Froh pull Child A's leg or yell at Child A and force him to sit against his will. However, Ms. Bae indicated that Ms. Froh was stricter than Ms. Shelton or herself, but said she was not overly strict with children. Ms. Bae observed that Ms. Froh repeated her instructions to children when they were not listening to her, and she got down on children's eye level when she spoke to them. She observed that Ms. Froh appeared stressed when she had to repeat herself too many times. Ms. Bae said that sometimes after Ms. Froh repeated herself to the children, she would grab the child (who was not listening), by the wrist and ask them to come with her. Ms. Bae said Ms. Froh would have Child A sit next to her to help him calm down when he was upset or not following directions. Ms. Bae said that early last week (the last week of March 2022) a child in the class cried. She observed Ms. Froh stressed by the situation. She asked the child to sit and calm down. Ms. Bae

    On Friday 09/03/2021 at 4:38pm staff Paige Prudhomme emailed me that Child D's parents had sent the center pictures of marks and bruises on Child D's body. The email stated the directors had spoken with lead teacher Ashlee Mlejnek, and she was no longer employed at the center. The email stated after a thorough investigation both classroom assistant teachers Desaray Froh and Raven Ragland confirmed they had seen Ms. Mlejnek being forceful with children. The email asked what the center's next steps would be. On Sunday 09/05/2021 evening around 6:15pm I saw this email and replied at 6:31p m via email asking Ms. Prudhomme if staff had reported this matter to the Michigan Department of Health and Human Services (MDHHS) hotline as mandated reporters must immediately report suspicions of child abuse and neglect. I provided the 24/7 hotline number for MDHHS in the email. On the morning of 09/07/2021 I called the center and spoke with staff Paige Prudhomme and owner Sandeep Chada. Ms. Prudhomme stated they contacted MDHHS this morning. Mr. Chada stated Child D's parents had contacted him the evening of Wednesday 09/01/2021 about marks on Child D. He and director Laura Colonius spoke with the three staff: Ashlee Mlejnek, Raven Ragland, and Desaray Froh, from Child D's classroom the morning of Thursday 09/02/2021 about the marks on Child D. The staff stated they did not know of anything happening to Child D that would have caused the marks. Ms. Froh stated she saw a mark on Child D's arm on 09/01/2021 while she was changing Child D's diaper and she had pointed it out to Ms. Mlejnek who said it looked like an old bug bite. I discussed with Mr. Chada the Child Protection Law and mandated reporter requirements and 4 requirements to report incidents to Licensing. I asked Mr. Chada to send a written incident report (BCAL 4605) to Licensing. The written incident report was emailed to me on 09/07/2021. On 09/08/2021 I completed an on-site inspection to the center . We observed Child D in the Young Toddler classroom. We then interviewed staff about the complaint allegations. Staff Desaray Froh stated that she began working at the center as a child care staff member . Ms. Froh stated she usually works in the Young Toddler classroom, but she also works in other infant and toddler classrooms. Ms. Froh stated Ashlee Mlejnek was the lead caregiver in the Young Toddler classroom and the other child care staff assistant was Raven Ragland, who started about a month ago. Ms. Froh stated there had been a previous staff member who left to go back to school Ms. Froh stated Child D just started attending the center On 09/01/2021 Ms. Froh was working in the Young Toddler classroom and was changing Child D's diaper and noticed a round mark on the inside of Child D's left bicep. Ms. Froh stated she mentioned the mark to Ms. Mlejnek, and Ms. Mlejnek said, "It's fine, it looks old, and don't worry about it". Ms. Froh stated she asked Ms. Mlejnek if they should tell Child D's parents about the mark, and Ashlee Mlejnek replied, "If they didn't say anything about it, it's fine". Ms. Froh stated she then said to Ms. Mlejnek, "I think we should say something to Child D's parents,". Ms. Froh stated she left this up to Ms. Mlejnek because she was the lead caregiver. Ms. Froh stated that on 09/02/2021 Ms. Mlejnek came into the classroom with Laura Colonius. Ms. Mlejnek seemed upset and was grabbing her things. Ms. Froh stated Ms. Colonius called her into the office and told her they had let Ms. Mlejnek "go". Ms. Colonius showed her pictures of marks on Child D. Ms. Froh stated she was "shocked" when she saw the pictures; she (Ms. Froh) thought she knew Ms. Mlejnek. Ms. Froh stated she never saw Ms. Mlejnek do anything physical to any child. Ms. Froh stated later that day (09/02/2021) Ms. Mlejnek texted her saying she never wanted to speak to Ms. Froh again. Ms. Froh stated in the past she saw Ms. Mlejnek get "overwhelmed," and Ms. Froh would "take over" to help Ms. Mlejn

    I attempted to contact the complainant several times to get additional information; however, the complainant did not return my voicemail or email messages. On 0823/2021 I completed an on-site inspection to the center and interviewed assistant director Leah Binkley and licensee designee Sandeep Chada. Program director Laura Colonius was not at the center at the time of my inspection. Ms. Binkley stated on 08/11/2021 around 2:30pm Mr. Chada received a phone call from a parent about a PreK child testing positive for Covid 19. That child tested positive on 08/11/2021 and had last attended the program on 08/09/2021. Mr. Chada, who was not at the center when he received the phone call, called director Laura Colonius to inform her of this. Mr. Chada called the Oakland County Health Department for guidance. Ms. Colonius sent an email to all parents and staff of the center about the positive case, and it also stated the parents of the PreK children needed to be picked up in the PreK room. Ms. Binkley stated children in both PreK classrooms had to be picked up because both PreK classrooms interact with each other during various times of the day, including outdoor play and at the end of the day. Ms. Binkley stated she and Ms. Colonius also went to the PreK classrooms to tell the PreK staff members about the positive Covid case and tell them to get the children's things ready to go home. All the PreK parents came and picked up the PreK children. Ms. Binkley stated the email sent out also informed the parents the PreK children and PreK staff members that were not vaccinated needed to quarantine for 14 days. Ms. Binkley stated the health department told Mr. Chada that PreK staff members that were fully vaccinated and had no symptoms did not have to quarantine, so those staff members remained at the center on 08/11/2021 and cleaned and sanitized the PreK classrooms. Ms. Binkley stated initially Mr. Chada was not advised that the PreK siblings needed to quarantine, so some of the siblings stayed at the center after 3:00pm on 08/11/2021. Ms. Binkley stated on 08/12/2021, 08/13/2021 and 08/16/2021 the center was closed due to no power after a storm. Ms. Binkley stated on 08/12/2021 Mr. Chada was advised the PreK siblings should also quarantine, so Ms. Colonius emailed the parents of the PreK children that had siblings in other classrooms about this. Ms. Binkley stated on 08/13/2021 a PreK staff tested positive for Covid 19, so Ms. Colonius sent another email to all parents and staff at the center informing them of this. Ms. Binkley stated the PreK children, PreK siblings, and PreK staff member that tested positive are currently in quarantine until at least 08/26/2021 when they can return if they have not had a positive Covid 19 test or any symptoms. 4 Mr. Chada stated on 08/11/2021 he was contacted by a parent about a PreK child testing positive for Covid 19. He communicated with the Oakland County Health Department right away and then called Ms. Colonius about it and told her the PreK children and unvaccinated PreK staff members needed to be sent home. Mr. Chada stated he believes Ms. Colonius spoke directly with the PreK staff and many of the PreK parents when they came to pick up their children. Mr. Chada stated he spoke with and emailed the health department multiple times on 08/11/2021, 08/12/2021 and 08/13/2021. Initially the health department advised him since the child last attended on 08/09/2021 the PreK children and the PreK staff that were not fully vaccinated or had symptoms should quarantine for 14 days from 08/09/2021. Mr. Chada stated it was a bit confusing at first regarding who at the center needed to quarantine. Mr. Chada stated at one point, he doesn't remember if it was on 08/11/2021 or 08/12/2021, he received guidance from the health department that the siblings of the PreK children should also quarantine. Ms. Colonius called the parents of the PreK children and informed them that the siblings should also quarantine for 14

    The complainant did not leave any contact information; therefore, I was not able to speak to anyone to obtain additional information. On 12/16/2020 Mr. Chada and Ms. Colonius stated that they had two child care staff members test positive for Covid-19 and parents and staff were informed. On 12/16/2020 Cheryl Amare Child Care Licensing Consultant stated that she was informed by Ms. Colonius about of two positive Covid-19 cases at the center. On 12/17/2020 Ms. Colonius emailed the notices that were sent to the parents for the two positive Covid-19 cases at the center. Ms. Colonius provided me a copy of the email that was sent to the parents with the information. Parents are not allowed in the building; therefore, the notices were emailed. The first positive case was Child Care Staff Member 2. Her last day at the center was Wednesday 11/25/2020 and she tested positive for Covid-19 on Thursday 12/03/2020. On 12/03/2020 Mr. Chada informed the parents and provided Covid-19 information. On Friday 12/04/2020 Ms. Colonius contacted the health department, licensing, and informed the child care staff members. The health department did not instruct Ms. Colonius to do anything at the center. The health department did not require the center to close or close the classroom because of the number of days that Child Care Staff Member 2 was absent from the center before testing positive. The second positive case was Child Care Staff Member 4. Child Care Staff Member 4's last day at the center was Thursday 12/03/2020 and she tested positive on Saturday 12/12/2020. Mr. Chada informed the parents on Monday 12/14/2020 and provided Covid-19 information. Ms. Colonius contacted the health department, licensing, and informed the child care staff members. The health department did not instruct Ms. Colonius to do anything at the center. The health department did not require the center to close or close the classroom because of the number of days that Child Care Staff Member 4 was absent from the center before testing positive. Mr. Chada and Ms. Colonius stated that the center and classrooms are cleaned daily. The center was cleaned and sanitized after each reported case of Covid-19. The center has UV-light in the duct work that kills virus particles in the air. The center has an Energy Recovery Ventilator (ERV) on the roof that brings in fresh air and exchanges the indoor air almost on an hourly basis. On 01/26/2021 Child Care Staff Member 1 stated that she worked with Child Care Staff Member 3 and to her knowledge the staff and parents were not informed when Child Care Staff Member 3 tested positive for Covid-19. Child Care Staff Member 3 informed Child Care Staff Member 1. Child Care Staff Member 3 had been absent 3 from the center for four or five days before testing positive for Covid-19 . The classroom is cleaned and sanitized daily. Child Care Staff Member 1 said that she was not informed by Ms. Colonius about any of the positive Covid-19 cases at the center. On 01/26/2021 Child Care Staff Member 2 stated that her last day at the center was Thursday 11/25/2020. On Monday 11/30/2020 she had a sore throat and mild cough. She did not go to work and she had a Covid-19 test and tested positive on 12/03/2020. She returned to work on 12/14/2020. She was not at the center, so she was not sure if the center was cleaned and sanitized. However, the center is cleaned and sanitized daily to reduce the spread of germs. She thinks that the parents and staff were informed about her testing positive for Covid-19 . On 01/28/2021 Child Care Staff Member 3 stated that she did not test positive for Covid-19. She had symptoms and did not work at the center from 11/30/2020 until 12/17/2020. Her symptoms were fever for 2 weeks, sore throat, nausea, dizzy, and head ache. She completed two Covid-19 tests and they were both negative. The parents and staff were informed when Child Care Staff Members 2 and 4 tested positive for Covid-19 . She received an email from Ms. Colonius
