R 400.1911 · R 400.1911 1 Care; supervision; children. (1) A licensee shall ensure appropriate care and supervision of children at all times.
On 09/01/2022, licensing consultant Mr. Don Vita and I made an unannounced on-site visit to the home. I explained the allegations to Ms. Autumn Leonard. She stated that yes, children sometimes go home with marks on them, but she has always communicated with the parents whenever their child gets hurt while in her care. On 08/16/20222, Child B (23-month-old, male) had been climbing on the couch in the living room when he dove off the couch and gashed his head. Ms. Leonard notified the parents immediately and then took him to urgent care, where his parents met her in the parking lot. After assessing the injury, the parents decided not to seek treatment for Child B. Other instances where children have gone home with marks on them include: • Child A (31-month-old, female) once had scratched her belly on the kitchen set, • Child F (16-month-old, male) was walking and he tripped and fell hitting the bottom of the couch and had a red mark on his forehead that disappeared after a few minutes. • Child H (13-month-old, female) first started attending, she tried climbing out of the play yard and bumped her teeth on her lip. • Child J (4-year-old ,female) was sitting for a time out she had pressed her head against the kitchen cupboard and it left a mark on her face. Child J would often say that Ms. Leonard had scratched her or was hurting her or that the other children were hurting her, but no one ever was. 4 Ms. Leonard stated that Child E (3-year-old, male) will arrive with scratches on him, but nothing concerning, and he does not leave with any additional scratches or marks. Child C (3-year-old, male), Child D (9-month-old, female) and Child G (31- month-old, male) have never left her care with any marks that she has been aware of. Ms. Leonard recently started using an app called Brightwheel which allows her to keep notes of when children get hurt. It also allows her to communicate with parents and send them immediate notifications. Ms. Leonard reported that her, and her child care staff member Ms. Ashley DeGroft, use the app on a daily basis and keep parents informed of any and all accidents and injuries, big or small. On 09/01/2022, I interviewed child care staff member Ms. Ashley DeGroft. She reported that she does not believe child care children are getting hurt more than what can be expected from children, "It depends on the situation, they're kids." She provided examples of one time a child fell off the couch and landed on a toy, hitting his head on the edge of the couch. Ms. Leonard immediately called the parents and met them at urgent care. The child did not end up getting treated for the injury and returned to child care the same day. On another occasion, when Child F was running around with the other children, he fell and bumped his head on the couch. A picture was taken and sent to the parents. Child A had been playing with the kitchen set and somehow ended up scratching her belly on the door. Child J had gotten a mark on her head from hitting her head on the cupboard when she was sitting in time-out. Ms. DeGroft stated that Child J would always say she was hurt, but she never was. Ms. DeGroft further reported that both her and Ms. Leonard notify parents via the Brightwheel ap, as well as, communicating with parents at pick up time. On 09/14/2022, I interviewed Child A's Mother. She stated that Child A does not come home with marks on her. If something has happened, Ms. Leonard will notify her right away in the Brightwheel app, in a text message and even verbally when she picks Child A up at the end of the day. She reported having no concerns with the care and supervision that Ms. Leonard and Ms. DeGroft provide, "They truly care for the kids." On 09/14/2022, I interviewed Child C and D's Mother. She reported that Child C and Child D do not come home with marks on them and she does not have any concerns with the care and supervision that Ms. Leonard and Ms. DeGroft provide. On 08/23/2022, conducted an interview with Child A and B's Mother and reported WejnFrma Pon to Mr. Vita on 08/25/2022. Child A and B's Mother reported that Child A has had a lot of issues at 04 Ms. Leonard's child care. She gets daily messages that Child A is throwing toys hitting, and kicking, but Ms. Leonard would praise Child B's behavior every day was there. She said she asked Child B if there were any issues at the child care and told her no. After Child B sopped attending Ms. M Leonard's child care, tol mother about Child A being held in the comer for a time out. did not say anything at the time because. thought ■ would get in trouble a c ild care. On two occasions, Child A was hysterical when Child A and B's Mother picked up from Ms. Leonard's. One time, Child A had . On 08/24/2022, attempted a Forensic interview with Child A at Child A an sRa Wers home. Child A yelled, "No, no," and "no, don't want to talk." On 0812412022, conducted a Forensic interview with Child B at school in accordance"wwiit h Michigan's Forensic interviewing protocols. Child B sta ed, "I'll show you how they put in the comer went over to the wall, put hands over face, an stood acing the wall. said Child A had to o stand like at for four menu es whe got in trouble at s. eonard's child care. Ms. Leonard is the only one who Child B also reported that Ms. Leonard and swore at ntia A, , and said "bad words." Child A gam trouble at child care for jumping on the couch, poking sleeping children, talking during nap, and jumping over children. Child A only had two good days the entire time was at child care. The rest of the days, did not cooperate. went into time out almost every day. Child B did not tell mother about wha was happening becausEE did not want her and Ms. Leonar to get into a fight, an knew that there was no one else who could watch and Child A. Once when hild A was jumping on the couch, At naptime, they forced Child A's be quiet, and stay there. Child B identified one other who got in trouble at child care and had to go to time out in the comer because did not want to pick up ■ toys. did not know how old was. There were then two other who would get yelled at, but they were not put in the corner. On licensing consultant Mr. Donald Vita received an email from with video attachments taken by Ms. Leonard. Only0 8/26/2022,omeniocou e opened. The videos show Child A sitting in a child sized chair while other children are engaging in outdoor activities. Child A is sitting, yelling something inaudible over and over, while C3 stomping his/her feet and slapping the arms of the chair with his/her hands and arms. On 09/01/2022, child care licensing consultant Mr. Vita conducted an unannounced onsite investigation with child care licensing consultant Ms. Dana Hart. After informing Ms. Leonard of the allegations, Ms. Leonard explained that Child A presents challenging behaviors daily that can include hitting or kicking other children, throwing toys, and pushing or hitting other children. Ms. Leonard's informed Mr. Vita that her approach to discipline is to first stop the behavior if it is posing a safety issue to other children. She added that if children need to be disciplined while in care, she will first talk to the child to understand what the issue is, and then help the child identify appro riate alternative behaviors. Ms. Leonard does use time outs with Child A because can violently act out, hitting or kicking staff or children in care if does not have ime to calm down. When time out is used, Child A is made to If Child A continues to be distracted by other children and is not able to calm Ms. Leonard will next to child A's peripheral vision. She does not hold any s head against anything. When asked about Child A's Ms. Leonard stated that on 08/15/2022, Child A had from leaning forward while in time-out. Ms. Leonard uses the Bnightwheel
