On 03/31/2021, 1 received an incident report, dated 03/31/2021 from Ms. Stacey Kramer, licensee designee/program director stating the following: was holding Child A while entering a gated play area and a child pulled the gate shut catching her foot in the closed gate. She fell with Child A in her arms. They both hit their head on the metal bar going down. I was told of the incident and after we did a check of the child's body for injuries, we found red marks on the side of head. I 1their immediately notified Child A's Mother of the incident. They took Child A10 On 03/31/2021, 1 completed an unannounced onsite investigation at the center. I interviewed Ms. Kramer, and Judy Wieber, child care staff members. Ms. Kramer stated she was"noint in the center at the time the incident occurred. She was returning from lunch. The injury occurred at approximately 1:30 p.m. on 03/20/2021 and she arrived back to the center at 1:40 p.m. Upon arrival she was told that fell with Child A in her arms. was going into the gated play yard in the room to console Child B who was crying. foot was stuck in the gate opening and had to be assisted by Ms. Wieber to get M unstuck. received a bruise on her head. Ms. Kramer stated Child A was examined by staff or any further noticeable injuries to body. Child A had a red mark on head. She contacted Child A's Mother righ away. Child A's Father was clos e center and arrived picking-up. Child A es 3 Ms. Kramer Child A had . cnita AS motfer continue to text tier tnrougn tree ild A's condition. Ms. Kramer stated Child A's Mother and Child A's Father appeared to be handling the situation well, but she had not spoken to them and all contact had been through text. I observed the metal play yard located in the infant room. Ms. Kramer provided a tape measure, and I measured the height of the play yard which was 30 inches in height. I did not observe any broken equipment parts and play yard appeared sturdy and stable. The play yard is used by the older mobile children and child care staff in the room to play, typically the mobile children are approximately 8 months old and older. The nursery room children ages are from birth to 14 months old. The play yard is used when non-mobile and mobile children are doing tummy time Child care staff members use the play yard as a designated play area with play equipment for mobile children. and Ms. Wieber were botWresent in the infant room. and s. ie er demonstrated where foot was located and M ow she and Child A were found after the fall. t was found in the entrance gate of the play yard, between two me a ars and Child A were found on the floor. She was in the middle of the warming Child A's bottle when Ms. Wieber left out the doorway of the gated play yard to provide care to another child that was crying in a crib leaving Child B in the gated lay yard. ssttaatteedd she went to soothe 1) Child B -old as i stare crying whilee still holding Child A in her arms, opened the gate to enter into the play yard and her foot became tangled in between the metal rail spindles of the play yard. stated her head definitely hit the bar but was not sure if it was the metal ar or the floor that caused Child A's injury, stated she was trying to protect Child A as she was falling by trying to brace herself,hold Child A close to her but she could not stop the fall. needed Ms. Wieber's assistance to untangle her foot. Child A was crying but she did not know if it was from the fall or from hunger. After checking Child A for injuries, reporting the fall, and giving a bottle Child A stopped crying. The only injury observed on Child A was noted on the left side of Child A's head (red raised bump). As soon as Ms. Kramer returned to the center, she contacted Child A's Mother and Child A's Father arrived shortly after 2:00 p.m. stated she felt horrible and nothing like this has

On 10/07/2020, I received an email from Ms. Kramer at 9:47 p.m. stating Child A was left on the play area. Child A's caregivers were unaware that Child A was left behind until Child A's Father arrived for pick-up. Ms. Kramer's email stated caregivers entered the center at 4:55 p.m. from the play area and it was not until Child A's Father arrived at 5:00 p.m. that caregivers became aware Child A was not in the center. On 10/08/2020, I received an email from Ms. Kramer with the copy of the incident report. On 10/08/2020, I interviewed Child A's Mother by telephone. Child A's Mother stated she was contacted by Child A's Father by telephone on 10/07/2020. He told her that Child A was not in the room when he arrived for pick-up although he was told otherwise by Ms. Bubel. Child A's Mother stated Child A was found outside crying. Child A's Mother stated later in the evening she received a telephone call from Ms. Kramer and Ms. Bubel apologizing for what had occurred with Child A. Child A's Mother stated when she dropped Child A off this morning at the center he did not want to let her go. I interviewed Child A's Father by telephone on 10/08/2020. Child A's Father stated he entered the child care center on 10/07/2020 at approximately 5:00 p.m. He walked in an observed about 8 children in the room who were watching television. Child A's Father stated he looked to where Child A usually sits and did not see him. Child A observed that Ms. Bubel was about to vacuum and asked her where Child A was; she indicated by pointing that Child A was on the other side of the room sitting. Ms. Bubel peeked over to the other room and did not see him and then walked to the bathroom, and Child A was not found there as well. Child A's Father stated Ms. Bubel walked to the other side of the center and asked Ms. Linda Kramer if she had seen Child A. Ms. Kramer asked if Child A could be outside. He observed Ms. Bubel go outside and bring Child A back in the center. Child A was upset and crying as he was brought back in. He asked her if Child A was out with another caregiver but Ms. Bubel walked away without saying anything. Child A's Father stated he was extremely upset and did not understand how Child A could have been left alone outside and none of the caregivers know he was missing. Child A's Father stated he called Child A's Mother and told her what had occurred and then Ms. Kramer (licensee) met with him to discuss this matter. Child A's Father stated he was told that Ms. Bubel and Ms. Romano returned to the classroom from 3 outside at 4:55 p.m. and it took approximately 3 minutes to find Child A when they noticed he was not in the classroom. Child A's Father stated Ms. Kramer and Ms. Bubel called them later that evening to further discuss Child A being left behind. Child A's Father stated it appear that Ms. Bubel attempted to minimize the amount of time (5 minutes per Ms. Bubel) Child A was left alone. Child A's Father indicated that it was unacceptable for Child A to be alone for any time frame. Child A's Father stated he wanted to make sure that this did not happened to his child or any children in the center again. He was told by Ms. Kramer that new practices would be put in place at the center. On 10/12/2020, I completed an onsite inspection at the center. I interviewed Ms. Stacey Kramer, Ms. Bubel, Ms. Romano, and Ms. Linda Kramer. Ms. Kramer stated since Child A was left behind and alone on the playground, she has been working hard with all of the caregivers, especially with Ms. Bubel and Ms. Romano. I received copy of typed statements from Ms. Stacey Kramer, Ms. Bubel, and Ms. Romano. She has never had a child left behind and no one knew where he was. She is extremely thankful that he was safe but recognizes that it should not have occurred. Ms. Kramer stated on 10/07/2020, she, Ms. Bubel and Ms. Romano were outside with their children from their classroom. They went outside at 4:15 p.m. She had (4 children), Ms. Bubel had (3 children) and Ms.
