On 10/20/2021 I conducted an unannounced inspection to Home Again Child Development Center. I observed the children sleeping in their classrooms as it was nap/rest time. In the infant room there were five children present and two child care staff. There were several rock-n-plays in the infant room, but they were folded up and set against a back wall, not being used. I interviewed program director, Stephani Shephard, and infant room child care staff members, Jordan Poth, and Melody Mattson. I obtained child information cards, the staff handbook, and the parent handbook with center policies. I read the complaint to program director, Ms. Shephard. Ms. Shephard reported that she had recently given a tour to a family with a young child and a baby on the way, and the mother had sent her an email with the same concerns in the complaint, one of them being rock-n-plays being used in the infant room, so she assumed the complaint was from the tour. Ms. Shephard indicated the center does have three rock-n-plays but they have never been used for sleeping and are currently not in use. The children were always awake and supervised while they were in the equipment. Ms. Shephard did not recall any child using the rock-n-play equipment during the tour but admitted the rock-n-play was out that day, so it is possible it was used. I told Ms. Shephard that rock-n-plays are a recalled piece of equipment for infants and that they could not be used in the facility in any capacity. Ms. Shephard apologized and agreed to throw them away immediately. Ms. Poth is a child care staff member in the infant room. She reported that she believed there were rock-n-plays in the infant classroom, three in the classroom, and two in storage. Ms. Poth indicated all of rock-n-plays will be thrown away today. Ms. Poth was not present the day a tour was given of the facility recently, so she would not know if a child was in a rock-n-play that day. Ms. Mattson has worked at the center since May 2019. She has always worked in the infant room primarily but does work in other classrooms if needed. Ms. Mattson recalled the tour that recently occurred. She reported that she was holding a very fussy child when the family came up to the gate of the infant room. Next to her was an infant in a rock-n-play, awake, that she was also supervising. It should be noted that on this same day I drove by the center approximately an hour after I had departed from the onsite and observed the rock-n-plays set out by the trash bin outside of the center. On 12/02/2021 I spoke with Child A, Child B, Child C, and Child D's Mothers. Child A and Child B's Mother did not have concerns with the center using the rock-n- plays in the infant room. She reported that the center threw the rock-n-plays away after I came out to the facility. Child A is currently in the infant room, and she has no concerns with the classroom or the staff. Child B was in the infant room prior to rock- n-plays being recalled and they used them at the facility back then and at home for Child B. 4 Child C and Child D's Mother did not have concerns with the center using the rock-n- plays and reported that she has never observed the child care staff using the rock-n- plays when she has come into the classroom for either of her children. Both children have been through the infant room. On 12/03/2021 I spoke with Child E, Child F, Child G, and Child H's Mothers. Child E, Child F, and Child G's Mother reported she has three children at the center and has no concerns with any of the classrooms or child care staff. She indicated Child E is in the infant room and she has been in the infant room many times and never observed any issues. When Child E started, the program director went over infant safe sleep rules with her and what Child E could have at the center with him and what was not allowed. On 10/20/2021 I conducted an unannounced onsite inspection to Home Again Child Development Center. There were five children and two child care staff present in the infant room. I observed three of the children appropriately/safe sleeping in their assigned cribs, and two children awake, one being held, and the other being changed on the diapering table. I interviewed program director, Ms. Shephard, and infant room child care staff members, Ms. Poth, and Ms. Mattson. Ms. Shephard reported that the center takes safe sleep very seriously and denied that any infant is/was left in equipment sleeping with a blanket. She indicated that during the tour that was given to a family recently, a child did fall asleep in equipment but was moved to a crib to sleep. I asked her how long until the child was moved, and she indicated it was less than three minutes. Ms. Shephard indicated the family talked mostly with her in the toddler and waddler rooms but just peeked into the infant room (the waddler room is between the toddler and infant rooms and separates from the infant room by a doorway with a baby gate). The family did not enter through the gate to the infant room, so they did not view the crib room. They talked in front of the infant room gate approximately 30 seconds to a minute and then the family left, the whole tour being only a few minutes total. Ms. Shephard reported she has every child care staff member take safe sleep and shaken baby/head trauma trainings. Child care staff also take a second infant safe sleep training from the Early Childhood Education Institute (ECEI) and safe sleep is discussed during staff meetings regularly. Ms. Shephard reported that the infant room child care staff physically check on the children in the crib room every 10-15 minutes when they are sleeping, and they also purchased a video monitor to be used as an extra way of monitoring the children on top of still physically checking on the children every 10-15 minutes. Ms. Poth reported that she was not at the center the day of the tour given to a family, so she would not be able to speak about that day, but she denied ever leaving children asleep in any equipment, or letting them sleep with any objects, blankets, etc. She admitted that children are sometimes given a blanket for soothing purposes when they are awake and being supervised, or rocked in a chair, but never when they are being put in their crib to sleep. Ms. Poth was able to tell me all the rules for infant safe sleep and indicated they are regularly trained on safe sleep practices. The center trained Ms. Poth when she started and they do a refresher training every three to six months at staff meetings, or online courses. Ms. Poth is also currently taking early childhood classes, which have content about infants and safe sleep practices. Ms. Mattson reported that the child in the rock-n-play did fall asleep in that moment that the family was present during a recent tour of the facility, but the baby she was holding was fussing, so she calmed the fussy baby and set the child on a mat and then moved the child that fell asleep from the rock-n-play to the crib. This was maybe over the span of a minute or two. Ms. Mattson indicated there was also a child in a swing, but that child was awake. Ms. Mattson was able to tell me the guidelines/child care licensing rules for safe sleep of infants and reported that she and other child care staff always follow safe sleep. Children are always supervised and moved to cribs if they fall asleep in equipment and are put in the crib on their back with nothing hanging or inside the crib with the child. Ms. Mattson confirmed she had received safe sleep training and indicated that they go over safe sleep regularly at staff meetings. On 10/20/2021 I conducted an unannounced onsite inspection to the center. I observed three children safely sleeping in their cribs, one child being rocked, and one child having their diaper changed. The program director and child care staff reported that children are always supervised and when they fall asleep, they are moved immediately to a crib to sleep. They indicated there was a recent tour of the facility given and during the tour a child did fall asleep in equipment but was moved in less than three minutes. Each person was able to tell me the guidelines/rules for safe sleep and reported they are trained at orientation and at least annually. The parents interviewed reported no concerns with the infant room and indicated they have been in the room and have always observed staff using safe sleep practices with the children.
