Ms. Marguery said she was never made aware of Child A having a black eye or having a bloody ear near her earring. Child A's Mother has not said anything to her about these concerns. Ms. Marguery said that accident reports are kept in a folder in the room until they get a chance to be put in the child's file in 7 the main office. Ms. Marguery might not ever see an accident report unless it is something serious that requires them to file with their insurance. Ms. Huizar-Pike was working the day Child A's ear was bleeding near her earring. All the children were playing and climbing on the cushions. She observed Child A run into one of the staff members and started crying. They then noticed the blood near her earring. Ms. Huizar-Pike figured that Child A snagged her earring on the staff member's shirt. She wiped up the blood and that was it. It was only a small amount and did not continue to bleed. Her earring was not ripped out of her ear. Ms. Huizar-Pike documented it in the Tadpole application for her parents. They would not have known Child A's ear had bled if they were not told about it. Ms. Huizar-Pike was not at the facility at the time of the incident with the rocking chair where Child A received a black eye. Ms. Jacob messaged her the night of the incident to let her know what happened. She thought it in was August or September. Ms. Jacob was in the rocking chair feeding an infant. Child A went behind the rocking chair and got bumped. Child A's Father was notified at pick up what happened and an incident report was written. Ms. Jacob forgot to have Child A's Father sign it when he picked up Child A from child care that day. Child A's Mother signed it the next day. Ms. Jacob messaged her the night of the incident to let her know what happened. She thought it in was August or September. The report should be in the Child A's file. Ms. Huizar-Pike said she knows Child A's Mother knew about the incident because the next day she said to the secretary, "look what they did to my baby." Ms. Huizar was not working when Child A was hit with the rocking chair. She thinks that Child A had a small mark under her eye. She was not present during the time Child A's ear was bleeding. When she arrived that day Child A was fine. She was informed that they thought that she snagged her earring on something which caused it to bleed. If a child gets hurt at child care a written accident report will be completed and the parents are verbally notified when they pick their child up from care. They would sign the report. They can also document it in the Tadpole application. Ms. Huizar would consult the director if a parent should be called such as when the injury is a little more serious like a child is not acting like themselves or if there is a lot of blood. Ms. Huizar has no concerns with how other staff treat the children. There have been no parents that have brought any concerns to her attention. Ms. Jacob was aware of the incident and she was the only staff in the room at the time it happened. Ms. Jacob said she was sitting in a gliding rocker chair feeding an infant. Child A and another child were playing near the second gliding rocker chair. Child A went behind the rocking chair and the other child was in front of it. The other child was trying to pull themselves up to stand by using the rocker. The child fell and moved the chair when they fell. Child A was also holding the rocking chair so this caused her to fall also. She hit her face on the rocker. Ms. Jacob got ice for Child A but she did not want to keep it on her face. Child A instantly had a bruise on her face as the rocker hit her on the bone right below her eye. Ms. Jacob completed an incident report and she notified Child A's Father about the incident at pickup. She forgot to have him sign the incident report though. Ms. Huizar-Pike had Child A's Mother sign the report the following day. Ms. Jacob also talked to Child A's Mother and apologized for what happened. She was not sure if

After reviewing Child A's information log for 3/22/22, 3/24/22, and 3/25/22 the information for times of feeding, sleeping, and changing diapers are not accurate as the times contradict each other.
