On 10/06/2022, I attempted to contact Child A's Mother. A voicemail message was left requesting a return call. On 10/12/2022 I completed an onsite inspection. I interviewed program director Ms. Kaylie Skutt and child care staff member Ms. Carly Lamphiear regarding the allegations. Area manager Ms. Amanda Woods was also present. Ms. Skutt reported that on 10/05/2022, she and Ms. Lamphiear were working together in the classroom. The incident occurred during free choice time. Ms. Lamphiere was toileting children, and she was helping the rest of the children to put their painting smocks on. Ms. Skutt observed Child A taking his smock off after he was done painting. She assumed that he was playing with the other children. A short time later, Ms. Lamphiear asked where Child A was because it was time to change his diaper. When she and Ms. Lamphiear discovered that Child A was not in the classroom, Ms. Skutt left the classroom to look for him. She looked down the hallway and asked the kindergarten teacher if she had seen him. The kindergarten teacher stated that she had not seen him. Ms. Skutt then returned to the classroom to see if Child A was hiding somewhere in the classroom. After determining that he was not hiding in the classroom, Ms. Skutt recalled an incident that had occurred a week or two before; while children were getting ready to leave for the day, child care staff members noticed that Child A was trying to go toward the door that is used for drop offs and pick-ups. Ms. Skutt then checked this door and Child A was standing outside of the door. Ms. Skutt brought Child A back into the classroom and contacted Child A's Mother. Ms. Carly Lamphiear reported that she began working at Reed City ECC in August 2022. She and Ms. Skutt were working in the classroom on the day that this incident occurred. She was assisting children with toileting. When she looked for Child A, she did not see him in the classroom. Ms. Lamphiear asked Ms. Skutt where Child A was, and she did not know. They searched the classroom, and they did not find him. s. Skutt then left the classroom to find him. Ms. Lamphiear reported that she did not recall if the door was open or closed when this happened. She stated that Child A had never attempted the leave the classroom before, but she recalled a previous incident where he ran around the corner to the door where children are dropped off and picked up. This incident happened while children were getting ready to leave for the day. Ms. Lamphiear had to run after him and bring him back to the classroom. Ms. Lamphiear stated that she has never had any concerns regarding supervision. I asked Ms. Skutt, Ms. Lamphiear, and Ms. Woods if the classroom door was ever left open. They showed me that there is a mechanism on the top of the door that will keep the door from closing if it is opened. Prior to this incident, they would sometimes use this to keep the door from locking each time it is opened. This piece is being removed from the door and the door will remain closed at all times. On 12/01/2022, I contacted Ms. Amanda Woods to confirm how many children were present on the day that this incident occurred. She stated that there were nine children present.
