I completed an onsite inspection on 03/09/2022. Program director Ms. Kaylie Skutt was not working. I interviewed child care staff member Ms. Crystal McGregor. She reported that she and Ms. Skutt were working on 03/03/2022. This is a four-year-old preschool program and there were fourteen children in care on that day. Ms. Skutt had left the room to talk with Head Start area manager Ms. Amanda Woods. Ms. Skutt was out of the classroom for approximately fifteen minutes and Ms. McGregor was alone with the fourteen children. Ms. McGregor reported that during this time, children were engaged in free choice activities and she was preparing snack. Ms. McGregor observed that Child A was the only child playing in the paint area. She noticed that Child A was on the floor next to the paint table. She told Child A that she could not paint there, and Child A stated that she had dropped her paint brush. Child A then left the area and moved to another play station. Ms. McGregor then turned around and began opening cheese. Ms. Skutt returned to the room a few moments later. Ms. McGregor reported that they talked in the doorway for a couple of minutes and she then left the room to take two children to the bathroom. Ms. Skutt stayed in the classroom with the other twelve children. She returned to the classroom about ten minutes later and Ms. Skutt informed her that she had discovered Child A and B underneath the paint table. Ms. Skutt told her that Child A's pants were down and she was straddling over the top of Child B. On 04/15/2022, I interviewed program director Ms. Kaylie Skutt. Ms. Skutt reported that there were fourteen children in care on 03/03/2022. She had left the room to get something out of the office and Ms. Amanda Woods was in the office. Ms. Skutt talked to Ms. Woods for approximately ten or fifteen minutes. When she returned to the classroom, she talked to Ms. McGregor in the doorway for a minute and Ms. McGregor then took two children to the bathroom. Ms. Skutt went over to make sure there was water in the portable sink that is located by the paint table. At that time, she noticed Child A and Child B underneath the paint table. She asked them to come out from under the table and she observed that Child A was pulling her pants up. When she asked Child A and Child B what had happened, Child A stated, "I must have had a wedgie." Ms. Skutt stated that Child B's pants were not pulled down. Child B stated something like, "I must have one too." Ms. Skutt contacted Child A and Child B's Mothers to discuss the incident. There are sixteen children enrolled in the preschool program and there are usually three child care staff members working to ensure that child care staff members to child ratios are maintained. On the day of this incident, she believes that the third child care staff member was out sick. She reported that there are rarely concerns with ratio, but on occasion, if there are only two child care staff members working and there are more than twelve children in care, there will be very brief times that they cannot maintain child care staff members to child ratios if one of them needs to use the bathroom. On 04/15/2022, I interviewed Child A's Mother. Child A's Mother reported that Ms. Skutt reported this incident to her. Ms. Skutt told her that Child A and Child B were in an area that is a blind spot and child care staff members did not realize right away that both children were in the area. Child A's Mother stated that Child A has never displayed any behaviors involving inappropriate touching prior to this incident, but 3 she was stunned that it happened. Child A's Mother has never had any concerns regarding supervision and she does not know what was going on that day regarding supervision. On 04/15/2022, I interviewed Child B's Mother. Child B's Mother reported that Ms. Kaylie Skutt called to report this incident to her. Child B's Mother is very concerned about how this could have happened because there are usually three or
Child A (four-year-old, female) and Child B (four-year-old female) were found under a table with their pants off straddling one another. 2 Child C ran into Child A causing her to fall into a table and cut her left eye.

On 04/18/2022, Ms. Kaylie Skutt reported that Child C was running in the classroom. When Ms. Skutt noticed this, she was preparing to tell her to stop running, when Child C bumped into Child A causing her to fall into a table. Child A's eye was cut when she hit the table. This incident occurred when child care 4 staff member Ms. Crystal McGregor had taken two children to the bathroom, and Ms. Skutt was in the classroom with the other twelve children. Ms. Skutt reported the incident to Child A's Mother. Child A's Mother took Child A to Reed City Hospital for treatment. On 04/15/2022, Child A's Mother reported that she received a call from Ms. Kaylie Skutt. Ms. Skutt informed that that there was an incident involving another child running into Child A and knocking her into a table. Child A's eye was cut and bruised as a result of this incident. Child A received medical treatment at Reed City Hospital's emergency room. The injury did not require stitches.
