On 11/17/2022, I interviewed Child A's (three-year-old, female) Mother. Child A's Mother reported that she had concerns about a child care staff member who attempted to take Child A out of her arms because Child A repeated told her not to touch her. Child A enjoyed attending Kiddies Klubhouse, but she did not like Ms. Mary Reid. On 11/10/2022, Child A complained that her back hurt. When Child A's Mother looked at Child A's back, she observed a mark on her back in the area of her lower spine that appeared red and bruised. Child A's Mother asked Ms. Reid how Child A had gotten the bruise. Ms. Reid told Child A's Mother that she did not know. Ms. Reid also stated to Child A's Mother that it would take more than a bruise for them to call Michigan Department of Health and Human Services on her. Child A's Mother later talked to program director Ms. Amber Hutchinson about the bruise and Ms. Hutchinson denied that Child A could have received the bruise while she was in care at Kiddies Klubhouse. Ms. Hutchinson was very aggressive toward her when she expressed her concerns. Child A also had a red mark on her ankle. When she asked Ms. Hutchinson about this mark, Ms. Hutchinson stated that it looked like it was caused from Child A's sock rubbing on the area. Child A's Mother stated that Child A is very clumsy, and it is possible that she caused the bruise and red mark herself, but she was concerned that the child care staff member did not know how the marks had occurred. Child A no longer has the bruise and she did not take a picture of the bruise. She stated that the red mark on Child A's ankle is still visible, and she would send a photo to me. On 11/17/2022, Child A's Mother sent me a photo of the red mark on Child A's ankle. The mark is very small in size and red in color. It appears to possibly be a scratch. On 11/29/2022, I completed an onsite inspection. I interviewed program director Ms. Amber Hutchinson, and child care staff member Ms. May Reid. Ms. Hutchinson also provided a written statement regarding this complaint. Ms. Hutchinson reported that Child A began receiving care at Kiddies Klubhouse on 10/07/2022. A few days after Child A began attending, Child A's Mother told her that she did not like the way "that lady" tried to take Child A from her. It was determined that "that lady" was Ms. Reid. Child A's Mother stated that she was running late for work, Child A was not listening, and she had to get going. Child A's Mother stated that Ms. Reid tried to take Child A out of her arms. Ms. Hutchinson explained to Child A's Mother that most parents ask child care staff members to help with drop offs in the morning as they are in a hurry to get going. Child A's Mother stated that she does not want help from child care staff members at drop offs. Ms. Hutchinson stated that she would 3 share that information with Ms. Reed and other child care staff members that open in the morning. Ms. Hutchinson reported that she had a telephone conversation with Ms. Reid on 11/10/2022 regarding a conversation that Ms. Reed had with Child A's Mother. Ms. Reid reported that Child A's Mother informed her that Child A had a bruise on her back. Ms. Reid looked for the bruise but did not see any injury. When Ms. Hutchinson arrived at the center, she checked for a bruise on Child A's back, and she did not see a mark or bruise. Ms. Hutchinson stated on 11/14/2022, Child A was dropped off prior to Ms. Hutchinson's arrival. She did talk to Ms. Reid, and she indicated that Child A's drop off was great that morning. At approximately 9:20 am, Ms. Hutchinson received a telephone call from Child A's Mother. Child A's Mother stated that she was going to pick Child A up in about 20 minutes and asked that she be ready to go. When Child A's Mother arrived, she appeared to be very agitated and she indicated that Ms. Hutchinson did not address her concerns regarding a bruise on Child A's back. Ms. Hutchinson stated that she informed Child A's Mother that she was under
Child care staff member Ms. Mary Reid aggressively attempted to take Child A (three-year-old, female) out of Child A's Mothers arms. Child A also had a bruise in the middle of her lower back by her spine and a red mark on her left ankle. Child care staff members did not know how Child A received the marks.

Child care staff member Ms. Mary Reid was heard yelling and screaming at children as well as observed dragging Child A up the stairs by his arm and child fell on the stairs. unannounced on-site inspection. We interviewed a program director and child care staff members. The allegations were discussed with program director, Ms. Hutchinson. She indicated that Child A (4-year-old, male) was present at the facility today. He did not have any behavior issues at drop off that morning. On the day of the alleged incident she was not working and only has second hand knowledge of the incident_ She received a phone call from the staff requesting the permission to have Child A picked up due to his behaviors. She trusted their judgement and approved them to make the call to have him picked up. Child A has only been attending the child care for-. She provided us with attendance on the of 2 the incident. She also allowed us to watch the video which captured audio and some footage of the alleged incident. Ms. Mary Reid is a child care staff member. On the day of the incident, she was scheduled to arrive to work at 6:30am. She was told that things were quiet at the center and she could come into work at 7am. At 6:44am she received a phone call indicating she needed to come into work as soon as possible. She could hear Child A in the background screaming, crying and shaking the gate. When she arrived she went to the infant room where the children and child care staff room were present. She made an attempt to calm Child A down by sitting down on the floor with him. Child A calmed down which allowed her to tend to other children. She was lining the older children up to go upstairs to their classroom and Child A took two steps toward the line and flopped down on the ground. She sat down with him "crisscross applesauce." She asked him to please stand up and walk in the line with the other children. Child A refused. She placed her arms under his arm pits to help him stand to go upstairs. When she did this his body went limp into her body. She kept her arms under his. When they were both standing, she guided him out of the room towards the steps. As they got to the top of the stairs, he tried to lunge his body into the door leading outside. She lifted his body up and proceeded up the stairs to the second floor landing. While going up those stairs he grabbed the railing in an attempt to prevent them from going upstairs. She was able to get his hands off of the railing and moved to the top of the stairs. She stood him up and he fell to the floor kicking and screaming. By the time they made it up the stairs another CCSM had called Child A's mother to come and pick up due to his behaviors. Ms. Reid explained she was doing her best to keep him safe from hurting himself and others. Ms. Taylor Holmes, child care staff member, stated that she arrived in the classroom around 7:20am. She saw Ms. Reid and Child A sitting on the floor. Ms. Reid was trying to calm Child A down. He was screaming and kicking his legs. She also tried to talk to Child A, but he was not listening to her. He was asked by Ms. Reid to stand up so he could go to breakfast. She helped him up from the floor but once they were up, Child A's body went limp. Ms. Reid tried to hold his hand and he refused to walk. Ms. Reid then put her arms under his armpits to keep him from falling to the ground. Child A went limp again causing his back to fall into Ms. Reid's body. While she was trying to guide him out of the room he was fighting and pushing his body against Ms. Reid's. Ms. Holmes remained in the infant room as Ms. Reid and Child A exited the room. She explained that Ms. Reid remained calm and never raised her voice at Child A. Ms. Lindsey Storey is a child care staff member who works in the preschool room. She arrived at the center at 7:30am. When she arrived, Ms. Reid was trying to get Child A upstairs. Ms. Powell and I were provided with child information cards and attendance records for 06/03/2022. We reviewed the documents and were told by child care staff members that Ms. Reid was told she did not have to come into work at 6:30am, but could arrive at 7am on June 3, 2022. According to the attendance records there were five children between the ages of birth and 3 years of age present with one child care staff member from 6:30am until about 6:50am.

During the unannounced on-site inspection conducted on 03/24/2022, I interviewed the program director Ms. Amber Hutchison, child care staff members, Ms. Mary Reid, Ms. Emma Kik, Ms. Aimmee Dow, and Ms. Shannon Hartley, and one child care aide, Mr. Zackery Bennett. Each of the staff stated that they have never seen any of the child care staff members treat children in a mean or malicious way. At times caregivers have raised their voice in a direct manner to get the children's attention. However, they do not yell or curse at child care children. 2 They have never forcefully sat a child down, dragged a child across the floor or seen another child care staff member do these things to child care children. Child A's Mother indicated that she is very pleased with the care that her child receives. She has never had concerns regarding physical discipline or inappropriate language being used towards her child. At the time this report was written I had not received a returned phone all from Child B's Mother.

All persons interviewed indicated that they are fully staffed and do not have issues with maintaining ratio. Ms. Hutchison indicated that she hires 17- year-olds through the vocational training program through the local public school. 3 They do not count in ratio and are not left alone with children. The current child care aide that is employed will be finished on 05/05/2022 as he will be graduating. The other child care staff members stated that child care aide does not count in ratio and is never left alone with the children unsupervised. Mr. Zackery Bennett stated that he is a floater and goes where he is needed in the center to assist the child care staff members. Technical assistance was provided to Ms. Hutchison regarding employing child care aides. They also must have satisfactorily completed at least 1 year of vocational- occupational child care aide training program approved by the department. Due to this program not being offered at this time in our area Ms. Hutchison was informed to submit a variance request since there is not a way to currently come into full compliance with the rule.

On August 11, 2021, I conducted an unannounced on-site inspection. I informed the program director, Ms. Amber Hutchinson, of the allegations. She stated that she has said a curse word on accident; however, it is nothing that is done consistently in front of children. Ms. Mary Reid also stated that in the past she has used a curse word on accident; however, it is nothing that she makes a habit of doing. The other CCSMs stated that they have not used profanity around children and they are not aware of other caregivers doing so. I spoke with Ms. Hutchison and expressed to her those mistakes happen; however, it is important to make an active effort to not use any profanity in the childcare center when children are present. I spoke with Child A's Mother, Child B's Mother and Child C's Mother. They all indicated that they have never had concerns regarding profanity being used by child care staff members in front of their children. 2

Ms. Hutchinson indicated that the center always has plenty of food, milk, and water to provide the child care children. Children are never not fed or denied food. Ms. Hutchinson gave me a tour of the center to show me the cabinets, refrigerator and freezers that are stocked with food supplies. I also observed child care children participating in lunch time. The children were being provided an appropriate amount of food and beverage. I interviewed child care staff members Ms. Mary Reid, Ms. Emma Kik, Ms. Shelby Swanson, Ms. Hailey Duchene, and Shannon Hartley. All CCSM interviewed indicated that child care children are provided with appropriate food and beverages while at child care. Most of the time there is enough food for children to have seconds if they are still hungry after receiving their initial portion of food. I spoke with Child A's Mother, Child B's Mother and Child C's Mother. They all indicated that they have never had concerns regarding their children not being provided proper meals or not being fed when they are hungry.
