Based on interviews with LT1, Teacher 1, Teacher 2 and Program Administrator, on 09/19/2025 staff provided appropriate care and supervision. LT 1 provided first aid, comfort and care when Child A was injured after being struck with a toy by another child.

Based on interviews with LT 1 and Relative of Child A, childcare center staff provided verbal report to Relative of Child A at pickup of Child A's injury. Staff interviews also indicate that parents were not notified of Child A's injuries. Also, upon reviewing the Parent Handbook, the center does not have a written plan for how and when a parent is notified of a child accident, injury or incident.
