    R 400.1911 · 1 R 400.1911 Care; supervision; children. (1) A licensee shall ensure appropriate care and supervision of children at all times.

    On 0311112021, I conducted a telephone interviewed with Child A's Father. He said, on 03/05/2021 Child A was picked up by mother. When A's Child Mother placed ■ in the car, she noticed that Child was not using left arm. Child A's Mother called Ms. Thomas outside and asked what happene o Child A's arm. Ms. Thomas denied knowing of any incident involving Child A hurting g arm. Ms. Thomas said earlier in the day, Child A ran into another child however, was okay, and the incident did not result in injury. Ms. Thomas looked at Child A an said that she did not see anything wrong. Child A's Mother left the child care home and took Child A to the hospital. Child A was diagnosed with Child A's Father called Ms. Thomas to informed her of the injury and s. "somas continue to act as if nothing happened. Child A's Father said he left a message for Ms. Thomas, requesting a meeting to discuss how the injuries could have occurred, however, Ms. Thomas has not returned his call. Child A's Father said the doctor informed him that the injured could have occurred in multiple ways and is not an indication of abuse. Child A's Father said he asked Child A is a friend pulled arm at the child care home. He said Child A told him that Ms. Thomas ``pulled me ng'. t here hard" and touch .d elbow. Child A's Father said last year Child A's Mother dropped Child A off at e home and Child A did not wanmt toy Mo. He also said that once Child A came home and said, "Ms. Thomas whooped hard." When Child A's Mother asked Ms. Thomas about. this, Ms. Thomas tried to clarify the statement by saying that Child A was trying to say that Ms. Thomas washed ■ mask. On 03111/2021, 1 conducted a telephone interview with the complainant. The complainant said Child A received a dislocated elbow while at the child care home. It is unknown how Child A obtained the injury and Ms. Thomas has refused to meet with the parents to discuss the issue. On 0311112021, 1 contacted and we agreed to conduct a joint on onsite inspection on /15/2021. On 03/1512021, and I conducted an unannounced inspection to the facility. We interviewed licensee, Sherrill Thomas, child care Staff member, Devonna Watson and Brianna Thomas, Child B -year-oId , Child C I -year-old.), Child D l..year.-old. and Child F -yearo ld . Ms. Thomas denied pulling on Child A's elbow. She said Child A fail out of~ chair while trying to sit down. Ms. Thomas said the chair is an inch and i from to round and all the children in care witnessed the incident. She said Child A fail on bottom and did not hurt ■ elbow. Ms. Thomas said Child C tried to help C i A up and they bumped heads. Ms. Thomas said Child A cried a little, however, was fine the rest of the day. Ms. Thomas said the incident happened at approximately 11:15 a.m. She said there were no other incidents that occurred with Child A on this day. Ms. Thomas said When Child A's Mother arrived, she informed her of the incident. She said Child A left the home and a few moments later. Child A's Mother called her, and she went out to the car. Child A's Mother told her that Child A's arm looked swollen. Child A's Mother asked Ms. Thomas to exam Child A, therefore, Ms. Thomas said she touch Child A up and down. arm. Ms. Thomas said she looked at Child A's arm, legs and heaad ev erything looked normal. She said she made sure Child A was able to move and showed no signs of discomfort or pain. She said she slightly squeezed Child A's arm and legs, and everything appeared to be fine. Child A did not cry and said was okay. Ms. Thomas said at approximately 5:53 .m. Child A's Father called yelling and saying things like, '= I'm going t you up if something is wrong with my Ms. Thomas said she was con, a 3ed because Child A was fine when left the child care home. Following her conversation with Child A's Father she tried to contact Child A's Mother to determine what was wrong. However, Child A's Mother instructed tier to speak with Child A's Father. The next day, Ms. Thomas texted Child A
