Based on interviews with Teacher 1 and Teacher 2 Child A has been allowed to sleep in a high chair on multiple occasions and Jacqueline Horne reported a single incident of Child A sleeping in a high chair.

On at least two known occasions, Child A was unsupervised while in care. One 07/30/2025, appropriate care was not provided when Jacqueline Horne self-admitted to becoming upset with Child A's and Child B's grandmother and in response, she did use profanity and derogatory remarks towards the grandmother in the presence of Child A and Child B.

Attendance records for staff is not being maintained as it was reported by Jacqueline Horne that staff are not required to sign in and out daily due to set schedules.

Teacher 2 has been employed at the child care facility since May 2025 and has not been deemed eligible under this license or connected.

The facility has operated outside of their approved age range of 2 1/2 to 12 years old. I observed two children under the age of 2 1/2, with one being enrolled March 3, 2025 at 1 year 9 months and the other being enrolled July 7, 2025 at 1 year 8 months. At the time of enrollment, Jacqueline Horne was aware of the approved age range.

During nap time on 07/30/2025, Jacqueline Horne was working alone and Teacher 2 was not near enough to immediately intervene if needed as she was on a lunch break off site.

An incident policy was not developed outlining a plan for how and when a parent is notified during an incident.

The department was not notified with 24 hours of two know incidents in which Child A was unsupervised.
