On January 18, 2023, I conducted a telephone interview with Child A's Mother. Child A's Mother said she had concerns regarding Child A's care at Hakuna Matutah Childcare Center. She observed a "mark" on Child A's index finger, on his left hand when she picked him up from school. She said she could not recall the exact date she saw the mark. The finger appeared to be smashed or burned. After taking a closer look, she realized Child A's fingernail was gone. She asked Child A's teacher, CCSM 2 what happened to Child A's fingernail, and she told her that she did not know. She took 2 Child A to and the doctor told her that Child A's finger was burned. She said she struggled with understanding how Child A's finger got burned considering she does not smoke. Child A's Mother said she misplaced Child A's discharge papers and that she would contact the hospital for a copy. Once she receives the copy, she will forward the documents to me. She could not recall the treating physician's name or the exact date she had taken Child A to the hospital. She said there was no aftercare treatment for Child A and that she was told the fingernail would grow back. She said she was going to seek another facility for Child A to attend. On January 19, 2023, I conducted an unannounced onsite inspection at Hakuna Matutah Childcare Center. During the inspection I interviewed child care staff members (CCSM) CCSM 1, CCSM 2, CCSM 3 and licensee designee, Stephanie Boyd. Stephanie Boyd said Child A has been enrolled in the child care program since October 10, 2022, however, he did not start participating in the program until January of 2023 because Child A was ill. Ms. Boyd said Child A's Mother called her on January 16, 2023, with concerns about Child A's left index finger. She said his finger looked burned and that she suspected the burn happened while he was at the child care center. In turn, she told Child A's Mother that the injury did not happen at the child care center. She said that there are no hazards in the media room and the media room is the only room Child A frequents. Furthermore, staff noticed his finger on January 11, 2023, during drop off. They spoke to Child A's Mother about his finger during pick-up on that same date. Staff did not generate an incident report because the injury did not happen at the child care center. Ms. Boyd said she documented the injury in her daily planner to keep a record. During Ms. Boyd exist conference on 2/23/2023 she said Child A has not been back to the program since the complaint was made. Child care staff member CCSM 3 was interviewed and said child A was dropped off at the child care center on January 11, 2023. CCSM 2 took Child A's coat off and noticed his nail was missing from his left index finger. The cause of his injury was unknown, but she poured hydroperoxide on his finger; put some ointment on it and a band-aid. Child A did not cry or appear in pain. She spoke to Child A's Mother about the injury during pick-up and Child A's Mother said she was going to take him to urgent care. CCSM 2 was interviewed and said she was a voluntary child care staff member. She has been volunteered at Hakuna Matutah Childcare Center for three years working from 8:30am to 3:15pm. On January 11, 2023, she noticed Child A's fingernail was missing from his left index finger. She asked Child A's Mother about the injury and was told he might have slammed his finger in the cabinet door at home. He plays with the cabinet doors in the kitchen when he is on the floor. said, Child A's Mother later alleged the injury happened in the childcare center. 3 Child care Staff member CCSM 1 was interviewed and said she noticed Child A's left index finger on January 11, 2023. CCSM 2 took his coat off and it looked like his finger was missing the fingernail. She brought him into the infant and toddler room. cleaned it up and put a band-aid on it.
