Lead Caregiver 1 and CCSM 1 provide appropriate care and supervision to toddlers in their classroom during circle time. There was insufficient evidence that the child care staff members are forcing toddlers to sit for 20 minutes during circle time. Charlene Han, Lead Caregiver 1, Lead Caregiver 3, CCSM 1, CCSM 2, and CCSM 3 all stated that the preschool children are redirected back to the circle time area, but if they do not comply, they are allowed to do another quiet activity in the classroom. During circle time, Lead Caregiver 1 is integrating music and movement, stories, and other developmentally appropriate physical activities into their whole group learning experience.

On 6/6/2022 I spoke to the Licensee Designee, Ms. Charlene Hans. Ms. Hans provided me with additional information and advised that the child care staff member that was responsible for the oversight, had been terminated. On 6/13/2022 I completed an onsite inspection at the Sunny Bunch Christian Childcare Center. During the inspection I interviewed the Licensee Designee Ms. Charlene Hans; the Program Director Ms. Cheyenne Smith; and the child care staff member Ms. Samantha Conley. Ms. Hans advised that there had two incidents within a short period of time, where the child care staff member Ms. Jennifer Campbell left two children unaccompanied on two separate occasions. Due to this, Ms. Hans reported that the child care staff member Ms. Campbell's employment was terminated; while the other child care staff member that was present with Ms. Campbell during the second incident, Ms. Samantha Conley was placed on a 45-day probationary period as a result of the incident. It should be noted that Ms. Hans provided me with an incident report on 4/26/2022 where Child B was temporarily left unattended while in the bathroom located inside the classroom. A special investigation was not initiated due to another child care staff member being present in the adjoining classroom connected to the bathroom where Child A was left. 3 While discussing the incident report drafted on 5/2/2022 pertaining to Child A being left unaccompanied in the foyer/coat room located at the entrance of the child care facility. Ms. Conley reported that she was the assistant child care staff member working in the AZ room on 5/2/2022. Ms. Conley stated that the former child care staff member Ms. Campbell, was working as the lead caregiver in the classroom. Ms. Conley stated they had a 12:2 ratio during the time the classroom lined up to walk to a splash pad located near the child care facility. Ms. Conley reported that the child care staff members perform two separate head counts of the children, once during the initial time when children line up, and again once they arrive at their destination. Ms. Conley disclosed that on 5/2/2022 she was located at the rear of the line while Ms. Campbell was positioned in the front of the line. Ms. Conley reported that she was holding two children's hands while walking and acknowledged that on 5/2/2022 she did not conduct a headcount aloud but stated that she took a "mental count." Ms. Conley said that it is the child care center's policy to call names during the head count to verify the child's presence. Ms. Conley reported that she is under the belief that Child A snuck out of the line after the count, prior to leaving the facility. Ms. Conley noted that they did not realize Child A was not with the group until they arrived at the splashpad. After noticing Child A was not present with the group, Ms. Campbell took out her phone to call the facility, at which point she noticed there were several missed calls from Ms. Smith. Ms. Conley stated that the total duration in which Child A was not in their care was approximately 10 minutes. Ms. Conley did identify Child A as a "wanderer," and reported that she would not be surprised if Child A had attempted to hide prior to the classroom leaving. Since the incident occurred, Ms. Conley reported that they have begun utilizing a different counting procedure prior to leaving the classroom. The children now sit down until they are called to stand up and wait in line, after the children are in line, they then perform the head count aloud, while cross-checking the attendance log. They have also implemented an additional count which is conducted immediately upon exiting the child care facility. She also maintained that the child care staff members then do a final head count aloud, once they arrive at their destination. Ms. Conley also reported that they have also begun to also implement a buddy system where the older children (she identified as the 4 and 5-year-olds) in the classroom will pair-up wit

On 5/25/2021 I made an onsite inspection at the Sunny Bunch Christian Childcare Center. During the onsite inspection, I spoke to the child care staff members Ms. Jennifer Campbell, Ms. Gabrielle Conley, and the Licensee Designee, Ms. Charlene Hans. Ms. Campbell stated that the incident that occurred between Child A and Child B happened on 5/19/2021. Ms. Campbell stated that she was in the "AZ room" and she witnessed Child A walking to the bathroom with Child B following behind. Ms. Campbell stated that as soon as she saw the door close behind the two children, she approached the bathroom, knocked at the door and at the same time she was pulling the door open. She reported that when she entered the bathroom, she saw Child A and Child B standing in the corner of the bathroom behind the door. Ms. Campbell reported that both children were fully clothed when she entered the bathroom and noted that the door was only closed for approximately 10-15 seconds before she knocked and opened the door. Ms. Campbell stated that upon entering the bathroom she noticed that the lights were off, and she asked what had occurred. Ms. Campbell stated that while Child A would initially not say anything, she later made the disclosure that Child B "told her to do something crazy." Ms. Campbell reported that the incident occurred later in the day after the two classrooms merged. Ms. Campbell denied that there had been any previous 3 concerns regarding Child B, and noted that the child care staff member, Ms. Gabrielle Conley was also present in the room at the time of the incident. It should be noted that there was a discrepancy noted in the incident report and what was relayed during the interview with Ms. Campbell. Ms. Campbell was adamant that both children were fully clothed and that they had only been in the bathroom together for a few seconds before she arrived. However, the incident report stated that Child B had his pants down when Ms. Campbell entered the bathroom. Ms. Campbell denied this and stated that while she did not draft the incident report, the incident report was written by the Program Director, Ms. Cheyenne Smith, after providing her with an account of what transpired. Ms. Campbell suggested that there was a lapse in communication as both children remained fully clothed. Ms. Conley informed me that she was a relatively new employee at the child care facility, and that she has only worked there for less than one month. Ms. Conley denied witnessing the two children enter the bathroom together until after this was brought to her attention from Ms. Campbell. Ms. Conley denied having any concerns or issues related to Child A or Child B in the past, but she acknowledged thinking it was odd that Child B had taken an interest in Child A prior to the bathroom incident, as she noted they were previously playing together. Ms. Conley stated that it appeared strange to see the two playing together, as there is a 4-year age difference between the two, and since there were older children that Child B could have been interacting with in the classroom. Ms. Conley stated that since the incident had occurred, Child B has refused to play or interact with any other children in the classroom. Ms. Hans reported that there have been some issues with Child B in the past, but she noted that this was typically just "general disobedience," denying she has ever had concerns of inappropriate interactions between Child B and any other child care children. Ms. Hans also stated that with Child B being one of the older children, he does not like attending, and often makes statements that he wished he did not have to come to the child care facility. I did make an inquiry in the discrepancies mentioned in the allegation on the incident report and what Ms. Campbell had disclosed.
