On 07/12/2023, Program Director Marci Dahlm left me a voicemail and also emailed me to notify me of an incident that occurred on 07/11/2023. She reported that when the school age children left the gym to go to the fellowship hall for breakfast, Child A (six years) was using the restroom and was left unattended in the gym. Child Care Staff Member (CCSM) 3 heard her crying and went to get her and take her to the fellowship hall where CCSM 1 and CCSM 2 were located. They were not aware that Child A had been missing. I conducted an unannounced onsite inspection on 07/14/2023, and interviewed Program Director Marci Dahlm, CCSM 2 and CCSM 3. Marci Dahlm explained that there were thirteen school aged children and two child care staff members present the day of the incident. The children were preparing to go to breakfast, washing their hands as well as using the restroom. Child A had gone to the restroom followed by three other children, who came out shortly after going in. CCSM 2 asked the three other children if they were the last ones in the restroom and they replied that they were. Marci Dahlm stated that CCSM 1 and CCSM 2 then counted all the children and compared their count with Brightwheel app, which houses their attendance. The staff must have miscounted because they counted 13 children which is what Brightwheel reported, then they left the gym. CCSM 3 was in her office which has a window that overlooks the gym when she heard crying and checked out the window seeing Child A alone in the gym. CCSM 3 immediately came to Child A, consoled her, and took her to the fellowship hall with the other children. CCSM 1 and CCSM 2 did not realize that Child A had been missing. I reviewed the attendance records from 07/11/2023 which confirmed 13 children present and two child care staff members. Since the incident, Marci Dahlm reported that staff now use a checklist tracking children at the beginning and end of a transition making a circle next to their name before leaving a space, and then adding a check when they arrive to the new space. Program Director Marci Dahlm explained that a similar incident occurred with Child A about a year prior, with a former staff member. At the time, Staff counted children as they left the gym and as they entered the Fellowship Hall. They counted children and noticed Child A was missing immediately going back to get her in the gym. Marci Dahlm did not report the incident to the department, adding that she was not aware something that happened that fast was considered an incident. I provided Technical Assistance, explaining that while staff immediately noticed Child A was missing as they were transitioning, Child A was left unattended, and incidents should be reported to the department. I added if she ever has a question of what needs to be reported to feel free to just reach out and ask me. CCSM 2 explained that she was on the opposite side of the gym with the boys when the incident occurred on 07/11/2023. After all children appeared to be done in the 3 restroom and they prepared to leave the gym, CCSM 2 reviewed Brightwheel and saw 13 children present. Both CCSM 1 and CCSM 2 counted 13 children and assumed that all children were accounted before leaving to go to the fellowship hall. CCSM 2 reported that when CCSM 3 brought Child A in she had not started serving breakfast yet and felt that "only about two minutes had passed." Child A seemed to calm down fast after the incident and was talking about it with her friends at the table. CCSM 2 added that since the incident, the staff members will add children's name to a list as they arrive for the day to use for transitions. When leaving a space staff will call a name and add a circle next to their name, and then when arriving to a new space staff will call a name and add a check to the circle. This has allowed them to see that all children are accounted for before leaving and then when arriving to the new space. CCSM 3 reported that she was in her office on th
