On 10#1412020, 1 received a phone call from regional manager Christy Finch and corporate partner Shelly Odell informing me that Child I's Mother had contacted the center on 10/12/2020 and spoke with Child Care Staff Member (CCSM) 5 reporting that Child I had disclosed that CCSM 1 touched Child I and Child A were talking about why CCSM 1 and Former Program Director Lacey Hendrick had been fired from the center. Child A told Child I it was because Ms. Lacey said it was ok for CCSM 1 to touch peoples privates". This conversation "tri ered something" in Child I, and Child I disclosed that "CCSM 1 had hands on (Child I's) privates while was taking a nap. Child I said one time it urt bad N(CCSM because 1) Ms. Finch and Ms. Odell inquired about what they should do next as they were not sure because CCSM 1 no longer worked at the center as a result of the previous special investigation SIR 2020D0248013. Child I had also been interviewed for the previous investigation and because it was an "open cas& they did not know how to proceed next. Ms. Finch added that the details that CCSM 5 received from Child I's Mother were "different" and there was "a lot of confusion" so she and Ms. Odell wanted to follow up with Child I's Mother the following morning of 10/13/2020. 1 explained that they should contact centralized intake since this is a new allegation of sexual abuse and there would most likely be a new special investigation opened. On 10115/2020, 1 contacted Child I's Mother and interviewed her via telephone. Child I's Mother reported that on Monday,10/12/2020, Child I told her that had spoken with Child A while at school. Child I told her about their conversation. ild A told Child I that CCSM 1 and former program director Lacey Hendrick had both been fired from the child care center, and it was "because Ms. Lacey (Ms. Hendrick) said it was ok to touchprivates." Child I then told mother that CCSM 1 "and that the last time it haDDened was uniia is morner sate inat unuua t reportea tnis occurrea ••aooui'tv times" aunng me months before COVID, when Child I was in the Pratt room. Child I's Mother said that she felt that Child I's conversation with Child A seemed to "jog" ■ memory and made recall the incidents. She added that she believes that CCSM 1 may have rubbe ild I's back to put to sleep and while asleep the incidents took place, and Child I was in a dream i e state. Child I's Mother reported that after the interviews in June from the previous investigation, SIR# 2020D0248013, she and Child I's Father did not really talk with Child I about the incidents as they did not want On 10/19/2020. I conducted an unannounced onsite inspection with During the investigation we interviewed program director Lauren Jamie who had lust started working for the center on s. Jamie exDlairied thatshe had received a "rundown and read a previous inves igation's report." She added that she was under the impression with the last investigation that a parent had witnessed something that they thought was inappropriate. Children were interviewed and it "didn't seem that anything inappropriate happened per the children interviews." 1 explained that the investigation ended with four separate incidents of "inappropriate contact" with four children. Ms. Jamie was aware of the phone call from Child I's Mother that CCSM 5 received on 10/1212020. She said that the call came in later in the evening and because CCSM 1 no longer worked at the center and the child was no longer in care, she then contacted the corporate office. We asked Ms. Jamie if that was the normal reporting procedures for a report of alleged sexual abuse at the center. Ms. Jamie explained that they were not sure what to do because it was "the same thing" as the first investigation. I explained that it was the same perpetrator, however this was a new allegation, incident, and child and not the "same thing." 4 We asked what staff members worked with CCSM 1 during the Jan-March 2020 timeframe. The staff membe
