On 11/10/2022, I received an incident report from Assistant Director Katey Jordan, notifying me that Child A was fed an incorrect bottle. Ms. Jordan stated that the bottle was labeled however, Child Care Staff Member Kaitlyn Townsend "made a mistake" and did not double check the label prior to feeding Child A. The mistake was noticed before the child finished the bottle and Child A's parents were immediately notified. On 11/14/2022, I contacted and interviewed Child A's Mother. Child A's Mother reported that she was called immediately and told that the bottles were similar and were being warmed at the same time on the counter. The staff accidently grabbed the wrong bottle to feed to Child A and caught the mistake after he had ingested about an ounce of milk. Child A's Mother said that the center had contacted the other parent to be able to explain any medical or health concerns, adding that she had no concerns after speaking to her doctor and Child A's pediatrician. I asked Child A's Mother if she had labeled Child A's bottles on the day of the incident, and she explained that the child care center provides labels for the child's first and last name that stay permanently on the bottle. On 12/1/2022, I conducted an unannounced onsite inspection and interviewed Program Director Jordanne Waite and child care staff member Katelyn Townsend. Ms. Waite was on maternity leave when the incident occurred. She explained that since the incident, staff were retrained on bottle labeling as well as using a process to double check the name on the bottle with the child being fed. Staff are now expected to read out loud the child's name when removing a bottle from the refrigerator, prior to feeding the child, as well as confirm the name of the child, to the face of the child in the staff members arms. Child care staff member Katelyn Townsend reported that she was fixing a bottle for a child when she noticed another bottle on the counter being warmed, that was not labeled. She became sidetracked when she stopped to make sure the bottle was properly labeled while continuing to prepare a bottle and then a number of children started getting fussy for their bottles. Ms. Townsend, stated that she quickly picked up Child A, and grabbed one of the three bottles from the warmers without looking and checking the name, and started feeding Child A. After about an ounce of milk ingested, Ms. Townsend quickly realized that the bottle was not Child A's bottle and stopped feeding Child A. I asked Ms. Townsend what her usual practices are when feeding bottles to children. She usually will say the name out loud when grabbing the bottle and then will double check when putting on the warmer and when removing. On the day of the incident, Ms. Townsend said she simply made a mistake and was trying to do too many things at one time and did not double check herself. Since the incident, Ms. Townsend checks herself by verbally reading out loud the name of the child on the bottle when getting it from the refrigerator, the warmer and before feeding. On 12/15/2022, I interviewed Child B's Mother, Child C's Mother and Child D's Mother. No parents expressed any concern with the labeling of bottles or bottles 3 S

On 07/22/2022, Assistant Program Director Katey Jordan notified me of an incident in which Child A, 18 months old, was left unsupervised in the room for about five minutes. Ms. Jordan sent the required incident form to me on 07/25/2022. I contacted Child A's Father and interviewed him regarding the incident. He reported that he had been notified shortly after the incident that Child A was fine and was playing with toys in the room "unaffected." He stated that it was the only time something has happened and that he has no concerns with care and supervision at the center. On 07/27/2022, at 10:00am, I conducted an unannounced onsite inspection and interviewed Assistant Program Director Katey Jordan. Program Director Jordanne Wait was out of the center for maternity leave and Ms. Jordan was serving as substitute program director. Ms. Jordan reported that while staff in the Lyric (toddler 18 month) class were transitioning from inside to the playground, Child A was left inside the classroom. Child Care Staff Member Jayden Fields was walking in the hall and saw Child A in the room unattended. She yelled down for Program Director Jordanne Wait, who went out to the playground to ask the Lyric teachers to count their children and inform them that Child A was inside unattended. The staff members were unaware they were missing Child A and reported that they had been outside for about five minutes. Ms. Wait discussed with the staff members if they completed their "name to face" process as they exited the room where they look at a child and say their name as they check their names on their checklists, or if they had used the tablets to electronically check children in and out of the room. The staff stated they had not completed these processes. I also interviewed Child Care Staff Members Destiny Maio, Cameron Reeves and Jayden Fields. Ms. Maio reported that she had just arrived to the center right before the incident happened, and that she is usually in a different classroom. When she arrived, all the children were dressed and ready for water day outside, and the class was lined up and in the processes of going outside. Ms. Maio stated that she came in the room and did not take the time to look at the name to face sheet or look at the tablet to see the children that had been checked into the room that day. She and Ms. Reeves took the children outside, counted the children and the number of children counted aligned with the number of children on their name to face paper sheet, so she was not aware Child A was missing. Ms. Maio reported that she feels that Child 3 A may have been hiding in the room, and while she usually does a lap around the room to look at all blind spots before leaving, on that day, she did not. Since the incident, Ms. Maio explained that she sets alarms to remind staff to do name to face, complete a "sweep" through the room when exiting and she checks the tablet regularly to ensure all children are accurately checked into the rooms. Child Care Staff Member Cameron Reeves reported that on 7/22/22, she and Ms. Maio were in the room with what they thought were six children, however there were actually seven. Ms. Reeves explained that neither her nor Ms. Maio had compared the "name to face" sheet that is kept on paper, with the tablet showing children that had been checked into the center. Child A had been checked into the center and it showed on the tablet there were seven children, however on the name to face paper, he had not been checked in which showed six children. Ms. Reeves also explained that they should have run through the process checking children in and out of the room on the tablet. She explained that as the children leave the room, the staff members "move the children from inside to outside" on the tablet app while also completing the name to face worksheet allowing all lists to be compared and any discrepancies to come to light.
