The center's heating system began to send signals of faulty functioning on 12/11/2025. Upon arrival the center in the morning of 12/12/2025, the heating system was not functioning properly and was serviced. The center provided documentation that the lowest reading was 64.9, however after repair on 12/12/2025, it rapidly rose to 70 degrees by 10:00 AM. On 05/11/2023, I spoke with Child A's Mother who received a telephone call from program director, Megan Porada, on 05/05/2023 and was informed of the incident. Child A's Mother was informed that Child A and another child were playing mom and dad games. Child A was on top of Child B and their faces were pressed against each other. Child A's Mother expressed concern 2 regarding supervision, how the children were able to get to the point that they were laying down and nobody saw it. Child A's Mother did speak with Child A regarding the incident and discussed with him about not putting hands on anyone. Child A's Mother indicated that Child A will remain enrolled in the center. However, during the 06/28/2023 exit conference, Mr. Chapman indicated that Child A is no longer enrolled. On 05/11/2023, a telephone call and message were left for Child B's Mother. I ultimately spoke with Child B's Mother on 06/23/2023. Child B's Mother was contacted by Ms. Porada and told her briefly and that Child B was caught kissing another student and that was pretty much it. Child B's Mother was told mom that Child A and Child B were kissing and not so much cheek to cheek. Child B's Mother described Child B to be shy, but that she told a family member that they were kissing and paying mommies and dads. Child B's Mother would have liked to have been provided additional detail, such as how long was this going on, had it happened before, and what steps are being taken to ensure that it does not happen again. She wants to be sure that the children are being watched. Child B's Mother did have a conversation with Child B about what is appropriate and not. Child B remains enrolled in the center and has since moved up to a different classroom. I completed an unannounced, on-site inspection on 05/15/2023 and first spoke with child care staff member 1 (CCSM1). CCSM1 has been working at the center since 02/2023. CCSM1 described that on 05/05/2023, she was finishing up a craft with some of the children and at the table. Child care staff member2 (CCSM2) and child care staff member 3 (CCSM3) were on the carpet area with the rest of the children, who were playing during free choice time. The ratio in the room at the time was three child care staff members to 10 children. There were several children in the house area. When CCSM1 began to clean up and walked over by the house area she saw Child A and Child B's legs sticking out. By the time CCSM1 walked over to them, the children were cheek to cheek. When she asked the children what they were doing, Child A said, "playing mom and dad" and Child B responded "yea." CCSM1 told Child A and Child B that they were not going to be playing that. CCSM1 does not know how long Child A and Child B were laying on top of each other. I also spoke with CCSM2 on 05/15/2023. CCSM2 has been working at the center since 04/14/2023. CCSM2 was present on 05/05/2023 during the incident and was playing with the children on the carpet. CCSM2 indicated that CCSM3 was also present and on the carpet with the children. CCSM2 did not see Child A and Child B laying on top of each other. She was not aware of the situation until she heard CCSM1 look over at them and them to stop. I then spoke with CCSM3 on 05/15/2023. CCSM3 has been working at the center since 05/03/2023. CCSM3 indicated that she was made aware of the incident involving Child A and Child B, but that she did not witness it. She shared that she was not sure if she was in the room when it happened because she was going in between the butterfly 1 and butterfly 2 classroom. 3

On 12/15/2022, the center self-reported the incident involving Child A on 12/14/2022. Program director, Meghann Porada, reported two classrooms (Butterfly 2 and Butterfly 3) combined at the end of the day while on the playground. CCSM1 and CCSM2 remained outside on the playground with a ratio of 2:15 and at 4:00PM transitioned back inside. Witness 1 was on her way to the parking structure and saw Child A child outside and alone. Witness 1 brought Child A back into the center and to the office. Ms. Porada had already left for the day, however CCSM3 was still at the center. CCSM3 went to the classroom to talk with CCSM1 and CCSM2 to have them count their children to see how many children they had. They did not realize Child A was missing until CCMS3 went to their room. Child A was still in the office at this time and was ultimately taken back to the classroom. Ms. Porada reported that approximately 15 minutes had passed before they noticed. Ms. Porada confirmed that there are not cameras inside or outside of the center. On 12/20/2022, I privately spoke with CCSM1 who has been working at the center since September 2022 and usually in the afternoon, from 1:00 to 6:00 PM. She also typically worked with CCSM2. They typically take the children outside around 3:30 PM and come in a little after 4:00PM. CCSM1 believes they had 14 children that day but could not remember the exact number. When the classroom goes outside, they have their own children, however they also combined with Caterpillar 1 and Butterfly 1 classrooms who were already outside. CCSM1 believes there are usually approximately 30 children when they are all outside together. The process for transitioning back inside is to complete a headcount of the children. CCSM1 did not realize that did not happen when they transitioned to come inside on 12/14/2022. I asked CCSM1 when they realized that Child A was not with the class and she indicated it was not until CCSM3 came, which was almost 4:30 PM. CCSM1 did not realize that Child A was not with them because they did not know he was unaccounted for. When asked where Child A was when the rest of the class came inside, CCSM1 was not sure and guessed he ran to a blind spot behind the shed when they were not looking. CCSM1 shared that it was only her and CCSM2 on the playground. She asked CCSM2 if he counted the children before they came inside and CCSM2 stated he did not, so counted when they got back inside. I asked CCSM1 who found Child A on the playground, and she believed in was CCSM3. She believes that CCSM3 saw Child A though the office window, which was facing the area Child A was in. The method for ensuring the right count if children is to use Brightwheel, which CCSM1 described as unreliable, so they must visually count the kids. Brightwheel is on the staff members phones and parents are supposed to sign their children in and out, but they do not always do that. Staff are not supposed to be on their phones when working, so it is hard to look at Brightwheel. CCSM1 indicated there are tablets with Brightwheel, but that their classroom did not have any. Another method 3 that is to be used is to write the number of children present on the door, however, that is supposed to change when a child leaves and CCSM1 reports that was not happening either. When asked why this happened and where the break was, CCSM1 relayed different reasons. The first was the additional children who were added to their class count when combined outside on the playground. CCSM1 and CCSM2 were not paying enough attention to the count or lining up. CCSM1 indicated that she nor CCSM2 were properly trained. People assume that CCSM2 knew what to do because of working at another child care center. CCSM1 indicated that she is still not sure what to do with transitions but will utilize Brightwheel to see what children are signed in and will count before transitions. However, expressed frustration that if Brightwheel is not accurate, then the count will be off.

On 11/30/2022, I received a telephone call from licensee designee, Josh Chapman, to report an incident involving a supervised volunteer caregiver, Tamara Olson. On 11/29/2022, Ms. Olson was observed to pull Child A's hair. Child A then later had an accident and urinated on her cot at nap time because Ms. Olson told her that she could not use the bathroom. There was a 2nd incident on 11/29/2022 when Ms. Olson was observed to forcefully redirect Child B to his cot when he was trying to get off his cot and run around. In doing so, Child B bumped his head on the shelf that was behind his cot. Ms. Olson's volunteer services were terminated by the YMCA on 11/29/2022. On 12/12/2022, I completed an unannounced, on-site inspection and spoke with licensee designee, Josh Chapman. Mr. Chapman explained that the volunteer caregiver, Tamara Olson, has been a supervised volunteer with the center since March 2022. Ms. Olson participates with the Council on Aging Foster Grandparent program. Ms. Olson is never alone with the children. I reviewed Ms. Olson's file which obtained DHHS Central Registry Clearance, Sex Offenders Clearance, FBI Fingerprint Clearance, State of MI Criminal Clearance, pre-placement physical exam and TB test, all completed by the Council on Aging. Additional documents in Ms. Olson's file as completed by the YMCA included the agencies volunteer application, verification of Bloodborne Pathogen training, mandated child abuse reporter training, YMCA confidentiality agreement, and a child abuse prevention code of conduct. Ms. Olson signed the YMCA child abuse prevention code of conduct on 03/24/2022. On 12/19/2022, I privately spoke with program director, Meghann Porada. Ms. Porada indicated that Ms. Olson has always worked in the GSRP classroom, however, there have been different child care staff members in there. Ms. Olson was scheduled Monday to Thursday from 9:00 AM to 1:30 PM. Her duties were to help with classroom activities and engage with the children. Ms. Porada had the opportunity to observe Ms. Olson interactions with the children and did not observe anything that was concerning. When asked how the children responded to Ms. Olson, Ms. Porada described it to "not be very grandmotherly." On 11/29/2022, at 12:15 pm, it was brought to her attention by lead caregiver, Samantha Dickson, that Ms. Olson was observed to pull Child A's hair. Ms. Porada went to the GSRP classroom and spoke with Ms. Olson and told her that she was not able to touch the children. Ms. Olson did finish out her shift on 11/29/2022. Ms. Porada then contacted the Council on Aging and left a message for the foster grandparent coordinator, Jason Crawford. 3 Mr. Crawford contacted Ms. Porada on 11/30/2022 at which time she shared that Ms. Olson could not return to the center due to the incidences involving Child A and Child B. On 12/21/2022, I completed an unannounced on-site inspection and first spoke with program director, Meghann Porada. Ms. Porada confirmed that the first incident was brought to her attention at 12:15 PM involving Child A. She found out about the incident involving Child B at 3:15 PM and after nap time. I asked to speak with lead caregiver Samantha Dickson, however she is out indefinitely on a medical leave. Ms. Dickson did complete a written statement on 11/29/2022 which stated: During bathroom breaks, Child A was playing by the bathroom door. I asked Child A "did you wash your hands?" Ms. Olson responded, "no she did not, she was playing with her hair" and proceeded to grab a handful of Child A's hair. I immediately intervened and took Child A aside to make sure she was okay. I then told Ms. Olson that we have a no touching policy at the YMCA and that under no circumstance should we ever put our hands on a child. I then went to my director to let her know. I then privately spoke with child care staff member, Courtney Meldrum, assistant in the GSRP classroom with Ms. Dickson. Ms. Meldrum became a permanent assistant in the GSRP cla

On 2/17/2022, 2/18/2022 and 2/21/2022 I attempted to call Child A's mother, no answer a voice mail was left requesting a call back. On 2/18/2022, I attempted to call Child A's mother, at a second number, and the individual indicated Child A's mother no longer received calls at this number. On 2/22/2022, I completed an unannounced onsite inspection at Warm World Early Child Care Center 2. I interviewed the Program Director Megan Pagoda. Ms. Pagoda indicated she was not present at the time the incident occurred. Child care staff member two (CCSM 2) notified her when she arrived at the center. Child A had been dropped off by his father and had a difficult drop off. He exited the classroom which had the door propped open and went into the hallway looking for his dad. He was crying and wandered down by the office. This occurred around 8:00 AM, there were only three children in the classroom, and Child care staff member one was in the bathroom with another child. The hallways do have cameras, and she reached out to licensee designee Joshua Chapman to let him know I would like to watch the video. Ms. Pagoda became the program director about a year ago but has been employed at the YWCA for about four years in totality. Ms. Pagoda already removed the door stoppers, and they will no longer be used. Child care staff member one (CCSM 1) has taken an active supervision class for toddlers, and the certificate was provided to me. All staff members came in on the following Saturday for an in-service day of training regarding this incident. Certificate, time sheets for staff, drop off sheets for children, and child information cards were provided to me. I interviewed Child care staff member one (CCSM 1), who reported she had been employed at the center since August of 2021 and has been the lead teacher in this classroom the entire time. Child A was having a rough drop off and it is reported mom and dad are having some marital issues, and it has affected Child A. Already before the main incident happened Child A followed dad out of room into the hallway and was crying, but dad was still in the hallway, and CCSM 1 had followed him out. Child E and Child F were also present, and were in the bathroom, due to one of the children having an accident and needing to change. The door was propped open but is not normally propped open and so she thought the door was closed and that the children would just be in the classroom while she was assisting in the bathroom (Inside the classroom). There is usually an activity already set out for the children to be doing while morning drop off is occurring. There is an assistant who comes into the classroom but does not usually get there until after 8:00 AM on Tuesday's. When another parent came into drop off their child, they indicated they thought one of her children were in the hallway, as CCSM 1 was coming out of the bathroom. Child D's mom asked if "my little guy" was in the hallway, thinking she meant CCSM 1's biological son who also attends the school she replied "No, he isn't," she did not know she meant someone from the classroom she was responsible for. CCSM 1 reported she looked out the door of her classroom and 4 saw her co-worker who was with Child A in the hallway. Child A would not go with Child care staff member two (CCSM 2) (CCSM 2), so she came to her classroom and CCSM 1 CCSM 1 went down the hallway to get Child A. Once Child A was back in room, the door was closed, and they had a discussion about staying in the room to be safe and not going in the hallway without their teacher. CCSM 1 believes Child D's mother talked to Ms. Pagoda about the incident, and Ms. Pagoda did meet with her to discuss the issue. On 08/31/2020, , and I made an unannounced, on site inspection to the center. We arrived at AM and first spoke with Licensee Designee, Denise Brooks. Ms. Brooks indicated that on 08/28/2020, was doing some observations in the toddler room when Assistant Caregiver, Melissa Morell, mentioned to her that had spanked Child A. Ms. Brooks was not aware of the specifics, such as if Ms. Morell had witnessed this or heard about it. Ms. Brooks stated that it surprised her to hear that raised her hand at a child. It might have looked bad, but I kept him from hurting another child. 2) The kids in the toddler room always have an "owie". They play rough and they hit each other. 3) 1 am a loud person. I speak loudly and no matter how quiet I try to be, it come across as loud", then sent an email to Licensee Designee, Lynnette Kraklan, on indicating that at the YMCA. In her indicated tha am exhausted and I do not want to believe one of my co-workers is after me. I do not want to keep looking over my shoulder thinking somebody's spying on me and that I am out to hurt the kids". Ms. Morell stated that she only worked with a couple of weeks after quarantine before she . however indicated that she has never witnessed be physically iñàjpropriate with the children. Ms. Morell stated that always sounded mean, not abusive, and that is how she even was when children were not around. Ms. Morell stated that she had a concern tone, as well as her expectations of the children. Santree did state that on 2/10/2021 while Child A was sitting af he lunch a e, around 11:15 AM, tipped sideways and fell out of chair. When she picked Child A up, Ms. San ree did notice that it looked like Chi A had bit the top of lip. Ms. Santree was putting lunches down on the table and walking toward the to a where Child A was sitting. Ms. Santree stated Child A was in the middle chair when tipped over. She saw it happen and stated that Child A did not hit■ head, r not cry, nor did ■ hit■ mouth on anything but Ms. Santree assumed bit it when she saw the red mark. Ms. Santree stated that she did tell Child A's o er about that when she picked ■ up and that Child A's Mother giggled about it. We asked Ms. Santree regarding scratches on Child A's face. She confirmed that there were scratches on Child A's face, but that she did not notice them until around 2:30 PM when they were in the tumble town room. Ms. Santree had child care staff member, Marley Tejeda, also look at them. The scratches looked to be older and scabbing, which made them think that they occurred at home. Ms. Santree denied that there were any issues in the classroom that day with children fighting, screaming, or any scuffles over toys. Ms. Santree denied that there are children in the room who scratch on a regular basis. The only incident was when Child A fell out of■ chair. Ms. Santree reported that Child A's Mother and another woman came to the center on 02/11/2021 around 12:30 PM expressing concern regarding the marks on the side of Child A's face. Child A's Mother stated that she had noticed 3 the marks when they went to Target and wanted to be sure that no one thought that she was causing the scratches on her . Child A's Mother denied that the scratches occurred at home. Ms. Santree stated that Child A was already at the center on 02/10/2021 when she arrived at _ and that no body had noticed any scratches at that time. E We asked Ms. Santree regarding the allegation that Child A had bruises or a red mark on ■ foot. Ms. Santree explained that Child A had socks on ■ feet the entire day and that she did not notice anything on ■ feet. hild A's shoes did come off at nap time and when in tumble town, but socks remained on. Ms. Santree described Child A's shoes to be little Nikes. The shoes did not have ties or velcro, but rather the slip-on kind. Ms. Santree did report that they were a little snug and difficult to put on, however Child A did not seem to be in pain and never complained about ■ feet. Regarding reporting accidents or injuries to parents, Ms. Santree explained that minor incidences are logged in the Brightwheel app and parents are also verbally notified at pick up. The Brightwheel app sends the parent a notification and they are able to communicate through the app. Incident reports are completed in the Brightwheel app the day of the incident, however the minor ones for bumps and scratches may not be entered until nap time or the end of the day depending on how busy they are. Parents do not sign incident reports; however, they do have to check their child in and out through the Brightwheel app, therefore it is believed they would see the notification.
