Ms. Simpson admitted that she will flick Child A on the lips if he gets "mouthy". She also admitted that she flicked him on the lips on 10/14/2025 after he told her that he yelled at his teacher at school.

On 04/27/2023, I made an unannounced on-site inspection to Ms. Simpson's home. I arrived at 11:30 AM and Ms. Simpson was present with four child care children. I reviewed the allegations in the complaint. Ms. Simpson stated that as a standard of care, she does not swear in front of the children. She does admit to typically say "crap," and there may have been moments when a swear word has slipped. She could not say that it has never happened but stressed that she does not go around swearing in front of the children. I asked Ms. Simpson if she talks about personal information with other families, to which she denied. Ms. Simpson has a family friend who will has present when families pick up their children and may hear conversations. However, Ms. Simpson does not talk about her enrolled families with others. Ms. Simpson has developed a parent handbook, which is not a licensing requirement of home-based child care providers. There is a statement in the handbook which is titled: Other Goodies Confidentiality; The information you supply to (Goin to Grandma's) childcare will be kept confidential. I will, at all times, respect your privacy. Before any of the information is released to outside persons, the parents will sign a release form. On 05/03/2023, I spoke with Child A's Mother who said there has been times she has heard Ms. Simpson swear in front of the children, however, was unable to provide specific details. Child A's Mother expressed concern regarding any Health Insurance Portability and Accountability Act (HIPPA) violations if Ms. Simpson is discussing personal information with other families. I discussed with Child A's Mother that HIPPA was created to protect sensitive medical information from being shared without patient consent and that child care providers are not necessarily bound to HIPPA laws. On 06/08/2023, I spoke with the following: • Child B (2 year old, male)'s Mother who shared that Child B has been 1/2 enrolled since March 2022 and attends 5 full days a week. Child B's Mother has not heard Ms. Simpson swear around the children, nor has she known her to talk about personal information regarding other families. • Child C (5-year-old, female) & D (11-year-old, female)'s Guardian who shared that Children were enrolled in Ms. Simpson's child care from May 2022 to May 2023. The only reason they no longer attend is because Child C and D's Guardian is no longer working. The children were attending 5 days a week from 8:30 AM to 5:15 PM. Child C and D's Guardian denied ever hearing Ms. Simpson swear in front of the children. 3 • Child E (6 year old, male)'s Mother indicated that Child E has been 1/2 attending Ms. Simpson's child care for over 1 year and attends 3 days a week. Child E's Mother has not heard Ms. Simpson swear in front of the children. She described Ms. Simpson to be transparent, and a swear word may have slipped in a conversation but she has not heard swearing that is constant and ongoing. • Child F (6 year old, male)'s Mother denied hearing Ms. Simpson swear in 1/2 front of the children. Child F has attended her child care for almost three years, Monday to Friday. • Child G (3 yar old, male). Attempts were made to contact Child G's 1/2 Mother, but a return call was not received at time of report completion.

During the 04/27/2023 on-site inspection, I asked Ms. Simpson if there were any children in the process of toilet training. Ms. Simpson indicated that currently, only Child A. The process is to use either regular underwear or pull ups, it depends on what the parent prefers and provides. If the child has too many accidents and without extra clothing, then she will put diapers on them. Ms. Simpson prefers not to do that, but shared she does not have a choice when the parents do not provide extra clothes. When that happens, Ms. Simpson informs and reminds the parents of the need to bring extra clothing. As she is working on toilet training, Ms. Simpson will take the child to the bathroom throughout the day and ask if they must use the bathroom. Ms. Simpson denied having any issue with toilet training any of her child care children and that it typically goes smoothly. Ms. Simpson denied that she spanked Child A or any other children for having an accident. She understands that children are going to have accidents as they are toilet training. Ms. Simpson stated that Child A is very quiet, does not talk much, and when she asks him questions, he will shake his head yes or no. Ms. Simpson will ask Child A every 30 to 45 minutes if he needs to us the bathroom and he usually says no. She can tell by his face and actions if he needs to go the bathroom 4 and will walk with him into the bathroom. Child A will not walk to the bathroom alone and use the toilet. Ms. Simpson has in her parent handbook a section for toilet learning which states: "I will assist you in toilet training your child with the understanding that it will be successful only if we work together. I will use cotton underwear or pull-ups supplied by the parent. Send your child ONLY in easy on/easy off clothing until they are able to completely undress and dress themselves. I require at least 5 complete changes of clothing during toilet learning. I do not launder soiled items and will send them home in a plastic bag. Please replace any clothing sent home the next day." Ms. Simpson stated that she has verbally asked Child A's Mother to bring extra clothing, but she does not always remember. Then if Child A does have an accident, Ms. Simpson does not have a choice other than to put a diaper on him. Ms. Simpson provided written verification that Child A's Mother received a copy of Ms. Simpson's parental handbook on 09/16/2021. On 05/03/2023, I spoke with Child A's Mother who said when she would pick up Child A he would be in diapers. They live only live 10 minutes away from Ms. Simpson and by the time they got home, Child A would have dry diaper of poop. Child A's Mother recalled this happening on more than one occasion. Child A's Mother was concerned as to why Child A was having accidents at Ms. Simpson's home because he did not when he was at home. When Child A's mother questioned Ms. Simpson if the toilet is being offered, Ms. Simpson started screaming at her that he is being offered the toilet. It was the weekend prior when Child A told her that Ms. Simpson hit him for having an accident. She questioned Ms. Simpson about what Child A said and Ms. Simpson responded by telling her that kids make stuff up. Child A's Mother does not believe Child A would make that up. I asked Child A's Mother if she had observed any marks or bruises on her son after being at Ms. Simpsons home and she stated that she had not. On 06/08/2023, I spoke with the following: • Child B (2 year old, male)'s Mother reported to having no concerns with 1/2 inappropriate discipline with her son and Ms. Simpson. • Child C (5-year-old, female) & D (11-year-old, female)' Guardian stated that she has "absolutely no concerns" with regards to discipline issues. The children are able to use the bathroom whenever they want and if there was an issue, Child C & D would tell her. • Child E (6 year old, male)'s Mother indicated that she has no concerns 1/2 with regards to discipline. She described Ms. Simpson and her to

During the 04/27/2023 on-site inspection the above allegation was reviewed with Ms. Simpson. I arrived at the home 11:30 AM and there were four child care children present who were sitting at the table and eating lunch. Child A was present and eating his lunch. All the children had cups with water. Ms. Simpson adamantly denied the allegation that she withholds food or liquids for any reason. During the day the children keep their cups on the counter and can have them anytime they want. There is fruit in a bowl on the kitchen table, and snacks along with the meals are readily available. I did observe there to be more than an adequate amount of food in the home. Ms. Simpson participates in the food program and offers three meals and three snacks daily. Child A is reported to attend Monday to Friday from 7:00 AM to 12:00 PM, so would typically be there for morning snack and lunch. On 05/03/2023, I spoke with Child A's Mother who stated when Child A would home, she would provide him with a full glass of juice, and he would "chug" the whole thing immediately. This caused her to have a concern that he was not drinking. On 06/08/2023, I spoke with the following: • Child B (2 year old, male)'s Mother reported to having no concerns to 1/2 believe that food or toilet use was being withheld from her son while in Ms. Simpson's care. Child C (5-year-old, female) & D (11-year-old, female)' Guardian spoke of the fruit basket on Ms. Simpson's counter and said that the children are able to eat or use the bathroom whenever they want. Child E (6 year old, male)'s Mother did not have any concerns about food 1/2 or toilet use being restricted. She discussed how the children have their cups on the counter and fruit basket on the table. Child F (6 year old, male)'s Mother does not have any concerns about 1/2 withholding food or bathroom use. She has observed fruit on the table and the counter has snacks and have cups with the children's names on them. Child G (3 yar old, male). Attempts were made to contact Child G's 1/2 Mother, but a return call was not received at time of report completion. ........................................ ..............................
