Based on the interviews that were conducted and the children’s attendance records reviewed, there is evidence to support that on 09/18/2024 and 09/19/2024, the ratio of 2 LANDMARK BUILDING • 105 W. ALLEGAN STREET • LANSING, MICHIGAN 48933 Michigan.gov/MiLEAP personnel to children present in the home at any one time was more than one CCSM to six children. On 09/18/2024, CCSM 1 worked alone from 5:30 a.m. to 12:29 p.m. caring for nine children. Ms. Coleman worked alone from 1:30 p.m. to 8:30 p.m. caring for ten children. At 2:35 p.m., Ms. Coleman began caring for six children. At 3:00 p.m., Ms. Coleman began caring for seven children. At 5:45 p.m., Ms. Coleman began caring for six children. On 09/19/2024, Ms. Coleman worked alone caring for nine children.

04/06/2023, I received a return email from Child A's Mother. She stated that Ms. Coleman was not monitoring Child A throughout the night. Ms. Coleman stated that she does not check on Child A once he goes to sleep at around 8:30pm. Eleven or twelve hours is a long time to not check on Child A. Child A's Mother does not wake Child A up throughout the night for diaper changes, so she was not expecting Ms. Coleman to do that. Ms. Coleman informed Child A's Mother that she put Child A down to sleep with a dry diaper. Child A's Mother could smell that Child A had "pooped" in his diaper on 04/01/2023 when she picked him up. Ms. Coleman attempted to place Child A in his car seat without changing his diaper. Ms. Coleman stated, "Oh, Child A just had a bowel movement, I could change him if you want." Child A's Mother declined and told Ms. Coleman that she would change Child A's diaper when she got home. When Child A's Mother got home, she called Ms. Coleman about her concerns. Ms. Coleman stated that she would check on Child A more often during the night.
