    On 11/12/2020, I completed an unannounced onsite inspection. I informed Ms. Metoyer of the allegations made against the facility. Ms. Metoyer denied that she's ever placed a covering over Child A. She shared that there are times when she raises her voice, but it is only in an effort to redirect and guide. She does not view it as yelling in a manner that causes any child to feel fearful of her. She stated that caring for Child A has been challenging due to her special needs and the center not having enough staff members to provide one on one care to manage some of the behavior concerns Child A exhibits. I inquired about other staff members yelling or any incidents of Child A being placed under a blanket. She denied any knowledge of this. I reviewed all staff members connected to the child care background check system to confirm current employment. The following individuals were identified: • Sherry Metoyer, designee. • Lori Holland, Child Care Staff Member (CCSM). • Nellie Hooper, kitchen staff who does not provide supervision or direct care. • Jacquelyn Caldwell, who is listed as the program director, however, she isn't (currently) working full time due to medical concerns during the pandemic. This concern was addressed under special investigation, 2021D 0450001. • Mikalyn Robinson, former CCSM, no longer employed at the facility who recently resigned. • Majhon Metoyer and Mr. Janera Knight are both listed as CCSM's, however, Ms. Metoyer shared they have not worked at the facility but remain connected as CCSM's for emergency cases only. In addition, Ms. Metoyer has Staff A who is employed; however, she is a minor and does not meet the requirements for employment under licensing rules. This has been addressed under special investigation, 2021D 0450001. I confirmed contact information for all actively working CCSMs which consisted of herself, Ms. Holland and Staff A. Due to Ms. Robinson still being connected to the facility and recently resigning within the last week I inquired about contact information for Ms. Robinson. Ms. Metoyer provided me with an employment application document with contact information for Ms. Robinson. 3 On 11/12/2020, I contacted Ms. Robinson. Ms. Robinson confirmed employment at the child care facility for approximately 1 week. I inquired about the allegations made. She shared that she only worked with Ms. Metoyer a couple times during her employment. She stated there were times when she heard Ms. Metoyer raise her voice at the children when they weren't following directions. This did not make her feel comfortable. She also felt one child was "fat shamed." She shared a specific incident where she reported hearing Ms. Holland make a comment about the child's weight and another incident in which she told a child to shut up. Ms. Robinson shared she has a strong passion for children and she just didn't want to work for the facility any longer because she didn't feel like they ran the facility like a professional business. I inquired about the care of Child A. She shared in her observation Ms. Holland and Ms. Metoyer tried to work with Child A, but they do not have the resources and the education to take care of her. She didn't feel like they were as patient with Child A as they could have been. She also denied witnessing anyone placing a cover over Child A's head. On 11/12/2020, I spoke with Ms. Holland regarding the allegations of this complaint. She denied ever seeing Child A being covered or covering herself with a blanket over her head. She shared there are times when both she and Ms. Metoyer will "yell" to get the attention of the children if they aren't listening. She denied that it's ever done in a way that she believes causes fear. She denied having any concerns regarding the care provided for Child A. On 11/23/2020, I contacted Child A's mother. She confirmed Child A was enrolled at the child care facility. She was enrolled in January of 2020, but due to the pandemic she didn't attend for several m
