    On 08/31/2022, I spoke with Child A's mother via telephone. Child A's mother stated that Child A attended the center for one week before she disenrolled her due to safety concerns. Child A is potty trained but had a toileting accident while at the center on 08/22/2022. Child A's mother was upset that they sent home Child A's underwear with fecal matter still on it. Later that day, Child A told her that the staff members had been fighting and had scratched her because she had a toileting accident. Child A's mother was not able to get any more information regarding the incident from Child A, and she did not observe any marks on Child A's body at that time. On 08/23/2022, Child A was upset during drop off and did not want her mother to leave her at the center. When Child A's mother picked her up from the center on 08/23/2022, Child A again mentioned that she was scratched by a staff member. When Child A's mother asked her where she got scratched, Child A pointed to her vaginal area. Child A's mother denied that she saw any marks or other injuries on 2 Child A at that time. Child A's mother disenrolled Child A from the center on 08/24/2022. Child A's mother took Child A to the emergency room on 08/25/2022 due to her concerns. Child A's mother stated that the doctors did an examination and did not find any tears or marks in the vaginal area. On 08/31/2022 I made an unannounced on-site inspection. I interviewed Tina Caverly (owner/program director), Ashely Williams (lead preschool caregiver), and Tasha Myryck (assistant preschool staff member). Ms. Caverly stated that Child A had only been in care for one week before her mother stopped bringing her to the center. Ms. Caverly said that Child A's mother mentioned that Child A had been in care in the past, but that she had removed her from a previous center. Ms. Caverly said that Child A's mother seemed preoccupied with making sure the staff members had criminal background checks during her initial tour of the center and repeatedly asked this question. On 08/22/2022, Child A had a toileting accident while in care. Ms. Myryck had Child A go to the bathroom and Child A changed and cleaned herself up. Ms. Caverly said that since Child A is in preschool, staff members have the children attempt to clean themselves as a lesson in selfcare. The staff members will hand the child items and verbally direct them, but they typically do not need to physically help with the changing and cleanup. Ms. Caverly stated that she has video cameras throughout the center and had the footage from 08/22/2022 at around 2:00pm. Ms. Caverly showed me the recording from a camera angle that views the bathroom door area. Ms. Myryck is seen standing outside the doorway directing Child A and handing her wipes from the doorway. Child A is standing in the bathroom changing and does not appear to be in any distress. Ms. Myryck was wearing gloves but did not touch Child A at any time during the video recording. Ms. Williams can also be seen in the video recording walking past the bathroom doorway. The recording was approximately 5 minutes long and appeared to capture the entire changing incident. Ms. Calverly stated that Ms. Williams and Ms. Myryck were the only staff members in the preschool classroom on 08/22/2022. Ms. Caverly stated that Child A was having a difficult time adjusting to the center and was not regularly following directions. Child A's mother did mention to Ms. Caverly on 08/23/2022 that Child A told her she got scratched and that the staff were fighting. Ms. Caverly said that Child A's mother mentioned this in passing and did not seem upset or concerned and said that this "scratch" was probably just an accident. Ms. Caverly denied that she or any of her staff saw a scratch, or any mark on Child A. Ms. Caverly denied that staff members got into a verbal or physical fight on 08/22/2022, or any other day. Ms. Caverly said that Child A's mother never brought Child A back to the center or contacted her after
