    R 400.8146 · 1 R 400.8146 Information provided to parents. (3) For infants and toddlers, a center shall provide parents with a written daily record that includes at least the following information: (a) Food intake time, type of food, and amount eaten. (b) Sleeping patterns indicating when and how long the child slept. (c) Elimination patterns, including bowel movements, consistency, and frequency. (d) Developmental milestones. K (e) Changes in the child's usual behaviors.

    On 10/11/2021, I completed an unannounced onsite inspection. I informed Ms. Walker of the allegations made against the child care facility. I asked about daily documentation provided to parents of infants and toddlers. Ms. Walker shared they only provide parents with a verbal update for infants and toddlers. She discontinued providing written documentation during the summer due to the increase in those affected with Covid-19. She shared there was a recent parent who requested documentation at which time she informed the parent of the reasoning for giving verbal updates. She shared the parent was okay with this and didn't appear to have a problem with this. I asked about what documentation is taken at the facility to note feeding times, bowel moments and any changes or milestones observed during the day for infants and toddlers. Ms. Walker shared the only written documentation they maintain is the children's arrival times, their temperature upon arrival, the date and diaper changes. They don't document anything regarding meals or snacks. Ms. Walker showed documentation of what she previously used and shared she didn't think this was a rule violation since verbal updates were provided to parents during the day. Ms. Walker shared she will begin using the written documentation log effective today. On 11/01/2021, I spoke with Child A's mother. I asked about daily written documentation provided to her about Child A. She shared "I don't like paper work so I just asked for updates." She denied receiving daily written verification (via an app link or paper documentation) but she did recall receiving written documentation in the past. She could not recall when this was but said it wasn't within the past month or two.

    R 400.8155 · R 400.8155 Child accidents and incidents; child and staff illness. (1)A center shall have a written plan for how and when a parent is notified when personnel observe any of the following: (a) Changes in a child's health.

    On 10/25/2021, I spoke with Ms. Walker to discuss the allegations made against the facility regarding parents not being notified during the day when their child is not feeling well. Ms. Walker denied this to be true and shared she oftentimes feels like she overly communicates. She provided an example of a recent time when a child had a temperature, and the parent was notified immediately. She expressed frustration that this was part of the complaint because she believes she communicates all concerns with her parents, and her staff does the same. On 11/01/2021, I spoke with Child A's parent. She confirmed she has a toddler enrolled at the child care facility. I asked about notification when (if) her child becomes ill while in care at the facility. She denied that there has been a time when her child has become ill while in care, however, she feels confident that Ms. Walker would notify her. On 11/01/2021, I spoke with Child B's parent. She shared she's been notified in the past when her child became ill while in care. She shared there was a time when her child had a temperature, and she received a call and when to pick her child up as a result. She shared "I've never had an issue with lack of communication." I received the plan for how and when parents will be notified of a change in a child's health. A portion of the plan states "should a child have accident, change in health, injury, illness, incident or is too sick to remain in class, child will be isolated with director or lead teacher and kept from other children until picked up by parent and any item touched or utilized by child will be cleaned thoroughly." 4

(4) If a center becomes aware that a staff member, volunteer, or child in care has contracted a communicable disease, then the center shall notify parents and provide all of the following information: (a) The name of the communicable disease the children were exposed to. 5 (b) The symptoms of the disease. (c) Prevention measures as recommended by the U.S. Centers for Disease Control and Prevention (CDC) at the following website: https://www.cdc.gov/DiseasesConditions.

    On 10/11/2021, I interviewed Ms. Walker. She shared when there's a communicable disease at the facility, she will notify parents via phone. I asked what information is shared during those contacts. She reported telling the parents the name of the communicable disease that was reported to her. I reviewed the requirements of the rule and asked if she provides the parents with the symptoms of the disease and the website to the CDC which provides additional information regarding the prevention measures. She denied providing this information during her phone contacts. I asked if documentation is posted at the facility with this information or placed in each child's box to take home. She indicated written information pertaining to the specific communicable disease is not provided. I received the center's written plan for communicable disease and noted a portion of the plan states, "If the center becomes aware of any in care exposure to a communicable disease all parents will be notified of the disease, provided with written communications on the symptoms as well as disease prevention measures as recommended by CDC."
