    On 06/08/2021 1 left a message for the complainant. On 06115/2021 Ms. Leak stated that Child A's Mother had called the center on 06/10/2021 and informed her that she had kept Child A home because on /0W.M1 had corn lamed that area was in pain. Child A had stated that Child B had touched at the center, but had recanted story after being seen by the doctor. C d A's Mother had con acted the an ad taken Child A to the doctor. with anyone. Ms. Davenport stated that she spoke with Child A's Mother on 06/10/2021 when she dropped off Child A at the center. Child A's Mother stated that Child A woke up on 06/0512021 and said that ■ was in pain and Child B had touched ■. She then informed her that she had contacted the and had Child A seen at the The Ms. Davenport informed Child A's Mother that when something happens at the center, she should inform her so that they can work together. She requested a written statement from Child A's Mother before she allowed Child A to stay at the center, and Child A's Mother provided a written statement. After speaking with Child A's Mother, Ms. Davenport because ofc tohen atlalecgatteioPnso.nP.PW Ms. Davenport received a letter from dated 06/10/2021 that stated an investigation was not required. Ms. Davenport did not realize that she was required to verbally inform the department within 24 hours and submit a written report within 72 hours of being informed about the allegations. Ms. Davenport provided the child information cards for Child A and Child B, and after review she acknowledged that they were not fully completed by the parents, and she had not recognized that the cards were incomplete. I reviewed Child A and Child B's child information cards at the center. Both cards were missing emergency contact and release of child, release of child only, and physician's phone number. Child A's information card did not have parent employer information, physician or health clinic name, allergies, special needs, and special instructions. 3 Ms. Davenport and Ms. Leak stated that they were not contacted by the . Child A and Child B have not had any issues at the center, they do not play together, and they have not been alone together. On 06/17/2021 Child A's Mother stated that on 06/05/2021 Child A stated, ' was hurtin ." Child A's Mother asked Child A if anyone touched and said, es, my friend [Child B touched me j." , She did ask any additional questions because she did not want Child A to think anything was wrong. Child A's Mother called the and took Child A to the She spoke with Child A after the and asked again if anyone had touched Child A said `No, mommy [ td not touch me, I was lying."
