On 04/12/2022, I interviewed the complainant regarding the allegations. She stated that the child care staff members (CSSM) in the Infant 1 classroom does not tend to the infants when they are crying. They leave the infants crying for 30 minutes. She observed a CCSM just sitting there letting the children cry. According to the complainant all of the employees at Childtime Learning Center 2 yell at the children and the co-director, Bethany Reed, knows about it but nothing is done about it. The complainant was unable to provide the child care staff member's name. On 04/13/2022, I completed an onsite inspection at Childtime Learning Center facility. I interviewed Ms. Reed and observed the facility. According to Ms. Reed, there are eight infants enrolled in the infant 1 room and 16 children enrolled in the early preschool room. She has never heard an infant cry for 30 minutes and she has never heard any staff yell at the children. A CCSM can be dismissed for yelling at the children. I observed the infant 1 room, there were eight children, ages 6 weeks to 12 months, with four CCSMs. In the early preschool room, there were 14 children, ages 2 to 3 years, with two CCSMs. Upon entry of the infant 1 room, there was an infant crying while laying on the floor. The infant cried for about two minutes, then a CCSM went over to console the infant. The CCSM sat on the floor and played with the infant and the infant stop crying. On 06/14/2022, I conducted telephone interviews with Jessica Ellis, program director, and CCSMs Colleen Williams, Diane Clemons and Tyler Singelyn regarding the allegations. Ms. Ellis said Ms. Williams is the lead caregiver in the infant 1 room. She has been with the center for the past 10 years. Ms. Williams does not let the children cry and she does not leave them unattended. If she is at a 4:1 ratio and she is changing a diaper. Another infant may cry for a few minutes until she is finished but she goes over and comforts the crying infant as soon as she can. She never leaves infants crying for 30 minutes when she is in the room. Ms. Ellis stated that she has not heard any of the CCSMs yell at the children. A CCSM would be dismissed from employment if they yell or speak to the children inappropriately. Ms. Williams said they always try to address a crying infant as soon as possible. The infants do cry but they are never left 30 minutes crying. There are times she is feeding an infant and another child starts to cry. She stated that she will put the crying infant into a bouncer seat near her and continue to feed the other child. All the CCSMs have been trained and they do not leave infants crying for 30 minutes. Mr. Singelyn stated that he is only in the infant room during CCSM's break. He has seen the infants cry because they are exhausted. The children are put in their crib and are left crying for 10 to 15 minutes until they fall asleep. Ms. Williams, Ms. Clemons and Mr. Singelyn stated that they have not heard any of the CCSMs yell or talk to the children inappropriately. Child A's (2 year-old, male) Mother and Child B's (7-month-old, female) Mother 1/2- stated that she has no major concern regarding the child care center. She stated 3 that the CCSMs are very caring with the children. Child A is in the early preschool room and Child B is in the infant 1 room.
The child care staff at Childtime Learning Center does not provide appropriate care and supervision to the children. They leave infants crying on the floor for 30 minutes and they yell at the children The child care staff in the Infant 1 room change the children's diapers one right after another and they do not wipe the changing table down after each diaper change. 4 The infants in the Infant 1 room sleep in bouncers. In the early preschool room, there is one caregiver with 20 kids each morning. 1 The child care staff use bleach to clean the classrooms to clean.

On 04/12/2022, I interviewed the complainant regarding the allegations. She stated the changing tables in the Infant 1 room is not cleaned properly with the three-step process. The CCSMs change the children one after another and they do not clean the changing table between diapers. On 04/13/2022, I interviewed Ms. Reed regarding the allegations. She stated that the changing tables are cleaned after each child's diaper is changed. There are three bottles in each room, therefore the changing table, classroom tables, etc. can be washed, rinsed, and sanitized when needed. During the onsite inspection, I observed an CCSM changed an infant's diaper. The CCSM wore gloves to change the child's diaper. She used the three- step cleaning process, which includes washing, rinsing and sanitizing the changing table, when she finished. Another CCSM checked an infant's diaper, while wearing gloves. The infant was dry and did not need to be changed. The CCSM cleaned the changing table using the three-step process after she finished checking the infant's diaper. On 06/14/2022, Ms. Ellis said the CCSMs use the three-step process to clean the diaper changing table after each child's diaper is changed. At the end of each day the changing table is clean with a hospital grade cleaning product by Betco. Ms. Williams said the changing table is cleaned using the three-step process after every diaper change. She has not seen any CCSM not use the three-step cleaning process. Mr. Singelyn stated that he has seen the CCSMs in the infant room clean the diaper changing table after each child's diaper is changed. At times they will use the three bottles (i.e., wash, rinse and sanitize). He stated that if a lot of infant's diapers need to be change the CCSM will only use the bleach water after each change.

On 04/12/2022, I interviewed the complainant regarding the allegations. She stated that infants are sleeping in the bouncer seats in the classroom. The CCSMs do not move them to a crib once they fall asleep. On 04/13/2022, I interviewed Ms. Reed regarding the allegations. She stated the infants are not allowed to sleep in bouncers. There are cribs in the classroom and the children are placed on their backs to sleep. There are times, when the infants fall asleep in the bouncers, but they are moved to their crib and are not left in the bouncers to sleep. During the onsite inspection, eight infants was observed with four CCSMs. One of the eight infants was sleeping on their back in a crib. I observed a CCSM rocking an infant to sleep in a rocking chair. The CCSM then placed the infant in the crib to sleep. No infants were observed sleeping in the bouncers. On 06/14/2022, Ms. Ellis said none of the infants are allowed to sleep in the bouncers in the infant rooms. If an infant falls asleep, the CCSMs remove them immediately. Picking them up and placing them in their cribs on their back. They are checked while they are sleeping every 15 minutes. Ms. Ellis also stated that non-mobile babies have to move around the room every 10 to 15 minutes. Ms. Williams said as soon as the infants fall asleep, they are moved to a crib and placed on their back to sleep. The infants are not left in the bouncer seats and left sleeping. She stated that all the CCSMs know the rules and she has not seen anyone leave a child sleeping in the bouncer seats. Mr. Singelyn stated he has seen infants fall asleep in the bouncers and they are left for about 10 to 15 minutes and then moved to the crib.

On 04/12/2022, I interviewed the complainant regarding the allegations. She stated that CCSM, Ms. Diane has up to 20 children by herself each morning between 8:00 a.m. to 10:00 a.m. On 04/13/2022, I interviewed Ms. Reed regarding the allegations. She stated there was an issue with staffing in January 2022 and licensing came out to investigate. They have more people scheduled in the mornings and they are not having an issue with ratio in the early preschool room currently. During the onsite inspection, there were 14 children with two CCSMs in the early preschool room. Circle time was observed and a CCSM was reading a book to the children. The other CCSM was assisting a child that was not participating in circle time. The attendance records for the following dates: 04/06/2022, 04/07/2022, 04/08/2022, 04/11/2022, 04/12/2022 and 04/13/2022 were provided for review. Upon review it was discovered that on 04/06/2022, 04/08/2022 and 04/13/2022 the early preschool room was out of ratio. On 04/06/2022 there were 20 children with two CCSM between 8:17 a.m. to 8:35 a.m. There should have been three CCSMs present. On 04/08/2022, there were nine children with one CCSM between 7:14 a.m. to 7:18 a.m. There should have been two CCSMs present. On 04/13/2022, there were 20 children with two CCSMs between 8:40 a.m. to 8:45 a.m. There should have been three CCSMs present. On 06/14/2022, Ms. Ellis stated that she looked at the attendance sheets and she did not see any times where the early preschool room was out of ratio. They had three classrooms (i.e., infant, toddler, and early preschool) when the center opens in the morning. They have since opened another classroom to accommodate the families and to ensure they are in ratio. Ms. Clemons stated that she is the early preschool lead caregiver, and she is an opener. The ratio for the early preschool room is 8:1. She stated that she has been in the room since February 2022, and she has never had 20 children at one time by herself. Mr. Singelyn stated that the early preschool room has been out of ratio once or twice. The last time he could recall was in April 2022. Management at the center has hired more CCSMs to address the issue. According to Child A's year-old, male) Mother, there are six to 12 children in the (1/2- early preschool room each morning with one CCSM during drop off. The children that are present, are 2 years old to school age. Child A's Mother believe the 1/2 mixed age of children are not in the class for long, approximately an hour each morning. 7 Child C's (2 year old, male) Mother stated that Child C attends a couple days a 1/2 week. She stated that the ratio is fine in the early preschool classroom. There is usually 8:1 ratio.

On 04/12/2022, I interviewed the complainant regarding the allegations. She stated that a CCSM was cleaning the Early Preschool room with bleach. The children in the classroom were crying because their eyes were burning from the smell of bleach. On 04/13/2022, I interviewed Ms. Reed regarding the allegations. She stated she was unaware of a staff cleaning with bleach. The cleaning bottles are made each morning by the opening staff. During the onsite inspection, a strong odor of bleach was not smelled in the child care center. On 06/14/2022, Ms. Ellis said there was an incident when she went into the preschool room and smelled bleach. There was a new employee that was not measuring the bleach-to-water ratio correctly for sanitized bottles. The issue was address and the CCSM was retrained. The sanitizing bottles are no longer made by the new employees. Each room cleaning bottles are taken to the kitchen at the end of the night and in the morning staff makes the bottles to ensure that the correct measurements are placed in each bottle. Ms. Clemons and Mr. Singelyn stated that there have not been any recent concerns with cleaning the preschool room. The cleaning bottles are made by the morning staff each day. They could not recall a child's eye being irritated from bleach being used. ..... ............................... ................................

On 1/26/2022, I interviewed program director, Jessica Ellis by phone. She after speaking to Child A's Mother concerning an allegation regarding Child A. On 1/27/2022 and 3/3/2022, I emailed was asked not to interview the staff before admitted to spanking As of stated in his disposition CCSM1 was determined to be not co3n/d2u9c/iv2e0 2to2 t,h"eewweelfare of children. The finding was based on the examination by of an adult handprint on Child A's buttock. Also, CCSM1 admitting to that she spanked Child A. On 1/27/2022, I interviewed Child A's Mother. On 1/20/2022, she dropped Child A off and Child A's Father picked him up. Child A was playing with her sister and said her "bum" hurt. When her sister asked why her bum was hurting Child A told her because she was being bad and not listening. Child A's sister asked who hit her and she named CCSM1. Child A's Mother said heard Child A and asked her what happened. Child A stated she was being bad and not listening. Child A's Mother took Child A in the bathroom and noticed a handprint on her buttock and open skin above her panty line. Child A's Mother called the center and left a message regarding the injury to Child A's bottom. Child A's Mother said on 1/21/2022, Child A's Father took Child A to the hospital for an examination, observed two long linear marks on Child A's bottom that appeared to be adult ingers and broken skin above her panty line. On 1/21/2022, Ms. Ellis returned Child A's Mother phone call. She asked Child A's Mother to take a picture of the bruised area and Child A's Mother declined. She did not feel comfortable sending the photo. Ms. Ellis informed her that she would investigate the allegation. Child A's Mother said on 1/24/2022, after reviewing the video footage, Ms. Ellis called her. She was told that CCSM1 was placed on suspension while the allegation was being investigated. Ms. Ellis informed Child A's Mother that she observed CCSM1 give Child A light tap on her bottom while she was climbing on the furniture. There was no other footage of Child A being hit due to blind spots in the classroom and no camera footage in the restroom. On 2/8/2022, I interviewed Child A's Mother a second time. She said on informed her that he spoke with CCSM1, and Me Pitted 3 to spanking Child A in the restroom. Child A was using the toilet and took off all her clothes and tried running out of the restroom. On 1/27/2022, I interviewed . She said Child A appeared to be a happy kid. This was her first-time seeing Child A. There was a faded mark that looked like two fingers on Child A's butt. Child A's Father stated the marks were darker when they first noticed them. Child A's injury did not require medical treatment. On 1/27/2022, I conducted an unannounced inspection and interviewed Ms. Ellis. After speaking with Child A's Mother by phone regarding the allegation, Ms. Ellis requested to get back in touch with her later after investigating. CCSM1 was not on schedule to work that day, but she needed to drop off a child she was babysitting. Ms. Ellis pulled CCSM1 into her office and asked if anything happened in the classroom the day before with Child A. She denied spanking Child A. She stated that a boy in the classroom was hitting the afternoon in question. CCSM1 left, and Ms. Ellis called Child A's Mother back. She asked Child A's Mother if Child A told her who hit her, and she named CCSM1. Child A's Mother explained to Ms. Ellis she overheard Child A talking to her sister. She went to check for bruises. Once in the bathroom she noticed the handprint on her buttock. Ms. Ellis said Child A told her mother she was being bad and not listening. Ms. Ellis called and text CCSM1 with no response. When asked to give her a call, she responded by text asking if she was fired. CCSM1 never returned Ms. Ellis phone calls. Ms. Ellis said CCSM1's There are no cameras in the restroom. On 1/11/2022, I interviewed complainant. The complainant said on 1/5/2022, the infant, toddler and preschool rooms were out of ratio. It is continuing to happen on a consistent basis. The child care staff member (CCSM) to child ratio in infants was 3:14. The CCSM to child ratio in toddlers was 2:9. The CCSM to child ratio in preschool was 2:21. In the past two weeks, six CCSM's tested positive for Covid. Five to six children tested positive in the toddler room causing the room to be closed. On 1/7/2022, CCSMs working in the infant room tested positive for Covid. The staff and children are rotating in and out of the center passing Covid to each other. The upper management has not closed the center for deep clean. On 1/11/2022, I conducted an unannounced inspection.
