I conducted an unannounced onsite visit to the center on 3/06/2023. I interviewed the Assistant Program Director, Ms. Debra Husted. She was aware of the allegations. She stated that the concerns were brought to her attention by Child Care Staff Member (CCSM) 1. CCSM1 had informed her that she had observed CCSM2 being rough with the children. Ms. Husted didn't have any proof, so they conducted an internal investigation, as the information was alarming. During the investigation they were unable to prove that anything occurred. She stated that they have a system called "Watch Me Grow" that allows parents to see the children live and watch what is going on in the classrooms. Nothing had been brought to their attention that was inappropriate. CCSM1 was not present during the onsite investigation, but she called me on 3/06/2023. CCSM1 stated she has observed CCSM2 use harsh language towards the children. CCSM2 gets easily frustrated with the children especially when she is dealing with personal issues. CCSM 1 said CCSM2 does not appear to have patience with the children, and she swears at them. While onsite I interviewed CCSM 2. She denied the allegations and stated that she always grabs the children by their arm pits and not by their arm. She denied being rough with the children and denied telling them to shut up or be quiet. She has had to act quickly sometimes when children touch other children with their hands, but she is not being rough with them. She alluded that other staff members maybe telling the children to shut up but never her. She said "other people may do it but never me." While onsite, I interviewed CCSM3. She stated that she has worked with CCSM2 in the morning time when the 2 classrooms are combined. She has concerns about how CCSM2 talks to the children but has never seen her be rough with them. She stated she has heard CCSM2 say things like, "I'm Pissed" around the kids, and "Jesus" and then a child's name as if she is upset with them. She has heard other CCSMs say that they have concerns that CCSM2 is rough with the children, but she has never witnessed it. CCSM3 ha never reported her concerns to management. While onsite, I interviewed CCSM4. She stated that she has observed CCSM2 swearing and scream at the children. She has over heard CCSM2 say that she wanted to "strangle the children". CCSM4 stated that she has seen CCSM2 grab the children and pull them by the arm, and she has pushed a child in the chest causing the child to fall. A few months ago, CCSM4 saw CCSM2 pull a child to the ground by the arm and the child banged their head on the wall. CCSM2 stated that none of the children were injured. She did report her concerns to management. I called Child A's Mother on 3/30/2023. She stated that she has no concerns about the quality of care that Child A (2-year-old, female) receives at the center. Her concerns were only involving the recent transition due to the closure of the other school location. 3 I called Child B's Mother on 3/30/2023. She doesn't have any issues with the care that Child B (1 year-old female) receives. She stated that she has 2 children attending the facility. She does log into the live app provided that allows her to see how Child B is doing and to just check on the classroom and activity. I called Child C's (3-year-old male) Mother on 3/30/2023 and on 4/1/20223. She did not return my calls. On 4/03/2023 I held an exit conference with the Licensee Designee, Jennifer Gunderson. Ms. Gunderson said she was not aware of the allegations prior to this investigation.

On 5/20/2022, I received an incident report from Childtime indicating Child A was left unattended in the classroom when her class went outside. I spoke with the assistant director, Brittany Dillion who stated the CCSMs miscounted the children and Child A was separated from her class for less than two minutes. On 5/24/2022, I arrived at the center to complete an on-site investigation. I interviewed CCSM Carlee Corwin. She stated about 4:08 p.m., she and Amanda Wandrie were taking the children outside. There were 10, 3-year-old children and two childcare staff members. They lined and counted the children before they left the classroom. As soon as they got outside Child B had to go to the bathroom and she was walking Child B back in when she saw CCSM, Debbie Husted walking Child A outside. Ms. Corwin guessed that no more than 2 minutes has gone by from the time they got outside, to the time Ms. Husted brought Child A out. Ms. Corwin stated she did a name-to-face count but confused Child A with another child and improperly fill out the transition form. Ms. Corwin said Child A said she left the line to go fill up her water bottle and they left her. I interviewed Debbie Husted. She stated she was on her way to the office getting ready to leave for the day. She heard Child A crying and saw her walking around in her classroom, in circles with her water bottle. She asked Child A, "Where are your friends" and she replied outside. Ms. Husted took Child A outside to join her group. Ms. Husted stated she found Child A between 4:00 p.m. — 4:10 p.m. I interviewed the program director, Nicole Partaka. She was not present when Child A was left alone. She had already retrained the CCSMs on proper name-to-face count. I suggest that it might be helpful for the CCSMs to be at the front and end of the line. The person at the front of the line could walk backward so that there are two sets of eyes on the children. On 6/2/2022, I interviewed CCSM Amanda Wandrie by telephone. She stated she feels horrible about the incident. As lead caregiver, she should have completed the name-to-face count instead of Ms. Corwin. She stated Child A was left alone for about 2 minutes. When Ms. Husted brought her outside, she was upset and crying. Child A told her she left the line in the bathroom to fill up her water bottle. She stated the director had them all go through retraining on the proper procedure for name-to face-counting. Ms. Wandrie informed Child A's parents about what happened. They were not upset and apologized to her for Child A not following directions. On 6/13/2022, I interviewed Child A's Father by telephone. He stated Child A is fine. It was an accident, and he has no concerns about her care at the center. Ms. Wandrie assured them it would not happen again. 4

On 08/11/2021 and 11/15/2021, I spoke with Child A's Mother. She received a call from child care staff Member, Michelle Harp, who informed her that Child A had an injury on his eyebrow, that was bleeding and he needed to be picked up. Child A's Mother was told that Child A did not cry and was sitting quietly. Michelle Harp walked over to Child A and his eyebrow was bleeding while they were outside on the playground. She cleaned the injury and it kept bleeding. Two band aids were applied to the injury. Child A was taken to urgent care and received liquid sutures on his eyebrow. Child A Mother. She stated that she was initially concerned that the child care staff could not tell her how Child A was injured. She explained that she did not think that a child care staff member injured Child A, however, she was not sure if another child hit Child A. Child A Mother was concerned that it took 2 weeks after the incident, 1/2 before she was able to review the video footage. Child A Mother told me that this was the second head injury that Child A had since February 2021. She told me that Child A returned to the child care center for one week and they decided to disenroll him. Child A Mother stated that when they reviewed the video footage, she saw Child A and Child B running around and playing, they collided and fell. She stated that both children got up and continued to play. Child A Mother explained that she did not see a child care staff member in the video check on the children after they collided. Child A went to the slide and slid down, he did not appear to fall or injure himself on the slide. Child A Mother told me that Child A was standing by himself after he got off the slide, the child care staff member walked over to Child A and checked on him. She stated that she received the call from the child care center regarding Child A being injured. On 08/30/2021, I completed on unannounced inspection at the facility. I interviewed Nicole Partaka, program director, she said Michelle Harp, child care staff member informed her Child A was hurt on the playground. Ms. Harp removed the band aid to reveal the cut on Child A's forehead. Michelle Harp immediately called Child's A Mother to inform her that Child A was injured. Child A's Mother came to the center to pick Child A up. Nicole Partaka said Child A's Mother told her the following Monday that Child A received stitches. Child A's Mother spoke with Michelle Harp, and Brittany Dillon, 3 assistant program director because they were supervising the children outside on the playground on the day of the incident. Child A's parents were provided with a copy of the incident report. Nicole Partaka stated that Child A's parents requested to watch the live video of the incident. Tanya Zoro, District Manager, Child A's parents and Nicole Partaka watched the footage together. According to Nicole Partaka, the video footage showed Child A running around on the playground. Child A and Child B (3-year-old, male) collided, Child A got up continued to run and play. Child B also continued to run and play. After numerous minutes Child A walked over to the picnic table area. Michelle Harp and Brittany Dillon checked on Child A and noticed he was bleeding on his eyebrow. Michelle Harp and Brittany Dillon immediately started first aid. Child B did not have any injuries. I interviewed Michelle Harp and Brittany Dillon; they both said they took the early preschoolers to the playground for their morning time outside. The children immediately began running around, playing on the playscape. They were both constantly walking around the whole playground supervising the children. Michelle Harp and Brittany Dillon said when they got outside Brittany Dillon took a minute to walk around and get a photo of each child to send to their parents. On 04/01/2021, I completed an unannounced onsite inspection at Childtime Learning Center. I spoke to Brittany Dillon, assistant director regarding the allegation. I observed the child care center during nap time. In the Infant 1 room, there were four infants with one child care staff member. In the Infant 2 room, there were three infants with one child care staff member. In the Toddler room, there were nine children with two child care staff members. In the Twos room, there were seven children with one child care staff members. In the Early Preschool room, there were five children with one child care staff member. In the Pre-K room, there were 12 children with one child care staff member. A copy of Child A's Child Information card was obtained. According to Ms. Dillion, Child A was enrolled at the child care center from September 2020 through 03/22/2021. At initially enrollment, she was in the Twos room, then transition into the Early Preschool room in January 2021. Kallie Iler, child is the lead care staff member in the Early Preschool classroom. Ms. Iller works on Monday, Wednesday and Friday. There is other child care staff members in the Early Preschool classroom on Tuesday and Thursday. Child A attended the child care center part-time on Monday, Thursday and Friday, from 8:00 a.m. to 3:00 p.m. The issue of Child A not eating and drinking at the child care center had been a problem for the past month. The child care center documented what Child A ate on the Brightwheel application. Through the Brightwheel application, messages can be sent to and from parents. The parents have access to their child's food intake, sleeping pattern, elimination patterns, changes in behaviors, pictures of activities and children. In regard to the allegations, the center brought the concerning issue of Child A not eating to Child A's Father and he seemed uninterested in the beginning. The concern had been discussed with Child A's Father by Ms. Dillion and Ms. Iler, when Child A was being picked up from the child care center. Initially Child A would eat the fruit and grains that were offered but then she stopped eating everything that was offered to her at the center. They tried several things to get Child A to eat while she was at the center. The food was pre-plated and cut up in smaller pieces. Child care staff members asked her if she needed assistance and/ or they allowed her to eat on her own. Child A's Father asked and brought in drinks for Child A to drink while at the center. He brought in apple juice and chocolate milk. Child A drank 1/2 of the apple juice. Child A's Father then brought Child A a lunch for her to eat while she was at the center. The lunch was mostly snack food and did not meet the food requirements. Child A's Father was told that documentation would be needed from the doctor for him to bring Child A a lunch. Child A's Father called Child's A doctor but was told it would take a couple of days to get the documentation. He asked them what he could do and they told him to pull Child A from the center. On 03/19/2021, Child A's Father called and spoke to Elizabeth Bazner, the center food specialist, regarding Child A sleeping in the direct sunlight. He referred to the Brightwheel application stating that Child A did not eat of drink anything that day. Child A's cot was moved but later the sun shifted and it was on the child again. Ms. Partaka was at lunch when the phone call was received by Child A's Father. Ms. 3 Partaka was informed of the phone call when she returned. She called Child A's Father back and he stated that he was upset that Child A had not eaten or drank anything since being at the center. Child A's Father stated that he wanted to withdraw Child A from the center. On 03/22/2021, Child A's Father sent an email to Ms. Partaka stating that he wanted to pull Child A from the child care.
