I made an unscheduled onsite inspection at Teri Hammond's child care home on 6/3/2022. I spoke with Teri Hammond and child care staff member Afton Rachut. Ms. Hammond and Ms. Rachut were familiar with Child A's peanut allergy. They informed me that on 4/19/2022, Child A was provided a snack around 2:30 p.m. His father picked him up around 3:00 p.m. They later found out that Child A vomited and then started wheezing while at Meijer with his father shortly afterward. It wasn't until after Child A's Mother contacted Ms. Hammond from the hospital at 5:00 p.m. that they realized their mistake. Ms. Rachut accidentally provided Child A with a packet of Nutter Butters instead of Ritz cheese crackers. The snacks were in the same box and have similar packaging. According to Ms. Hammond, Child A's parents were very understanding at the time and Child A returned to care two days later. It wasn't until the child care home had to close recently due to COVID that Child A's Parents became upset with her. Child A's Parents were concerned about missing work during the temporary closure. Ms. Rachut showed me a text conversation she had with Child A's Parents asking her to care for Child A in their home while the child care was closed. Child A no longer attends Ms. Hammond's child care home. I spoke on the telephone with Child A's Mother on 6/8/2022. Child A's Mother provided Ms. Hammond an action plan from the allergist and an Epi-Pen at the time of enrollment. A few months ago, Child A was picked up from Ms. Hammond's child care home and taken to the grocery store. While at the store, Child A vomited and then started wheezing. Child A was given an Epi-Pen and then transported to the emergency room to receive a steroid treatment. Child A's Mother contacted Ms. Hammond from the hospital and found out that Child A was accidentally provided a Nutter Butter for a snack shortly before he was picked up. According to Child A's Mother, Child A only returned to Ms. Hammond's child care home because she had nowhere else to take him. She called several licensed facilities, however, no one had openings. I spoke on the telephone with Child B and Child C's Mother on 6/8/2022 and Child D's Father on 6/13/2022. Neither parent expressed any concerns about the care their children receive at Ms. Hammond's child care home.
