On 10/06/2021, 1 completed an on-site inspection at the Pinconning Early Childhood Program. I observed the classroom and children sitting at the tables. I requested to interview the staff members who were present during the time of the reported incident. I interviewed Ms. Shawna Paulik, Program Director. Ms. Shawna reported that 10 children were outside with Ms. Sarah. Ms. Paulik reported that she still had four children inside with her getting ready to go outside. Ms. Paulik reported that it was starting to rain, and Ms. Sarah started to come inside prior to the other children going outside. Ms. Paulik reported that during this time she received a phone call, set up the projector, and the children came in and started dancing to YouTube videos. As Ms. Sarah was coming into the classroom Child A must have exited the classroom onto the playground. Ms. Paulik recalls only hearing the door buzzer that alerts staff when the door to the playground opens, one time. The buzzer is loud. The schedule was off due to the rain 2 and normally they would have the children sit and complete another child count. They count the children when they leave the classroom and when they come inside the classroom. They have the children line up against the wall to come inside. They are now counting the children one by one as they enter the classroom. Ms. Paulik reported that she could hear a child outside on the playground but thought it was another classroom that went outside due to the rain stopping. Her blinds in the classroom were closed and she could not see outside. Another individual notified her that Child A was outside and brought the child back to the classroom. She did not know the exact amount of time that Child A was on the playground, but they had about three YouTube videos with the children. I completed an interview with Ms. Sarah Davies. Ms. Davies reported that she had 10 children outside with her. She counted the children when they lined up to go inside the classroom as it started to rain outside. Child A has been trying to hide and stay outside on the playground. She had the children come inside and take off their jackets. Child A must have gone past them as they were coming in and taking off their jackets. She did not hear the alarm to the door go off again after the children entered the classroom. Ms. Davies reported that Ms. Paulik called Child A's mother after the incident. They usually count whenever the children move, and they count the children as they line up on the wall outside. She counted the children outside as usual. The children came into the classroom and were dancing and moving around. They would normally be doing large group. Ms. Addie was the staff member who brought Child A to the classroom from outside. Child A was wet from going down the slide as it was his backside that was wet. I completed an interview with Ms. Addie O'Laughlin. Ms. Addie reported that she was notified of a child being left on the playground by Ms. Kris. She checked to make sure that it was not one of her children and went to meet up with Child A. She took Child A who was not crying at the time to find Ms. Paulik. They have alarms on the doors that are loud, and you can hear when a child exits the room. I interviewed Ms. Amy Briggs. She had gone into the room to inform Ms. Paulik that she had a message. Ms. Briggs reported that Ms. Addie had come in with Child A after she left the classroom. It was the lunch aide Ms. Kris who informed them that Child A was outside on the playground. Ms. Briggs was not sure how long Child A was left on the playground unsupervised. I interviewed Ms. Kris Brissette. She was informed that Child A was outside on the playground. She went to Ms. Addie and they brought Child A to the classroom. On 10/04/2021, I made a phone call to John Sanford, licensee designee. Mr. Sanford reported that he was notified about Child A being left outside on the playground. The 3 classroom staff person contacted Child A's parent. He spok
