If the department determines that a service, facility, applicant, licensee, child care staff member, or member of the household is not conducive to the welfare of the children, the department shall deny that application or revoke that licensee's license according to section 11.
I conducted an unannounced on-site inspection on 06/28/2023 at approximately 10:38 AM. I interviewed Licensee, Rebecca Ramlow and Child Care Staff Member 1, Rebecca Ramlow denied the allegation that she or Child Care Staff Member I Pa Child A. Rebecca Ramlow stated both she and Child Care 1 changed Child A on the day that the alleged abuse occurred and neither one of them saw any Child A's Rebecca Ramlow stated she changed Child A's the day before she did not see anything unusual. Child Care Staff Member (CCSM) 1 explained that he changed Child A's the day before. CCSM 1 did not see any Child nor anything unusual. CCSM 1 did not know why the a ega ions were being made. He did mention the parents were going through a divorce which could have caused some miscommunication between the two households. On 07/05/2023 I interviewed Child E's Mother over the telephone with Child E's Mother did not have any concerns with the care her child is receiving at the child care home. 3 On the same and I attempted to call Child B's and Child C's Mother, Child dDa'st eMWothaern ild F's Mother. I have not heard from them to date. emailed me his report on 07/31/2023. The following "inorma ion was found in report: On 06/28/2023 interviewed Registered Nurse from Registered urse stated she conducted a physical exam on Child A the night before. She did not see any marks or bruises on Child A but did observe some on the ■ area around that looked like slight irritation. She did not have any concerns that Child A was abused or neglected. interviewed Child A's Mother 1 and made face to face contact with Child A on 18/2023 at Child A's Motherl's home. Child A's Mother 1 stated she noticed Child A's =on 6/26/2023. She went on to explain that she took Child A to the child care the next day as usual. Child A's Mother 1 then spoke to a friend about the situation and decided to take Child A to the doctor that day after she picked Child A up from the child care home. Child A's Mother 1 stated the medical staff who examined Child A told her that the Child could be because of poor hygiene and there was no broken s in or any othAe'"evi ence suggesting that Child A had suffered some sort of abuse. Child A's Mother 1 explained that she had her wife are currently going through a divorce and that Child A is with her one week on and one week off. verified Child A's welfare but did not interview him due to his age an Feve opmental stage. On 06/30/2023, interviewed Child A's Mother 2. She explained that she was aware that Childwas seen by a on 06/27/2023, as she saw the information on Child A' 's Mother 2 believed that Child A's Mother I took Child o because they had an upcoming on 07/10/2023. Child A's Mother 2 did not have any concerns that an was occurring at the child care home. received a police report regarding Child A on 07/11/2023. According to the police report, there was currently no evidence of child however the investigation was still pending. On 07/12/2023, , received an email from ~~~W!Ppai lined that she reviewed Child "M ical record and his examination and did not find evidence that Child A was 4 PoMother"anPt , interviewed Child B's and Child C's Father, Child D's d Child Es Father and Child F's Mother on 07/1812023. None of the parents had any concerns regarding the care their children were receiving at the child care home. On 07120/2023, interviewed regarding the allegation that at the child care home. = did not have any concerns and stated that he was closing his case. It should be noted that ,didnot have any concerns of child abuse/ neglect occurring at the care home and closed his case without any further action.

I conducted an unannounced on-site inspection on 03/09/2023 at approximately 10:48 AM. I interviewed licensee, Rebecca Ramlow and Child Care Staff Member (CCSM)1. Ms. Ramlow denied the allegation that CCSM1 hit Child A on the back. According to Ms. Ramlow, physical discipline is not used on any of the children in care. She explained that only redirecting and time outs are used as methods of discipline at the child care home. CCSM1 also denied the allegation that he hit Child A on the back. CCSM1 explained that he only uses redirection and time outs as a way of disciplining the children in care. During the on-site inspection, I reviewed the discipline policy. The policy stated that no corporal punishment would be used on the children in care. I interviewed Child B's Mother and Child E's Father on 04/11/2023. Child B's Mother stated that her child (3-year-old male) only attends the child care home on days she has to work. Child B's Mother stated that Child B has never said anything about being hit by either CCSM1 or Ms. Ramlow. Child E's Father stated that he disenrolled Child E (2-year-old female) from the child care home. Child E's Father stated he was concerned with how CCSM1 and Ms. Ramlow interacted with the children. Child E has never complained about being hit, nor has Child E's Father seen any evidence that corporal punishment is used at the home. On 05/03/2023, I interviewed Child A's Mother and Child C and D's Mother. Child A's Mother stated that Child A (3-year-old male) told her that CCSM1 hit him on the back. Child A's Mother stated that she didn't know why CCSM1 hit Child A on the back other than he was being punished for breaking one of the rules. Child A's Mother did not see any injuries on Child A. Child A's Mother disenrolled him from the child care home. Child C and D's Mother did not have any concerns and stated that Child C (3-year- old male) and Child D (6-year-old-female) were very happy at the child care home. I conducted an exit conference with licensee, Rebecca Ramlow over the telephone on 05/03/2023. Ms. Ramlow did not have any questions regarding this allegation.

During the unannounced on-site inspection on 03/09/2023 at approximately 10:48 AM, I interviewed licensee, Rebecca Ramlow and CCSM1 about the allegation that CCSM1 called Child A, a liar. Ms. Ramlow explained that earlier that month, Child A told CCSM1 that Child C scribbled in a book. Ms. Ramlow stated that Child C did not scribble in a book, so Child A was placed in time out for lying. Ms. Ramlow stated that Child A was not called a liar. She stated that he was talked to about "telling stories" that aren't true. CCSM1 stated that Child A reported to him that Child C had scribbled in a book. Child C became very upset and started to cry. CCSM1 stated Child C had not scribbled in a book. CCSM1 sat Child A down and told him that he shouldn't tell stories because of how it affects other people. CCSM1 never called Child A, a liar but redirected him and put him into time out. CCSM1 stated that when Child A's Mother came to pick him up, he (CCSM1) told Child A's Mother that Child A was lying and telling stories. CCSM1 stated that Child A could did not hear him use the word lying or liar. During the on-site inspection, I reviewed the home's discipline policy. The discipline policy stated that redirecting and time outs on children 3 years of age and older are used as methods of discipline. On 04/09/2023, I interviewed Child B's Mother and Child E's Father. Child B's Mother stated that she did have some concerns with the way Ms. Ramlow and CCSM1 handled discipline at the child care home. She explained that Child B has never complained about the care he is receiving but she does have a "bad feeling" about the way the children in care are treated. Child B's Mother stated that she only uses Ms. Ramlow for child care when she has to work. On the days she does not have to work, she prefers to keep him home with her. Child B's Mother had no knowledge of CCSM1 calling Child A, a liar. 4 Child E's Father stated that he recently disenrolled Child E (2-year-old female) because of the way Ms. Ramlow and CCSM1 disciplined Child E. Child E's Father has no knowledge of CCSM1 calling Child A, a liar. Child E has never been called a liar or any other inappropriate name to his knowledge. On 05/03/2023 I interviewed Child A's Mother and Child C and D's Mother. Child A's Mother stated she has disenrolled Child A from the child care home. Child A reported to her that CCSM1 called him a liar. Child A's Mother stated she believed that CCSM1 didn't understand what Child A was trying to tell him and instead of listening to try and get a clear answer, he called Child A, a liar. Child A was trying to tell CCSM1 that Child C drooled on the book not scribbled on it. Child A's Mother stated when she picked Child A up on the day of the incident, CCSM1, told her Child A was a liar. Child A was standing in the room and heard CCSM1 call him a liar. She explained that Child A even asked her what a liar was when they got into the car. Child C and D's Mother did not have any concerns.

Licensee, Rebecca Ramlow and CCSM1 use time outs for children in care under the age of 3 years. Child B's Mother stated that time outs have been used on Child B since he was 2- years-old Child E's Father stated that time outs have been used on Child E, who is currently 2-years-old.
