On 06/23/2023 I conducted an unannounced onsite inspection to the child care home of Brooke Ward. There were four child care children present playing with toys in the living room with Brooke Ward. I read the complaint to Brooke Ward, who looked very confused. She reported that she does not have a child by the 2 name of Child A, and that no incident had occurred at her home with a child falling against a fence. I asked Brooke Ward the names of the children enrolled at her child care home and several weeks of attendance. Brooke Ward gave me these documents for review. Child A is not an enrolled child at this child care home and is not listed as a child that attends child care at her home on attendance records. I apologized for the confusion and told Brooke Ward I would contact the family and central office to see why her child care license and home address was listed for this complaint. On this same day I contacted Child A's Mother. I asked Child A's Mother who the name of her child care provider is for Child A. She indicated it was "Busy Bees" in Battle Creek, MI, and gave me the provider's name and complete address. She indicated she had photos of Child A's injury from the incident and was concerned with how it happened as there were four child care staff present. I asked her to text the photos to me and told her I would pass the photos on to the child care licensing consultant that was looking into this complaint or would look into this incident myself if I remain the assigned worker. I explained that this had been assigned to me because an incorrect child care home address and license number was entered and assigned for this complaint and that I oversee that particular license, but do not have Battle Creek homes. On this same day I exchanged emails with area manager, Yolanda Sims, Licensing and Regulatory Affairs/Child Care Licensing Bureau central office supervisor, Monica Sturdivant, and central office department technician, Ajla Alisic. It was determined that the complaint intake license number, provider, and facility address were incorrect, and the wrong "Busy Bees" child care home was assigned for the investigation. The complaint with the correct information was then assigned to another child care licensing consultant for investigation.
Child A fell against a fence and received facial scratches/bruises while at his child care, "Busy Bees." There is concern with supervision and whether this is how the injury actually happened.
