On 09/29/2021, I received a phone call from Program Director Tasha Sairls stating during the first weeks of school opening, they have had some issues. She stated the center has had three incidents of a child being left unsupervised. The first incident occurred on 09/13/2021 and the second and third incidents occurred on 09/23/2021. Ms. Sairls stated she would complete incident reports for all three incidents and provide them to me. Ms. Sairls stated she was originally informed that all incidents were being taken care of and she was to not be a part of it. Ms. Sairls felt like this was not right and that is why she reported them. Ms. Sairls stated the first incident occurred in the Roosevelt 1 classroom while they were outside. Ms. Sairls was in the conference room inside the building, and when she walked out of the room, Child A had opened the door from outside and was walking into the building. Child A did not have a staff member with him. She asked him what he was doing; however, he did not answer. Ms. Sairls then walked the child back outside and child care staff members Denise Drlik and Erin Ibarra were both outside and stated they did not know that Child A had even walked inside the building. Ms. Sairls then stated Child A returned to his class outside. The second incident occurred in the Roosevelt 4 classroom. Ms. Sairls stated she was not involved in this incident, but when she spoke to staff members, they stated that Child B returned to the classroom from the bathroom and then walked back out into the hallway where she was found by child care staff member Sheryl Hehrer and she returned Child B to his classroom. 3 The third incident occurred the same day on 09/23/2021 in the Roosevelt 3 classroom. During this time, Ms. Sairls stated she was not involved in this one either, and she only knows what she was told. She stated that child care staff member Lisa Richards left the classroom to go get lunch from the kitchen. During this time Child C was found in the hallway by family advocate Rhonda Perkins and was returned to the classroom. Ms. Sairls stated that all staff members involved in all three incidents were written up and given 10 days of no pay. She stated that due to the incidents and staffing issues, the facility closed from 09/28/2021 to 10/11/2021. She stated none of the families have been contacted at this time. On 10/14/2021, I spoke to Ms. Sairls via telephone, and she stated she will speak to administration about providing me phone numbers for all staff members as she did not feel she was able to provide this to me. She stated administration wants her to stay out of all incidents and wants her to stay "protected". She stated she will email secretary Jean Hamlin and have her send over all phone numbers. A few hours later, Ms. Sairls and Ms. Hamlin emailed all staff member phone numbers. On 10/14/2021, I spoke to Child B's mother via telephone, and she stated she received a call last week one day but could not remember what day exactly from Ms. Sairls; however, she was not provided a lot of information. She stated she was informed that Child B went into the hallway and one of the staff members brought him back to his classroom. Child B's mother stated this was the first time she had ever received a phone call like this or something like this happened with Child B. She stated Child B is very "adventurous" and may have forgot what class he was in and needed to return to. Child B's mother stated she is not concerned about it and feels that mistakes happen. Child B is always getting into things and running off at home and she understands what happened. She stated she really likes Roosevelt and all of her older children attended there as well. Next, I spoke to Child C's mother via telephone on the same date. She stated she was informed of the incident about a week after it had happened. Child C's mother stated she was only told that Child C was found in the hallway by a different caregiver than his caregiver, and he

On 09/29/2021, I received a phone call from Ms. Sairls stating that over the past few weeks Roosevelt Head Start has had multiple incidents resulting in children being left unsupervised. She stated the first incident occurred on the first day of school; 09/13/2021 and the other two occurred on 09/23/2021. Ms. Sairls stated she did not call to inform me of the incidents within 24 hours of them occurring due to being advised by upper management that it was being taken care of "in house" and it was not her responsibility to call licensing. Ms. Sairls stated multiple times on the phone that she was being "taken out of the situation to protect her" from the staff members. She stated that they did not want her involved so that she was protected due to working with the staff members daily. Ms. Sairls also stated she did not send in an incident report within 72 hours of any of the three incidents for the same reason. She stated the employment of two staff members have been terminated due to the situation, and all others involved were suspended with no pay for 10 days. At this time the center is closed due to staffing and these incidents occurring and will be closed until further notice. Ms. Sairls acknowledged she did not think this was right to not report the missing children to licensing and that is why she was calling today. She stated none of the families had been contacted at this point either, and she was requesting guidance on what should happen now. Ms. Sairls was advised to fill out incident forms for all three incidents, contact the families and provide licensing with all staff members names and phone numbers. At this time, Ms. Sairls did not have any further information for me regarding the incidents due to not being involved in the investigations. On 10/14/2021, I made phone contact with Ms. Sairls to request all staff phone numbers again. She apologized for not sending them and stated she will contact management to get the numbers sent through. She stated Roosevelt Head Start had reopened on 10/11/2021 and that some of the families have been called that were involved in the incidents. She stated one of the families disenrolled while the other two children are still enrolled in the program. 10 On 10/15/2021, I received an email from Ms. Sairls providing all three incident reports. She also stated she has made phone contact with all families.
