On 07/10/2023 I completed an onsite inspection at the group child care home. I interviewed licensee Jesus Colan. She and her child care staff member have been walking the children across F-41 on Tuesdays to attend a library program on the old Air base. She reported she had thirteen children, four of which were related to either her or the other child care staff member. I advised her that walking children across F-41 was too dangerous due to the 55 miles per hour speed limit, and the lack of sidewalks and crosswalks. She reported they all wear reflective vests, and she has been considering getting a hand-held stop sign for additional safety. She stated she would rather drive but they have to walk when they have too many children to fit in her car. I provided consultation on transportation rules and advised her that with parent permission and following the transportation rules she could have two cars, one driven by her and the other driven by her additional child care staff member. Jesus Colan reported she was unaware that they could both transport children with permission and said that is what they will do going forward.

On 02/17/2023 at approximately 9:40am Child A was bitten/ scratched by Ms. Colan's dog. Child A had red marks and swelling by her right eye. I completed an onsite inspection on 03/02/2023 at the Wawa Wasi Group child care home. Ms. Colan reported she was in the kitchen area at the time of the incident and child care staff member Ms. Danielle Story was in the bathroom changing a diaper. Ms. Story corrected Ms. Colan and said she was in the living room within arm's length of Child A and the dog at the time of the incident. Ms. Colan stated she did not see the incident, but she thinks Child A was trying to pet the dog. She heard Child A cry and turned around and saw Child A by the dog. Child A had red marks and swelling on her upper cheek by her right eye. There was no blood or puncture wounds. They put ice on the area and contacted Child A's Father. Ms. Colan stated the dog has been more anxious and upset since she opened the group license in November because another child has been pulling the dogs tail and being mean to the dog. I advised Ms. Colan that the dog cannot be around children in care anymore. As soon as she noticed the dog was more anxious and upset, she should not have allowed the dog near children in care. Ms. Colan stated the dog has been kept outside or in a cage since the incident and will be kept away from children in care from now on. I interviewed child care staff member Ms. Danielle Story on 03/02/2023 while onsite. Ms. Story reported she was within arm's length of Child A and the dog at the time of the incident. She did not see the incident because she was facing the other direction. She heard the dog bark. It was not a "nice bark" and then Child A started to cry. The dog was on a toy couch for children at the time of the incident. 2 That toy couch was the only piece of "furniture" for children to sit on in the living room during the onsite inspection on 03/02/2023. There were seven children in care, and they had one small bin of dinosaurs to play with. The other toys were in bins with closed lids on shelves behind cloths, so they were not visible. I advised both Ms. Story and Ms. Colan that the children needed to have direct access and be allowed to play with a sufficient supply of toys. I instructed Ms. Story to get more bins of toys out for the children to play with which she did. On 04/14/2023 I interviewed Child A's Father. Child A's Father reported initially after the incident they had concerns about the dog and Child A's Mother talked to Ms. Colan about their concerns. Since Child A's Mother talked to Ms. Colan the dog has been kept separate from the children in care. They no longer have concerns now that the dog is not by the children.

On 12/21/2022 an incident report was received from licensee Jesus Colan. Ms. Colan was getting ready to cook. She asked the children to clean up the toys. She was on her phone looking for a clean-up song when she saw Child A (male, 3 years) and Child B (male, 3 years) hiding in the closet. She asked the children to get out of the closet. Child A started taking his clothes off and went back 2 in the closet with Child B and closed the door. She overheard Child B say, "can I suck your pee pee". On 12/29/2022 I completed an on-site inspection at Ms. Colan's child care home. I observed an open floor plan; the kitchen, dining room and living room are all one room. There is a small coat closet located in the living room. I reviewed attendance records dated 12/21/2022. When the incident occurred, Ms. Colan was caring for four children. I interviewed Ms. Colan. Child B just started in November 2022. He came from another child care that closed. She and child care staff member Danielle Story have been redirecting Child B not to play in the closet since he started care. On 12/21/2022 Child A and Child B were in the closet. She asked the children to get out of the closet. Child A came out of the closet and said " Ms. Jackie look at me I'm naked". Child A then shut the door. She heard child B say, "can I suck your pee pee". She opened the closet door and Child B was bending down toward Child A's private parts. Child B was clothed. This was the first incident of this kind. She called Child A's mother and Child B's mother. She terminated Child B's child care. On 01/12/2023 I interviewed child care staff member Danielle Story via telephone. She was not present on 12/21/2022 when the incident occurred. She had redirected Child B out of the closet on several occasions but there had never been any previous incidents. She believes children receive appropriate care and supervision at Ms. Colan's child care home. On 01/12/2023 I interviewed Child C's mother and Child D's mother separately via telephone. Neither had any concerns regarding the care and supervision children receive there. On 01/12/2023 1 interviewed Child B's mother via telephone. On 12/21/2022 she received a call from Ms. Colan stating that Child B had asked another child to go into the closet and take his clothes off. On 12/22/2022 Ms. Colan then told her that Child B asked the other child to "suck on his pee pee". Child B reported to her that he heard that talk from his other child care. Child B watches YouTube and may have seen something there. There had never been an incident like this at Child B's past child care facility nor in the month he attended Ms. Colan's child care home. She has no concerns about the care Child B received at Ms. Colan's child care home. Child B's child care was terminated after the incident. On 01/13/2023 1 interviewed Child A's mother via telephone. On 12/21/2022 she was notified of the incident by Ms. Colan. Child A reported that Child B told him to pull his pants down and then Child B "sucked my pee pee". There were no prior incidents. She has no concerns about the care Child A receives at Ms. Colan's child care home. 3
