The information gathered through interviews and documentation does indicate that program staff did not inform parent of feeding time at time of pick up.

On 12/16/25 Teacher 4 did not provide appropriate care and supervision to Child B. Child B’s finger was caught in the door requiring medical attention.

The licensee designee was informed that Child B received medical attention on 12/19/2025. A verbal report was not submitted to the department within 24 hours of the notification of medical treatment.

The information gathered through interviews and evidence does indicate that program staff were wrapping children in blankets inappropriately.
