On April 18, 2023, I spoke with the complainant. The complainant noted the incident with Child A on April 19. 2023, was observed on the classroom camera around 8:45 a.m. The Pedal classroom is also the opening classroom and therefore children from other classrooms could be present at this time. The spray bottle the complainant observed on the camera had a label, but the complainant was not able to see what the label said on the camera feed. With Child A. Child Care Staff Member 1 sprayed his food 5-10 times and then Child A kept eating the food after it was sprayed. Child Care Staff Member 1 continued to clean tables. There was another child care staff member in the classroom on the other side of the classroom. On April 20, 2023, and I completed an on-site inspection, brought me up-to-speed on a telephone conversation she had with program director Kali Wiseman in that the cameras in the center do not have audio only video and they do not record. During pection, Ms. Wiseman logged into the live feed camera system and and I observed the classroom on the camera system. We noted the vide is very small and cannot be expanded to a larger view, this limits visibility. The camera in the classroom does not move and is directed at the main table in the classroom where children eat and do activities. On April 20, 2023, and I spoke with program director Kali Wiseman. Ms. Wiseman stated she has been working for CT DBA Gilden Woods Okemos (hereinafter "the center") for 13 years and has been in her current role as program director for two years. read the allegations to Ms. Wiseman. Ms. Wiseman confirmed Child Care Staff Member 1 was the staff member cleaning the tables while a child was eating cereal on April 17, 2023. Program director Kali Wiseman was able to articulate the process used to clean the tables, washing with warm soapy water, rinsing with warm water, and using a bleach/water solution to sanitize. She said these three steps are done with spray bottles which are each individually labeled with what is inside them. The labels are the same size and color, the only difference being the word(s) on the label. Child care staff members normally do not clean tables while the children are at the table, they do this after the children have all left the table and/or before they come to the table. All child care staff members are trained on the three-step cleaning process as part of their new hire orientation process. This is a topic that also appears during in- service days. Ms. Wiseman stated that Child Care Staff Member 2 was the staff member involved in the allegations regarding Child B and Child C. Ms. Wiseman stated there have been no complaints or concerns regarding either Child Care Staff Member 1 or Child Care Staff Member 2. On April 20, 2023, and I spoke with Child Care Staff Member 1. Child are Staff Member 1 stated she has been at CT DBA Gilden Woods Okemos for a year and she has been the lead caregiver in the Pedal classroom for two months. She works mainly with Child Care Staff Member 2 and they have up to eight children at a time in the classroom. On April 17, 2023, Child Care Staff Member 1 was working when the room opened around 7:45 a.m. This classroom is an opening classroom, so children from other classroom(s) are in that classroom to start their day. At approximately 8:30 a.m. the children dispersed to their own classrooms. She knows on that day there were six children in the classroom after 8:30 a.m. and Child Care Staff Member 2 was also with her working. On April 18, 2023, I spoke with the complainant. The complainant confirmed two other concerns that were also observed on the classroom camera feed. During this same time Child Care Staff ember 2 was also observed Child C b W her. It is unknown if there were any injuries. The incidents with d Care mber 2 were also observed on the camera live feed. On April 20 2023, and I completed an on-site inspection, said she spoke with e complainant, and she reported to the complainant e incident with Child Care Staff Member 2 and Child C appeared to be normal actions as Child C was upright, her feet were on the floor, and Child C and Child Care Staff Member 2 moved in tandem with one another. There were no visible injuries, the child did not cry, and got up and continued to play. As to the incident with Child B and Child C, these occurred on picture day (1-2 months ago). On April 20, 2023, ■ and I spoke with program director Kalie Wiseman. Ms. Wiseman said are Staff Member 2 was the staff member involved in the allegations regarding Child B and Child C. Ms. Wiseman stated there have been no complaints or concerns regarding either Child Care Staff Member 1 or Child Care Staff Member 2. Ms. Wiseman stated she received a telephone call from a parent who reported seeing the incident with Child B on the live feed. She pulled up the live feed while she was speaking to the parent, and she did not see anything unusual; the cameras do not record. Ms. Wiseman stated she went to the classroom in person after the telephone call and did not observe any children upset, with injuries, or any concerning behaviors. She spoke with both the child care staff members in the classroom at the time, Child Care Staff Member 1 and Child Care Staff Member 2. They both said no child was grabbed, no child was dragged, no child was pushed, and no child fell. They were both surprised and stated they had no idea what the parent that called was referring to. Ms. Wiseman stated the Lyric and Pedal classrooms are combined in the morning from opening until 8:30 am. When the concerned parent called her, the classrooms were no longer combined. When escorting a child or helping a child move from one place to another, she takes children by their hand and leads them (if necessary), and she knows this is the way Child Care Staff member 2 does it as well. She stated she never dragged a child and never would, nor has she ever seen Child Care Staff Member 2 drag a child. She has never seen Child Care Staff Member 2 be aggressive with any child in any way at all. She has seen Child Care Staff Member 2 sigh loudly when she is frustrated, she scoops children up more quickly than she needs to for the situation, and this is something she has addressed with her. She has never seen Child Care Staff Member 2 take her frustrations out on any child. Historically, transitions are a more frustrating time, it is harder with more negative behaviors; this makes it easier to become frustrated. Child Care Staff member 2 has a happy disposition when she comes in for work. Child Care Staff Member 1 said there have been times where both she and Child Care Staff Member 2 have stepped backward and bumped into a child, knocking them over; this was never intentional. She has also seen where both she and/or Child Care Staff Member 2 help push a child's chair in and the child is on the edge of the chair they fall over;

On February 5, 2023, I spoke to Child A's Mother. She stated that on February 2, 2023, she received a telephone call from program director Kali Wiseman informing her that Child A (male, age 2) sustained an injury to his wrist and asked if she wanted to pick him up. Ms. Wiseman told her that while Child Care Staff Member 1 held Child A's (male, age 2) hand he pulled away and they both fell on the floor and Child Care Staff Member 1 fell on Child A. She stated that she immediately went to the child care center and when she arrived Child Care Staff Member 1 was holding Child A applying ice to his wrist. When Child A's Mother asked Child Care Staff Member 1 what occurred she told her that while holding Child A's hand and walking him to pick up the LEGO's, he pulled away and threw himself on the floor falling on his hand. Child Care Staff Member 1 denied falling on Child A. Child A's Mother stated that she transported Child A to Sparrow Hospital, and he was diagnosed with a sprained left wrist. She stated that he must keep his arm in a sling for a few days, but he is able to move his fingers. Child A's Mother was upset about Child A's wrist being sprained and was concerned that Child Care Staff Member 1 was not telling the truth about how the sprain occurred. I observed Child A present at the child care center. He was observed playing in the classroom with a sling on his left arm. On February 6, 2023, I completed an unannounced on-site inspection. I interviewed program director, Kali Wiseman. Ms. Wiseman stated that Child Care Staff Member 1 and Child Care Staff Member 2 were present in the classroom on February 2, 2023, when the incident occurred with Child A. She stated that Child Care Staff Member 2 was out ill today, and Child Care Staff Member 1 is on vacation this week. Ms. Wiseman stated that on February 2, 2023, Child Care Staff Member 1 called her from the classroom and informed her that Child A had fallen and "appeared to be favoring his left wrist." Ms. Wiseman took an ice pack to the classroom. She stated that when she arrived at the classroom Child Care Staff Member 1 told her that Child A was throwing toys at the other children in the classroom. As Child Care Staff Member 1 held his hand and attempted to walk with him to pick the toys up from the floor Child A pulled away and threw himself on the floor falling on his hand. Ms. Wiseman called Child A's Mother and informed her of the incident. Ms. Wiseman stated that she did not tell Child A's Mother that Child Care Staff Member 1 fell on 3 Child A. Ms. Wiseman stated that she has never observed Child Care Staff Member 1 to be physically aggressive with any child or have concerns regarding the care and supervision provided by her. Ms. Wiseman stated that there are cameras in the classroom that do not record but provide a livestream for parents. On February 12, 2023, I spoke to Child Care Staff Member 2. She stated that on February 2, 2023, Child A was throwing toys and Child Care Staff Member 1 held his hand to lead him to the toys that needed to be picked up. Child A pulled away and began crying and threw himself on the floor and fell on his hand. She stated that Child Care Staff Member 1 attempted to console Child A. While Child Care Staff Member 1 held Child A, she observed him not moving his hand and when she touched his hand he began to cry. Child Care Staff Member 1 called program director Kali Wiseman to bring an ice pack to the room for Child A's wrist. Ms. Wiseman called Child A's Mother and informed her of the incident and Child A's Mother arrived approximately ten minutes later and picked him up. Child Care Staff Member 2 stated that Child Care Staff Member 1 held Child A's hand gently and did not touch him aggressively. She also stated that she has never observed Child Care Staff Member 1 to be aggressive with any child or have concerns regarding the care and supervision provided by her. On September 12, 2022, I called Child A's Mother to discuss the allegations in the complaint. Child A's Mother was aware of the incident that occurred with her daughter on September 8, 2022. Child A's Mother stated that someone from the center called her to let her know Child A was left in the classroom alone for a few minutes when the rest of the classroom went outside. Child A's Mother went on to say that she does not believe it was a situation of neglect and 2 was not surprised by what happened. She is positive that her daughter intentionally was left behind or hid, as she is very mischievous and likes to do things like this. On September 12, 2022, I made an unannounced onsite inspection to the center. I spoke to the program director Kali Wiseman and made her aware of the allegations in the complaint. Ms. Wiseman stated that she was aware of the incident in the complaint but was not in the center the day of the incident. She stated that her assistant Rosanna Vaquera was present and handled the situation. During my onsite inspection on September 12, 2022. I interviewed assistant director Rosanna Vaquera. Ms. Vaquera stated that child care staff member Julia Armbruster came up on lunch break and told her Child A was left in the Terrance classroom alone. Child care staff members from the Petal classroom were coming inside and saw Child A at the door of the Terrance classroom. Child A has hid in the classroom before but she has never been left alone in the classroom before. I asked Ms. Vaquera for the names of the child care staff members involved in the incident. Ms. Vaquera stated that child care staff members Brittnee Knight and Miranda Williams were the two child care staff members working in the Terrance classroom. Ms. Williams does not regularly work at this location and was substituting from the New Hudson location. Child Care Staff members Nakia Taylor and Julia Armbruster work in the Petal Classroom and were the ones who saw Child A in the classroom alone. During the onsite inspection I interviewed child care staff member Brittnee Knight. Ms. Knight works in the Terrace Classroom. Ms. Knight was aware of the incident in the complaint. She stated the child care children lined up to go outside. Miranda [Miranda Williams] opened the door to let the child care children outside, where the children lined up at the fence. Ms. Knight was at the back of the line and counted 11 children in front of her and one behind her. I asked if the one behind her was Child A? Ms. Knight said no, Child A was in front of her in line with the other children. The one behind her came around and also exited the classroom. Ms. Knight stated she was not sure how Child A snuck back into the classroom. Since this incident, Ms. Knight said she developed a new system for counting the children. The children will sit on the carpet and Ms. Knight will call the children up one by one to line up and then count the children before they leave the classroom and again when they are outside. During the onsite inspection I interviewed child care staff member Nakia Taylor. Ms. Taylor works in the Petal Classroom and was aware of the incident in the complaint. Ms. Taylor stated the Petal classroom was outside at approximately 9:15a.m., when the Terrance classroom was starting to come out and at 9:30a.m. the Petal classroom started going inside. Child Care Staff Member Julia Armbruster said to her "What's up with {Child A} I just found her at the door of the classroom alone." Ms. Taylor then went over and told Ms. Knight {Child A} was in the classroom alone. Ms. Taylor stated {Child A} had to be alone for approximately ten minutes and was scared when Ms. Julie [Julia Armbruster] found her. 3 During the onsite inspection on September 12, 2022, I interviewed child care staff member Julia Armbruster. Ms. Armbruster stated she is aware of the incident in the complaint, and she works in the Petal classroom. Ms. Armbruster stated the Terrace classroom was outside on the p

On August 31, 2022, I made an unannounced onsite inspection to the child care center. I discussed the allegations in the complaint with the program director Kali Wiseman. Ms. Wiseman stated that the phone application that parents use provides a 20-minute video of the classroom, therefore if she did fall asleep, she was not observed for 30 minutes. Ms. Wiseman stated that the child care staff members are good about transferring children to their cribs when they fall asleep in cribs or chairs. During the onsite inspection on August 31, 2022, I observed the infant one classroom. During the onsite inspection I did not see any children sleeping in any unapproved equipment. At the time of arrival Child B (male, age 6 months) was asleep on his stomach in his crib. Ms. Wiseman said Child B can roll from his back to his stomach on his own. During the onsite inspection on August 31, 2022, I interviewed child care staff member Katera Henderson. Ms. Henderson stated that she has been working at the center since September 1, 2021. Ms. Henderson said she has been trained in safe sleep. Her understanding of safe sleep is when putting a baby down to sleep, the babies who can't roll must be put on their backs. Ms. Henderson said if the baby can roll, they can sleep on their stomach. I explained to Ms. Henderson that even if a baby can roll, they are to be put on their backs to sleep then allowed to roll over if they did it on their own. Ms. Henderson stated that she has never seen any of the infants asleep in a swing or bouncer. During the onsite inspection on August 3, 2022,1 I interviewed child care staff member Desiree Tho-Baiz. Ms. Tho-Biaz stated she usually works in the Lyric classroom which is a toddler classroom for 1 -2 years old, however she has 1/2 recently been working in the infant one classroom from 3-5p.m. Ms. Tho-Biaz stated she has been trained in safe sleep. Ms. Tho-Biaz stated that on August 29, 2022, there were two infants in swings, Child A and Child D. Child A was fighting sleep and was placed in her crib. Child D was falling asleep but was then picked up by her father. Ms. Tho-Biaz was not in the classroom when Child A was picked up from the center and did not know whether she was placed on her back or stomach when she was placed in the crib. 3 During the onsite inspection on August 31, 2022, I interviewed child care staff member Megan Dake. Ms. Dake stated she is currently in training. She has been in the Infant two classroom, but on August 29, 2022, she worked in the Infant one classroom from 4:00p.m.-5:45p.m. When she was in the classroom one of the infants [Child A] fell asleep in the swing and was moved to her crib. Ms. Drake said moved "Desiree" moved Child A to her crib and "she laid her on her stomach because she can roll." During the onsite inspection I re-interviewed child care staff member Desiree Tho- Biaz. I explained to Ms. Tho-Biaz I wanted to clarify the information she provided in the first interview. Ms. Tho-Biaz confirmed she moved Child A from the swing to the crib. Ms. Tho-Baiz said she laid Child A down in the crib on her stomach and now realizes that was a mistake. During the onsite on August 31, 2022, I viewed the infant safe sleep training certificates of the infant child care staff members. I conducted an exit conference with program director Kali Wiseman and discussed the findings of the investigation. informed Ms. Wiseman that all infant child care staff members needed to be retrained in infant safe sleep, as they do not have the correct understanding of how infants must be placed to sleep. On September 1, 2022, I called Child A's Mother and discussed the allegations in the complaint. Child A's Mother stated that she saw her daughter sleeping in a infant swing on live stream, she watched a few minutes and about 3:00p.m. she started getting ready to leave work to go get her. She left work between 3:15p.m. and 3:20p.m. to go pick Child A up from the child care center. Child A's Mother stated Chi
