On 12/15/2022, the allegations on Premier Academy were originally called into the Health Department but were forwarded to the department for investigation. No information was given to contact the original complainant for further information. On 12/16/2022 I completed an unannounced onsite inspection at the center. I spoke with program director, Stephanie Kline, assistant program director, Erica McBuney, and licensee designee, Rochelle Scott. They stated that both children and staff have been sick lately, but nobody has been allowed, much less forced to stay when they did not feel good. Ms. McBuney stated that she has doctor's notes when children and staff are allowed to come back, but the notes do not confirm which illness they had except for those that test positive for covid. She does not recall the 2 last time anyone had a communicable disease, but the policy shows that they notify parents and staff by posting it in the communications app and placing a notice on the door. I reviewed the policy onsite and confirmed this information. Ms. Kline and Ms. Scott reassured me that when staff are not feeling well, they are sent home. Staff do not tell them if they have diarrhea, so they do not know for sure if anyone has had that while they were working recently. They confirmed that nobody is ever made to stay at work when they are ill. Ms. Kline stated that in the past week, there were two staff that got bodily fluids from the children on their clothes and they both went home to shower/change and come back. I spoke to staff member, Charlotte Morrison. She stated that a child threw up on her and it got in her hair. Ms. Morrison stated that since she already had the vomit on her, she sat with the child until he was picked up and she instructed the other staff to take the rest of the class to the gym. Ms. Morrison stated she cleaned up the best she could and comforted the child until he was picked up. This took about 35-40 minutes. Once the child left, she also left to go home and shower. She returned to work later that afternoon. Ms. Morrison stated she knew she could have left immediately but it was her idea to stay a short time with the sick child and not leave right away because it made more sense for the rest of the staff and children to stay away from the sick child. I spoke with staff member, Marylou Hernandez. She stated she had picked up an infant that had a diaper blow out and it soaked onto her sleeve and her pants. She went home immediately to change clothes and came back. The center would never make someone stay with body fluids on them.

On 12/16/2022, I spoke with kitchen manager, Kristian Parsons. Ms. Parsons stated that the food is prepared onsite and then immediately served to the classrooms. I observed the kitchen and saw the food warmer. This keeps food warm at 175 degrees until it is served. The cold items stay in the refrigerator that stays under 40 degrees. There is a thermometer in the refrigerator and a food thermometer is used to measure the temperature of the warm food before it is served to the children. Food temperature is logged on a daily chart. I did not observe any food sitting out at room temperature. Program director, Ms. Kline, denied that food is ever sitting out at room temperature. I observed the classrooms. There was no food sitting around.

On 12/16/2022, Ms. Kline, Ms. Scott, and Ms. McBuney all denied that there was leaking in the roof anywhere in the center and said there was no mold anywhere. I walked around the center and did not see any evidence of ceiling damage or leaks. The center was clean and updated. I saw no mold. In the infant room, staff members, Alyssa Orlo and Jacklyn Wheaton, confirmed there were no leaks or mold in the infant room. They have not seen any water damage in the room. The infant room was very clean and organized. I saw no evidence of any old or new leaks and did not see any mold. The center received an "A" rating on their last environmental health inspection dated 6/16/2021.
