On 02/21/2023, I completed an unannounced onsite inspection. Present for this onsite was. I spoke with program director, Gabriele Paul, and child care staff members (CCSM) A. B and C. Ms. Paul was aware of the allegations regarding the injury to Child A's cheek area from CCSM C. She shared a family member questioned CCSM C about an injury to Child A's face during drop off the following day, which was February 9, 2023 after the injury was observed. In addition, the designee, Patricia Adams received a call from Child A and B's parent the following morning. She shared the family reported the injury as occurring on February 8. 2023. Everything was brought to her attention on February 9, 2023. Ms. Paul shared an incident report was never completed because there was no record of Child A having a bruise on her face during her time in care, and there was no incident of her falling or having an injury while in care. She provided CCSM's A, B and C as the staff member providing care for Child A on February 8, 2023. She shared Child A and Child B were only enrolled in care for approximately one week before this incident. She shared Child A was brought back to care on February 9 (the day after the family member said they observed the bruise). It was at this time the bruise was observed, however, the staff members working with Child A the prior day denied that the bruise was present while she was in care. Ms. Paul denied having any concerns regarding the care and supervision of the three staff members identified as working with Child A on February 8, 2023. She denied any disciplinary action, nor has she seen concerns with their interactions with children. She shared an incident report was not completed and provided to the parent because there was not an incident that occurred while Child A was in care to note and provide. I contacted CCSM A. She was made aware of the bruise on Child A's cheek area from Ms. Paul. She confirmed working in and out of the infant room on February 8, 2023. She said Ms. Paul showed her a picture, and at no time did she observe the bruise shown to her on Child A. She specifically recalled covering lunch and walking in the room a couple times to interact with the children. While in the room, she denied observing Child A fall or have injured herself. She shared she and CCSM C talk about what's going on in the classroom often, and anytime she covers a lunch or restroom breaks, the other CCSM's always communicate anything that she needs to be aware of. I contacted CCSM B. She identified herself as the lead caregiver in the room. She shared the day in question, February 8, 2023, she worked in the room from 6am until 3pm. Child A and Child B came in around 9am that day. She denied Child A had any visible marks on her face during drop off. She shared this is something she always pays close attention to with any child during drop offs because she wants to address any immediate visual concerns with the parent at that time. She denied ever speaking to the mother or any family members about the bruise. However, it was brought to the attention of CCSM C on February 9, 2023, when Child A's Aunt brought her in the following morning. She shared Child A is a typical mobile . She pulls up on things and is an active crawler. She denied Child A ever getting injured during her short time of enrollment (which she said was about a week "or so."). She confirmed when she left at 3pm, Child A had no visible marks/bruising on her face. She also denied any observations of Child A falling or hitting her face on anything. She went on to share Child A typically only cries when she's getting hungry. I contacted with CCSM C who works in the infant classroom. CCSM C shared on February 8, 2023, Child A's Aunt picked Child A and Child B up from care. She was working and able to interact and gather Child A and Child B. She denied at this time Child A having any marks/bruises on her face and confirmed Child A's Aunt never brought anything to her atten

The infant classroom was out of ratio on 02/08/2023 with six infants to one staff member for 1h r and 43mins. The toddler classroom was observed out of ratio with five toddlers to one staff member on 02/21/2023 for 1h r and 11m ins.

Based on the demonstration shown to me by Ms. Paul, there appeared to be approximately 1 foot or less of space in between the diaper changing table and the highchairs places on both sides of the changing table.

The facility did not have attendance records to indicate each classroom's head count, however, the child care staff member indicated she was alone with five children from approximately 11:20am until the incident occurred at 11:55am.
