Monitoring of Compliance: • Daily administrative spot-checks and weekly unannounced audits will focus specifically on verifying consistent staff compliance with proper handwashing and hygiene practices. • Administrators will document compliance observations, promptly addressing and correcting any deviations during weekly administrative meetings. How Compliance Will Be Maintained: • Clear visual reminders outlining mandatory handwashing protocols have been posted near sinks and throughout all classrooms in the facility. • Weekly quick-talk sessions and bi-weekly structured training sessions will continue to reinforce handwashing, hygiene practices, and bloodborne pathogens protocols, ensuring consistent adherence. • All new hires will participate in orientation explicitly covering detailed handwashing policies and bloodborne pathogens procedures, with signed acknowledgment documented in their personnel files.     Based on the interview with LC 1 and the videos related to the incident, I do not find LC 1 conducive to the welfare of children based on the following: • LC 1 did not provide appropriate care to Child A when she pulled Child A's finger out of her mouth eight times, threw Child A's pacifier into the crib, pushed the crib with her leg, yelled at Child A that she was not going to clean up her throw up, did not make an effort to comfort Child A in distress, and forcefully patted her to sleep. • LC 1 failed to provide appropriate supervision when she was on the phone while working with the children. • LC 1 showed more anger and concern that the previous employer shared a video of her being inappropriate with a child, even though she appeared remorseful for being aggressive in her interview. • LC 1 was not aware of safe sleep practices and talked about disciplining infants/toddlers by restricting them even though she has worked in the child care field since 2005 and has a current CDA in infant/toddler care. • LC 1’s attitude and actions displayed on the videos and the interview are not conducive to the welfare of the children.

    On 4/5/2024, I spoke with the Complainant, who stated that there have been several occasions where there is no milk to give the children. This would occur every couple of weeks where the center would not have milk for a couple of days. 2 LANDMARK BUILDING • 105 W. On 4/12/2024, I observed the Infants, Toddlers, and Preschooler having milk with lunch. On 4/12/2024, Ms. Jefferson stated that there is always milk at the center for the children. On 12/06/2024, I observed the Infants, Toddlers, and Preschooler having milk with lunch. On 12/6/2024, LC 1, LC 2, LC 3, and PD stated that the center has always provided the children with milk and there has never been a time when there was not milk at the center for the children.

    On 4/5/2024 Complainant stated that the Infant classroom has ten children with one CCSM. She did not have any specific dates, but it was witnesses by everyone working at the center. On 4/12/2024, There were ten children present with LC 1, CCSM 3, CCSM 4, and CCSM 6. The CCSM to child ratio was maintained. Infant and Young Toddler classroom is approved for 16 children ages birth to 18 months. On 4/12/2024, Ms. Jefferson stated she has staff that left the center, and she was short staffed, but she has several interviews this week with what seems to be well qualified individuals. She is confident that she will be overstaffed (at least by 2 people) before the end of this week. Hiring fast is not something Ms. Jefferson likes to do, but she will move swiftly. On 12/6/2024, LC 1 stated that in March of 2024 there would be ten children with two CCSM’s. To maintain compliance another CCSM was required but the center was short staffed because CCSM’s would call off, they would come to work late. LC 1 was not sure how frequently this happened but the ratio issues have improved because three to four new CCSM’s have been hired over the summer and since that time the Infant classroom has maintained the required 1 to 4 ratio. On 12/6/2024, the PD 1 stated that she was not aware of the classrooms being out of ratio.

    On 4/5/2024, I spoke with the Complainant, who stated that a CCSM was sleeping in the Infant classroom while feeding an Infant their bottle. She was holding the child while sleeping. The Complainant did not know the name of the CCSM that was sleeping. On 4/12/2024, I did not observe a CCSM sleeping in the Infant or any of the other classrooms at the center. On 4/12/2024, Ms. Jefferson stated that there was a temporary employee from ECE Subhub that had fallen asleep in the classroom. The employee was not allowed to return to the center because she was sleeping in the classroom. Ms. Jefferson provided the email correspondence between herself and Beth Pierson from ECE Subhub. This incident occurred on 7/13/2021. Per the email exchange Ms. Jefferson was not charged for the Subhub employee that was sleeping at the center. Since that incident no CCSM’s have been sleeping at the center and if CCSM’s were sleeping at the center they would be immediately terminated. She does not allow the staff to sleep at the center. This incident occurred three years ago and there was not a current incident. On 12/6/2024, Lead caregiver 1 (LC) stated that CCSM 1 was sleeping in the Infant classroom in the spring of 2024 and she was immediately fired by Ms. Jefferson. CCSM 8 was the only CCSM that she was aware of sleeping at the because sleeping is not allowed at the center. On 12/6/2024, LC 2, LC 3, and PD 1 stated that they have no knowledge of a CCSM sleeping in the Infant of any of the classrooms. On 1/7/2025, Ms. Jefferson stated that CCSM 1 was sleeping in the Infant classroom on 6/28/2024 and she was immediately terminated for “violation of company policies”. On 1/7/2025, I called CCSM 1 and her phone number is not working.

She is confident that she will be overstaffed (at least by 2 people) before the end of this week. Hiring fast is not something Ms. Jefferson likes to do, but she will move swiftly. On 12/6/2024, LC 3 stated that in March or April of 2024 a CCSM left the center during lunch and did not return, leaving LC 3 alone with fifteen children in the Preschool classroom. The CCSM left the center at around 12:30 pm and she was alone with fifteen children until 4:00 pm when the children started going home. There was no one at the center that could help in the Preschool classroom. On 12/6/2024, the PD stated that she was not aware of the classrooms being out of ratio.

Two CCSM’s left the center in April 2024 and that was why the Toddler classroom was out of ratio. LC 2 was not sure when the two new CCSM’s were hired for the Toddler classroom. Since they were hired the Toddler class has maintained the required 1 to 4 ratio. On 12/6/2024, the PD 1 stated that she was not aware of the classrooms being out of ratio.
