Based on my interviews with the Program Director and parents and review of children’s records, the required ratio of 1 staff to 4 was not maintained on 03/20/2025 when Child A was in attendance.

Based on my interview with the PD and according to the children’s attendance record, on 03/20/2025, only 1 adult, the PD, was present from 8:20am to 3:30pm when Child A, Child C, and Child D were present.
