    R 400.8125 · R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On 09/18/2023, I made an unannounced onsite inspection at the child care center from 10:27 a.m. to 12:03 p.m. I interviewed program director, Marcella Carter. Ms. Carter informed me that the incident took place on 09/13/2023 around 5:10 p.m. after she had left the child care center for the day. The following morning, Child Care Staff Member (CCSM) 1 asked her if CCSM 2 informed her of the incident that took place after she left yesterday. Ms. Carter stated that CCSM 1 told her about Child A being left unsupervised in the restroom during transition as they were returning inside the child care center from outside time. Ms. Carter stated that she called Child A’s Father and apologized for the incident and asked about Child A’s well-being, then notified me of the incident and followed up the phone call with an incident report. I asked Ms. Carter of her understanding of what led up to the incident, and Ms. Carter replied, it was the end of the day, and the classrooms were outside. The child 2 care typically utilizes Door C when entering the building, however, they came in Door A to allow several children to use the bathroom before returning to the child care center. The child care center is located on the campus of Brightmoor Christian Church along with Novi Christian Academy. As the staff members were allowing children to use the restroom, several parents were arriving to pick up their children. When the children arrived at the child care space, Child A’s Father was waiting to pick him up. CCSM 1 and CCSM 2 realized that Child A was still in the restroom, and CCSM 2 went back to the restroom, where Child A was playing in the water. Ms. Carter shared that Child A’s Father was very concerned that Child A was left unsupervised and wanted to know what would be done to prevent another incident from occurring. Ms. Carter stated she informed Child A’s Father that she would speak to the staff regarding the importance of counting children and utilizing face to name sheets. Ms. Carter shared that while she apologized for the incident, Child A’s Father was very calm, and Child A has remained in the child care center. Ms. Carter stated that she spoke with CCSM 1 and CCSM 2 regarding the incident. Ms. Carter reiterated the importance of always counting children before leaving an area and again when returning with face to name counts. CCSM 1 and CCSM 2 shared that Child A is very active and can be difficult to redirect or focus. Ms. Carter suggested that at least one staff member holds Child A’s hands when in transition. Ms. Carter shared that CCSM 2 has never had any disciplinary actions in the past and this is CCSM 2’s first year as a lead caregiver. Ms. Carter stated CCSM 2 apologized for the incident and reassured her that it would never happen again. On 09/13/2023, I interviewed CCSM 1. CCSM 1 stated that Child A was very active the day of the incident, consistently running away from staff members when they called his name and not listening very well. CCSM 1 stated that it was nearing the end of the day, which is when they combine classrooms as attendance begins to drop. CCSM 1 shared that she and CCSM 2 were outside with approximately 10-12 children, when one child had to use the restroom. Because they had been outside for a while and parents were arriving, they decided it was time to go in. CCSM 1 shared that while they were trying to line children up to come inside the building, Child A kept running off, which prompted CCSM 2 to chase him quite a bit. Once they were inside the building, Child A began running up and down the hallway and then asked to use the bathroom. The boys’ and girls’ bathrooms are next to each other, and children are typically sent into the restrooms while staff members and the other children are outside in the hallway. CCSM 1 took the child who initially stated she had to use the restroom inside and stayed in the bathroom to assist her as she was one of the smaller children who needed assistance.
