    On 03/29/2021, I spoke with Child A's mother regarding the concerns reported to Ms. McCausland. She shared her child was enrolled at the facility for approximately 1 month prior to her recent disenrollment. There were two incidents that concerned her. The first incident happened a week before she decided to disenroll her child. During pick up she witnessed her child hit another child in the face and the other child was crying. She didn't like that no CCSM's tended to the child who was hit in the face and crying. When her child was brought to the main entry, she brought her concern to Ms. Nelson's attention. The second incident occurred while watching the cameras during pick up as well. She said she observed a CCSM Ms. Ritu Singh kneeling down, and Ms. Singh grabbed a child by the upper arm in an aggressive manner pulling him down. She shared that Ms. Nelson was working the front desk and was the staff who went to the classroom to bring her child to her at pick-up-time. On Monday 03/15/2021, she spoke with Ms. McCausland over the telephone. She expressed her concerns with Ms. McCausland, and Ms. McCausland told her that there would be an investigation and she would turn the information in to upper management. Child A's mother followed up by calling customer service and spoke with district leader, Ms. Cheryl Tanasoff. Ms. Tanasoff informed Child A's mother that Ms. McCausland had not made her aware of the concerns. As a result of both incidents and her belief that Ms. McCausland was not taking the incidents seriously, she did not feel comfortable with continuation of care for her child. On 04/06/2021, I conducted an unannounced onsite inspection with program director, Lauren McCausland. Ms. McCausland was aware of the allegations as they were self- reported. She shared that an internal investigation was conducted and there were no concerns noted by other CCSM's that work with Ms. Singh. Cameras are in the classrooms and in the main entryway for viewing, however, all cameras are only live feed with no recording. In addition, she denied that she has personally observed or had prior complaints made against Ms. Singh. There has been no disciplinary action taken against her for her temperament or inappropriate discipline towards children. Ms. McCausland spoke with Child A's mother who expressed concerns and informed her that an internal investigation would be conducted. She asked Child A's mother if there was a reason that she didn't bring to her attention the incident that occurred during pick up since she was the one who went to the classroom to get her child. She told her due to other parents being present she didn't want to bring it up at that time. On 04/06/2021, I interviewed Ms. Devyn Nelson. Ms. Nelson shared that during the first incident expressed by Child A's mother, she was immediately notified by the parent after 3 she returned to the foyer area to take the child to his mother for pickup. This is when Child A's mother told her that a child was crying because her child hit the child in the face. She was upset because she did not observe the crying child being comforted. The only thing she saw was the toy being taken away from her child. Ms. Nelson stated that she went into the classroom following the incident, and the child that Child A's mother said was crying was tear-free, did not appear to have been tearful, and both CCSM's were tending to the needs of the children and actively engaged. Regarding the second incident, she spoke with Child A's mother the following day. Child A's mother voiced concerns regarding Ms. Singh. She shared that she observed her pull a child down by the arm and she did not like it. As a result, she wanted to put in her notice that her child would not be completing his last of her two-week disenrollment notice. Ms. Nelson shared the facility completed an internal investigation which consisted of speaking with Ms. Singh and other CCSM's, and there were no findings to support concerns of Ms. Singh
