I spoke with licensing consultant Kortney Williams on 3/8/2022. Ms. Williams is the child care licensing consultant that is assigned to Ms. Dorman's child care. I asked Ms. Williams if she has ever had a concern with an overwhelming smell of bleach. Ms. Williams stated that she has never had any concerns with the smell of bleach at the child care. She also indicated that she has never seen bleach in access to the children in care. Ms. Williams indicated that the child care is always very clean and well cared for. I completed an onsite inspection at the child care on 3/15/2022. During my inspection, I was able to observe the child care and did not have any concerns with a bleach smell. I also did not observe bleach in reach of children in care. I asked Ms. Dorman about her sanitization process for the diaper changing area. She indicated that she sanitizes the changing pad between each child. Ms. Dorman uses Clorox clean up sanitizing spray to sanitize the changing pad. During the first day of using, it she did note that it even after allowing it to dry in between children it left bleach marks on the pants of Child A. She stated that as soon as she noticed it, she started wiping it down with water after spraying the sanitizing spray. She offered to replace 2 the pants that Child A was wearing due to it leaving a small bleach mark, but Child A's Mother indicated it was not necessary. I spoke with Child B's Mother on 4/29/2022 in regard to the care Child B (male, age 4) is provided at Ms. Dorman's home. Child B's mother stated that Child B loves goes to child care and when he is picked up, he always says that he has fun. She stated that she has never had a concern of the child care smelling like bleach during drop off and pick up. Child B has never had bleach marks on his clothes from child care. I left messages with Child C's Mother and Child D's Mother on 4/29/2022, at the time of this report writing the phone calls have not been returned. I reviewed Ms. Dorman's child care licensing reports for the last two years, she has not been found in violation of any hazardous materials in reach of children.

Ms. Dorman indicated that she uses an app to record infant's daily records. She stated that at times she has had made the entry in the app without changing the time so it looks like Child A would eat two bottles right in a row. Ms. Dorman stated that she had several conversations with Child A's mother about their feeding amount and schedule. Ms. Dorman stated that she followed the instructions given. 3 Child B's Mother stated that Child B is a very picky eater and she has instructed Ms. Dorman that if he does not eat what is provided at the child care not to force him to eat it. Child B never complains about being fed too much or too less. Sometimes he will be offered a different snack because he does not like what is being served but if not, he just eats when he gets home. Child B's mother stated that she loves the care at Ms. Dorman's home and has no concerns.

MCL 722.115m Child care center, group child care home, or family child care home; requirements for licensure. (2) A person, partnership, firm, corporation, association, nongovernmental organization, or governmental organization shall not establish or maintain a child care center, group child care home, or family child care home unless licensed by the department. Application for a child care center, group child care home, or family child care home license shall be made on forms provided, and in the manner prescribed, by the department, including the fees required under subsection (10). Before issuing or renewing a child care center, group child care home, or family child care home license, the department shall investigate the applicant's activities and proposed standards of care and shall make an on-site visit of the proposed or established child care center, group child care home, or family child care home. Except as otherwise provided in this subsection and sections 5q and 5r, if the department is satisfied as to the need for a child care center, group child care home, or family child care home, as to its financial stability, and that the service, facility, applicant, licensee, child care staff member, or member of the household is conducive to the welfare of the children, the department shall issue or renew the child care center, group child care home, or family child care home license. If the department determines that a service, facility, applicant, licensee, child care staff member, or member of the household is not conducive to the welfare of the children, the department shall deny that application or revoke that licensee's license according to section 11.
On 09/16/2021, I received a complaint alleging that Ms. Dorman's live-in boyfriend was in prison for three charges of domestic violence and should not be around children. Ms. Dorman confirmed that she has been dating Mr. Randy Suzor since July or August 2021. Ms. Dorman denied that Mr. Suzor lives with her but that he stays at her home on weekends and sometimes during the week. She denied that he is ever present during child care hours or while child care children are present. She reported that he has criminal history from 2012 that may preclude him from being found eligible to be an adult household member but stressed that they are not ready to move in together. Mr. Randy Suzor confirmed that he does not live with Ms. Dorman. He sometimes comes over after he is out of work at 5:30 P.M. on weeknights and sometimes spends the night. He also spends the weekends at Ms. Dorman's home. He identified that there have been days where child care children are arriving as he is leaving the home but stated that he has only been around child care children twice. He denied that he has ever provided care for child care children. Child A's Mother confirmed that Child A has been attending child care Monday through Friday from 5:45 A.M. — 3:30 P.M. for over four years. Things are going well and they really like it. Child A is the first child at the facility in the morning. Child A's Mother reported that in the last week or so a man has been leaving the home as she is dropping Child A off. She denied that she has seen him at pick up and denied that she has asked Ms. Dorman who he is. Child B has been attending the child care Monday through Thursday from 7:30 A.M. — 5:15 P.M. for approximately 3 years. Child B's Mother reported that she just found out that Ms. Dorman has a boyfriend and has only seen him one time. During the exit conference, Ms. Dorman again stated that Mr. Suzor does not live in her home and stated that she should be allowed to have him there when child care children are not present. In a follow-up email to Ms. Dorman's concerns, child care licensing manager Ms. Candice Case-French provided Ms. Dorman with a copy of the section of the law, Public Act 116, which states "Member of the household" means any individual who resides in a family child care home, group child care home, foster family home, or foster family group home on an ongoing basis, or who has a recurrent presence in the home, including, but not limited to, overnight stays. ..." During a follow up telephone call, Ms. Dorman agreed that Mr. Suzor would not be allowed in the home, regardless of whether child care children are present. 3

(1) A licensee shall be responsible for all of the following: (g) Report to the department within, 3 business days, any changes in the household composition or when any new or existing member of the household or child care personnel has any of the following: (i) An arrest. (ii) A conviction. (iii) An arraignment for an offense that if convicted would lead to that individual's ineligibility to be connected with a child care home. (iv) Is being investigated by the state department of health and human services for an allegation of child abuse or neglect. (v) Is under court supervised parole or probation. (vi) Has been admitted to, or released from, a correctional facility. (vii) Has been admitted to or released from a health facility or agency that was providing mental health or substance use disorder treatment services to the individual.
Licensee Robin Dorman failed to report to the department within 3 business days any changes in the household composition. Randy Suzor has been staying at Ms. Dorman's home on an on- going basis including overnight stays. Ms. Dorman did not report to the department Mr. Suzor's presence in the licensed child care home.
